Mar 25th 2024, 15:11

The United States is the only developed nation in the world without guaranteed universal healthcare. “We must keep making the point that profit-driven healthcare is not only worse for patients—it’s a national embarrassment,” Kemp added. “Our leaders must act to kick insurance companies to the curb and enact Medicare for All now”…. While opponents—including U.S. lawmakers who take substantial donations from the for-profit healthcare and insurance industry—often argue that Medicare for All would be too expensive, a 2020 Congressional Budget Office analysis found that such a program would save between $300 billion and $650 billion annually.

Mar 25th 2024, 14:43

Abstract
There is a notable lack of literature evaluating peer support for people with a diagnosis of borderline personality disorder (BPD). Peer-led groups have been shown to positively impact outcomes related to empowerment, hope, and self-efficacy beliefs for consumers diagnosed with serious mental illness and may also be a helpful option for those experiencing personality disorder symptoms. We outline the coproduction of a peer group for people with a lived experience of BPD and a qualitative analysis of feedback post-group participation. Twenty-two consumers participated in post-group interviews and described their experiences of peer support. Interview transcripts were coded, and three main themes were identified: growth and change, connection and feeling understood, and creating safety. The findings suggested that participation in a coproduced peer support group for people with a diagnosis of BPD facilitated positive personal transformation as well as opportunities to develop connection through shared experiences in a safe environment grounded in mutuality. These themes were particularly powerful for participants because they countered previous experiences of not feeling safe, understood, or equal when seeking treatment. These findings support the acceptability of implementing a coproduced group for people diagnosed with BPD in a community mental health setting.

(https://ifp.nyu.edu/2024/meta-analyses-systematic-reviews/comparison_of_bias_adjustment_in_meta_analysis-282-aspx/) Comparison of bias adjustment in meta-analysis using data-based and opinion-based methods
Mar 25th 2024, 13:06

Introduction:
Several methods exist for bias adjustment of meta-analysis results, but there has been no comprehensive comparison with non-adjusted methods. We compare 6 bias-adjustment methods with 2 non-adjusted methods to examine how these different methods perform.
Methods:
We re-analyzed a meta-analysis that included 10 randomized controlled trials. Two data-based methods: i) Welton’s data-based approach (DB) and ii) Doi’s quality effects model (QE) and 4 opinion-informed methods: i) opinion-based approach (OB), ii) opinion-based distributions combined statistically with data-based distributions (O+DB), iii) numerical opinions informed by data-based distributions (OID [num]), and iv) opinions obtained by selecting areas from data-based distributions (OID [select]) were used to incorporate methodological quality information into the meta-analytical estimates. The results of these 6 methods were compared with 2 unadjusted models: i) the DerSimonian-Laird random effects model and ii) Doi’s inverse variance heterogeneity (IVhet) model.
Results:
The 4 opinion-based methods returned the random effects model estimates with wider uncertainty. The DB and QE methods returned different results and aligned with the IVhet method with some minor downward bias adjustment.
Conclusion:
Opinion-based methods seem to just add uncertainty rather than bias adjust.

Mar 25th 2024, 09:41

What is wrong with you?” is a rich and open question. When it comes from other people, it is “aggressive” or “rude,” but when you pose it to yourself, it is endlessly fascinating. What is wrong with you? For better or for worse, a buffet of terminology has emerged that might help explain your terminal unease. Maybe you are struggling with boundaries or your social circle is overrun with toxic people. Maybe your mother or roommate or ex-boyfriend is a narcissist. Maybe you are traumatized, which is perhaps one reason you are also anxiously attached. The problem with these words, of course, is that their impact fades with use, leaving us to hunt for new language that can justify our suffering. By early 2024, we seem to have landed on our latest favorite diagnosis: Have you considered the possibility that you might be dysregulated?

(https://ifp.nyu.edu/2024/journal-article-abstracts/jomf-12971/) Theorizing White heteropatriarchal supremacy, marriage fundamentalism, and the mechanisms that maintain family inequality
Mar 25th 2024, 03:22

Abstract
In this article, I draw upon critical feminist and intersectional frameworks to delineate an overarching orientation to structural oppression and unequal power relations that advantages White heteropatriarchal nuclear families (WHNFs) and marginalizes others as a function of family structure and relationship status. Specifically, I theorize that marriage fundamentalism, like structural racism, is a key structuring element of White heteropatriarchal supremacy. Marriage fundamentalism can be understood as an ideological and cultural phenomenon, where adherents espouse the superiority of the two-parent married family. But it is also a hidden or unacknowledged structural mechanism of White heteropatriarchal family supremacy that is essential to the reproduction and maintenance of family inequality in the United States. Through several examples, I demonstrate how—since colonization—marriage fundamentalism has been instantiated through laws, policies, and practices to unduly advantage WHNFs while simultaneously marginalizing Black, Indigenous, immigrant, mother-headed, and lesbian, gay, bisexual, transgender, queer/questioning (LGBTQ+) families, among others. I conclude with a call for family scientists to further interrogate how marriage fundamentalism reproduces family inequality in American family life and to work toward its dismantling. A deeper understanding of how these complex and often covert mechanisms of structural oppression operate in family life is needed to disrupt these mechanisms and advance family equality and justice.

(https://ifp.nyu.edu/2024/journal-article-abstracts/sextrans-2023-055964v1/) Association of same-sex criminalisation laws and national HIV policies with HIV testing in African MSM: an ecological single-level and multilevel cross-sectional study of sub-Saharan African countries
Mar 25th 2024, 02:49

Background
HIV incidence among men who have sex with men (MSM) in sub-Saharan Africa (SSA) remains high compared with the general population. Many countries in the region still criminalise consensual homosexual relationships, and some are yet to adopt WHO-recommended interventions for MSM into national HIV policies. This study examines how HIV testing of adult MSM in SSA varies according to the legal climate and presence of targeted HIV policy using data from the cross-sectional 2019 Global LGBTI Internet Survey study.
Methods
Using data from 3191 MSM in 44 SSA countries, we assessed associations of legal climate and HIV policy with ever and recent HIV testing using linear ecological and logistic multilevel analyses. From the single-level analysis, we can compare our findings to previously reported data, then, extending to a two-level multilevel analysis, we account for the hierarchical structure of the population and simultaneously adjust for differences in context and composition in each country. We then test the sensitivity of our analyses to excluding countries from the model.
Results
We find evidence that legalised same-sex relationships were associated with increased odds of ever testing (OR=2.00, 95% CI 1.04, 3.82) in multilevel analyses. We also find evidence of an association of targeted HIV policies with increased odds of ever testing (OR=2.49, 95% CI 1.12, 5.52). We did not find evidence of an association of the legal climate (OR=1.01, 95% CI 0.69, 1.46) and targeted HIV policies (OR=1.26, 95% CI 0.78, 2.04) with recent testing.
Conclusions
This study suggests elimination of discriminatory laws and policies might be important for increasing HIV status awareness of MSM, an important first step in epidemic control. Additionally, we highlight heterogeneity between South Africa and other SSA countries, which has implications for studying SSA countries as a homogeneous group.
