ABSTRACT

People often pay little attention to the impact of their charitable donations. Recent research has attempted to promote more effective giving by providing information about donation effectiveness. However, how people judge effectiveness remains unclear. This study examined whether high-empathy conservation targets are perceived as more effective donation recipients. In two experiments, participants viewed advertisements for endangered species that varied in their empathic appeal. Study 1 (N = 200) revealed that empathy toward the target was linked to perceived donation effectiveness. Moreover, this empathy–effectiveness link persisted even when participants were encouraged to deliberate, as confirmed in Study 2 (N = 400). These findings suggest that people often base their judgments of effectiveness on how much empathy they feel toward the target, emphasizing the importance of interventions that foster empathy when promoting effective giving.

ABSTRACT

Objective

To estimate the effect of two provider supply components—(1) clinic time from providers and (2) the number of new patient appointments provided per day—on the wait times for new patient appointments in specialty care.

Study Setting and Design

In this observational analysis, we examine wait times in the largest public health care delivery system in the US—the Veterans Health Administration (VHA). We introduce new data on wait times for specialty appointments at the VHA. We estimate the impact of the two supply components on new patient wait times for specialty care appointments, while controlling for patient characteristics, local area characteristics, medical center fixed effects, and national changes over time and across seasons. We use an instrumental variables approach to handle endogeneity concerns.

Data Sources and Analytic Sample

Secondary administrative data from the VHA were analyzed, focusing on four specialties: cardiology, orthopedics, urology, and gastroenterology. The sample includes 126 medical centers with the specialty departments observed at 2-week intervals spanning July 2014 to December 2019.

Principal Findings

Our estimates indicate that a 10% increase in the sample average clinician time from specialists (equivalent to 2.5 full-time clinicians per 100 enrollees) would reduce the average wait time for a new patient appointment by 1.1 days [95% CI: 1.08–1.13] or 3.9% of the sample average wait time. A 10% increase in the average number of new patients seen per clinic day by providers (0.37 visits) would reduce the average wait time by 4.0 days [95% CI: 3.97–4.03] or 12.9% of the sample average.

Conclusions

Increasing clinic time and the new patient visit rate among providers can improve access to care for new patients. Scheduling policies that prioritize new patients can affect this visit rate without substantially reducing access for other patients.

ABSTRACT

This article examines the trend in fathers’ uptake of parental leave following the 2004 parental leave reform in Estonia and analyses the factors that determine whether fathers take leave. The reform introduced generous earnings-related parental leave benefits paid over a long period but did not implement a fathers’ quota. The study population includes children born between 2004 and 2018. We use a register-based dataset and logistic regression to analyse variation in the probability that fathers take parental leave. The main explanatory variables are fathers’ earnings and educational attainment. Following the reform, fathers’ uptake of parental leave more than tripled over the study period. We find a strong positive association between fathers’ earnings and their use of parental leave, likely due to the full replacement of previous earnings and the high ceiling on parental leave benefits. Incorporating an interaction between parents’ income levels into the regression analysis suggests that economic optimisation plays an important role in fathers’ decisions to take leave, while relative resources with the couple also matter. This study expands the geographical scope of research into the determinants of fathers’ parental leave use. We find both similarities as well as differences in fathers’ leave uptake compared to Nordic countries.

Abstract

Objectives

Early intervention in psychosis (EIP) services provide care for individuals experiencing first-episode psychosis and are considered both clinically and cost-effective. While many have positive experiences of EIP, there have also been reports of perceived harm. This scoping review synthesized qualitative evidence on service user perspectives of potential harm in EIP services in the UK, to consider how services may continue to improve practice to meet the needs of their service users.

Methods

We conducted a scoping review following a pre-registered protocol. Systematic searches were performed in PsycINFO, Web of Science Core Collection, Scopus, Embase, CINAHL Plus and Medline on 9th October 2024, and updated searches on 12th January 2026. We included peer-reviewed qualitative studies exploring service user experiences in EIP services in the UK. Data on reported harms from eligible studies were thematically analysed.

Results

Thirteen studies met inclusion criteria. We identified four main themes: (1) Intersection of stigma and lack of cultural awareness; (2) Inappropriate or inadequate support; (3) Poor relationships and communication; and (4) Challenges in the discharge process.

Conclusions

While EIP services offer the current gold standard of care for first-episode psychosis, our review highlighted some areas where harm was perceived to have arisen. Such information presents potential targets for service improvement. Strengthening communication and enhancing information sharing may help mitigate potential harms and optimize service user experiences. Further research to quantify the prevalence of harms within EIP services is needed to inform best practices and ensure equitable, person-centred care.

ABSTRACT

Introduction and Aims

Cannabis use among university students is a growing concern, particularly in demanding medical programs. We estimated prevalence, identified predictors, and compared coping strategies among medical students in Uganda.

Design and Methods

Cross-sectional survey of 318 undergraduates at Kampala International University (Western Campus). Cannabis use and hazardous/disordered use were screened with CUDIT-R (hazardous 8–11; probable use disorder ≥12, DSM-5-TR aligned). Coping was measured with the Brief COPE. Predictors were assessed using logistic regression; coping differences with the Wilcoxon rank-sum test.

Results

Cannabis use prevalence was 30.8% (n = 98); 7.6% met criteria for hazardous use and 9.4% for probable cannabis use disorder. Independent predictors of use were being separated (AOR = 12.00), being single (AOR = 3.45), Catholic faith (AOR = 2.76), and longer time at campus (AOR = 1.16 per year). Users reported higher emotion-focused and avoidant coping; problem-focused coping did not differ.

Discussion and Conclusions

Cannabis use among Ugandan medical students is common and associated with relationship status, religion, and time at campus. Coping profiles suggest greater reliance on maladaptive strategies among users. Findings support campus policies and mental-health programs that integrate substance-use screening and strengthen adaptive coping skills.

ABSTRACT

Background and Aims

Delayed initiation of breastfeeding is associated with poor child development outcomes and increased risk of infant mortality, which remains a major public health challenge in low- and middle-income countries. Despite global and national efforts to promote optimal breastfeeding, early initiation remains suboptimal in much of Sub-Saharan Africa. This study examined the prevalence and factors associated with delayed initiation of breastfeeding in Tanzania.

Methods

We analyzed secondary data from the 2022 Tanzania Demographic and Health Survey and Malaria Indicator Survey, a nationally representative cross-sectional survey. The study population comprised children under 24 months. The outcome variable was delayed breastfeeding initiation, defined as not initiating breastfeeding within 1 h of life. Weighted data were analyzed using multilevel mixed-effects logistic regression to identify associated factors.

Results

A total of 4478 children were included, with a mean (±SD) age of 11.5 ± 6.9 months. The prevalence of delayed initiation was 24.4%. Child-related factors included first (aPR = 1.32; 95% CI: 1.13—1.55) and second birth order (aPR = 1.21; 95% CI: 1.06–1.39). Maternal age 35–49 years was also associated (aPR = 1.16; 95% CI: 1.02–1.32). Health-related factors included absence of immediate skin-to-skin contact (aPR = 1.85; 95% CI: 1.64–2.09), cesarean delivery (aPR = 2.45; 95% CI: 2.17–2.78), lack of birth assistance (aPR = 1.63; 95% CI: 1.16–2.28), and assistance from non-skilled provider (aPR = 1.42; 95% CI: 1.09–1.84). Geographically, delayed initiation was more prevalent in Zanzibar (aPR = 1.62; 95% CI: 1.33–1.98) and less prevalent in the Southern zone (aPR = 0.54; 95% CI: 0.36–0.80).

Conclusion

Nearly one in four newborns in Tanzania experience delayed breastfeeding initiation. Delayed initiation was associated with child, maternal, health-related, and geographical factors. Strengthening skin-to-skin contact, skilled birth attendance, cesarean support, counseling for first-time and older mothers, and addressing geographical disparities may improve early initiation and support progress toward Sustainable Development Goal target 3.2.

ABSTRACT

Objective

Although social media use is associated with disordered eating, prior research has predominantly focused on adolescent and university student populations; evidence from adult occupational groups remains limited. The present study examined the longitudinal association between short-form video use and disordered eating among Chinese female nurses, specifically examining the mediating role of body dissatisfaction and the moderating role of sociocultural attitudes toward appearance.

Method

A two-wave longitudinal design was employed. Participants were 400 registered female nurses recruited from a hospital in Wuhan, China, at baseline (T1), with 270 retained at the four-month follow-up (T2). Measures assessed short-form video use and sociocultural attitudes at T1 and body dissatisfaction and disordered eating at T2. Structural equation modeling with bootstrapping was conducted to test mediation and moderated mediation hypotheses.

Results

Short-form video use exhibited a significant indirect effect on subsequent disordered eating through increased body dissatisfaction. This indirect pathway was moderated by sociocultural attitudes toward appearance: the association between short-form video use and body dissatisfaction was stronger among nurses with higher levels of perceived media pressure and thin-ideal internalization.

Discussion

These findings identify body dissatisfaction as a key psychological mechanism linking short-form video use to disordered eating and highlight sociocultural attitudes as critical susceptibility factors shaping vulnerability to media effects. Overall, this study extends media effects research to adult occupational populations and has important implications for prevention and intervention strategies to protect nurses’ psychological well-being in the digital age.
