With advances in medical care, more youth with intellectual and/or developmental disabilities (IDD) are transitioning into adulthood. Patient- and family-centered, integrated care is warranted around this time of transition. Support teams (including the youth, caregivers, teachers, and pediatricians) should engage in transition planning, ideally starting between 12 and 14 years of age, to identify and develop resources to support the maturing youth’s capacity for independent decision-making. Care teams should consider the varied levels of alternative decision-making support, which may include supported decision-making, medical proxy decision-making, power of attorney, and/or establishment of legal guardianship arrangements, to support the youth’s health and well-being optimally. Ultimately, if independent decision-making is not appropriate, the goal for youth with IDD should be the least restrictive alternative, while preserving human rights and human dignity and promoting their autonomy. These considerations review alternative decision-making support, concepts, and legal requirements available for youth with IDD and their care teams. Pediatricians can support youth with IDD and their families in the transition process and decision-making autonomy by actively engaging the youth in care decisions, supporting needs for augmentative communication, fostering their expression of preferences and understanding of care decisions, and linking them to resources such as the medical-legal partnership model.

Postpartum depression (PPD) is a major public health concern, yet we lack tools to predict PPD during pregnancy. We found that lower sensorimotor gating, as measured by prepulse inhibition (PPI) of the startle response, can predict risk of PPD in women who were not depressed during their pregnancy.

Abstract

Aim
This study aimed to explore the ‘real time’ expectations, experiences and needs of men who attend maternity services to inform the development of strategies to enhance men’s inclusion.

Methods
A qualitative descriptive design was adopted for the study. Semi-structured face-to-face or telephone interviews were conducted with 48 men attending the Royal Brisbane and Women’s Hospital before and after their partner gave birth. Data were coded and analysed thematically.

Results
Most respondents identified their role as a support person rather than a direct beneficiary of maternity services. They expressed the view that if their partner and baby’s needs were met, their needs were met. Factors that contributed to a positive experience included the responsiveness of staff and meeting information needs. Factors promoting feelings of inclusion were being directly addressed by staff, having the opportunity to ask questions, and performing practical tasks associated with the birth.

Conclusion
Adopting an inclusive communication style promotes men’s feelings of inclusion in maternity services. However, the participants’ tendency to conflate their needs with those of their partner suggests the ongoing salience of traditional gender role beliefs, which view childbirth primarily as the domain of women.

Abstract
Although unemployment plays a central role in the emergence of labour market disparities, rarely any studies have linked its dynamics with the overall level of wage inequality. This study addresses this gap by assessing how unemployment is associated with both between- and within-group wage disparities. Hypotheses derived from labour market theories are tested using comprehensive longitudinal data from the Dutch OSA Labour Supply Panel spanning 1985–2008. Results from newly developed variance function panel regression models demonstrate that wages of previously unemployed workers are both lower and more dispersed compared to the wages of continuously employed workers. We also found that wage disparities were more pronounced among previously unemployed men than women. The results also demonstrate the usefulness of the variance function panel regression as a method for analyzing sources that drive wage inequality within persons and over time.
