ABSTRACT

Child protection (CP) systems are increasingly incorporating procedural and restorative justice principles in their practice frameworks, which are interpersonally focused and aim to respect and empower families. In addition, the importance of professional judgement in CP is increasingly being acknowledged. Yet, little research has investigated practitioners’ experiences of enacting these approaches. In this qualitative study, 15 frontline practitioners working for CP in Victoria, Australia, offered their perspectives about the feasibility of implementing restorative and procedural justice principles in practice and the role of professional judgement in risk assessments. A thematic analysis revealed a host of personal, familial, organisational and systemic facilitators and barriers to enacting evidence-informed principles in practice. The utility of professional judgement as a complement to structured risk assessments was clear. Findings demonstrated that practitioners are a committed workforce who want to implement evidence-informed practice, although several competing demands and pressures can impede their ability to enact these crucial principles in practice.

ABSTRACT

Aims

Detection and intervention of psychosis-risk states are typically delivered through clinical high risk for psychosis (CHR) clinics grounded in CHR research frameworks. However, real-world implementation is limited by major challenges, including clinical efficiency, comorbidity, and transdiagnostic presentations. At McLean Hospital, we launched the Outreach and Enhanced Referral Support project to address these challenges and extend early detection and intervention beyond traditional CHR clinic settings. Here we describe the project and report interim findings.

Methods

This pilot project comprises three phases: needs assessment, outreach training, and enhanced referral support. Surveys assessed feasibility, acceptability, and selected clinical outcomes.

Results

Ten referral support cases were completed between July 2024 and April 2025. Feasibility ratings ranged 80%–100%, and acceptability was 91.7%. The mean wait time for support services was 3 days (95% CI 2.11–3.89, SD = 1.25; range, 0–7).

Conclusions

Preliminary findings indicate that the Outreach and Enhanced Referral Support model is feasible and acceptable in a large psychiatric hospital setting, warranting larger-scale evaluation of its clinical impact.

ABSTRACT

Objective

To estimate the effect of two provider supply components—(1) clinic time from providers and (2) the number of new patient appointments provided per day—on the wait times for new patient appointments in specialty care.

Study Setting and Design

In this observational analysis, we examine wait times in the largest public health care delivery system in the US—the Veterans Health Administration (VHA). We introduce new data on wait times for specialty appointments at the VHA. We estimate the impact of the two supply components on new patient wait times for specialty care appointments, while controlling for patient characteristics, local area characteristics, medical center fixed effects, and national changes over time and across seasons. We use an instrumental variables approach to handle endogeneity concerns.

Data Sources and Analytic Sample

Secondary administrative data from the VHA were analyzed, focusing on four specialties: cardiology, orthopedics, urology, and gastroenterology. The sample includes 126 medical centers with the specialty departments observed at 2-week intervals spanning July 2014 to December 2019.

Principal Findings

Our estimates indicate that a 10% increase in the sample average clinician time from specialists (equivalent to 2.5 full-time clinicians per 100 enrollees) would reduce the average wait time for a new patient appointment by 1.1 days [95% CI: 1.08–1.13] or 3.9% of the sample average wait time. A 10% increase in the average number of new patients seen per clinic day by providers (0.37 visits) would reduce the average wait time by 4.0 days [95% CI: 3.97–4.03] or 12.9% of the sample average.

Conclusions

Increasing clinic time and the new patient visit rate among providers can improve access to care for new patients. Scheduling policies that prioritize new patients can affect this visit rate without substantially reducing access for other patients.

ABSTRACT

Introduction

Self-doubt is a pervasive yet often unspoken experience among mental health clinicians, manifesting as moments of uncertainty in which they question their ability to make sound clinical judgements, support therapeutic progress, or meet professional expectations. This study aimed to deepen understanding of self-doubt as a complex, nuanced, and meaningful aspect of clinical practice, exploring how it shapes clinicians’ professional experience, decision-making, and personal growth.

Methods

Using descriptive phenomenology, the lived experiences of self-doubt among 12 Filipino licensed psychologists were examined. Data were collected through in-depth, semi-structured interviews and analysed using thematic analysis to capture the nuanced and contextualised experiences of participants.

Results

Analysis revealed five core themes: the Inner Critic, the Contextual Triggers, the Demanding Pressure, the Double-Edged Sword, and the Reflective Space. Findings indicate that self-doubt is not inherently detrimental; it can function as a regulatory mechanism that fosters humility, ethical sensitivity, and heightened self-awareness when approached with reflection, supervision, and self-compassion.

Conclusions

These findings suggest the potential value of creating supervisory, organisational, and peer contexts that acknowledge, normalise, and provide structured opportunities to reflect on experiences of professional self-doubt. While the present study draws on a small, context-specific sample, the phenomenologically grounded insights it offers may be relevant to considerations in clinical training, supervision, and organisational support systems.

Abstract

The school achievement and career aspirations of 23 sixth form students at a multi-cultural urban academy in the UK are explored through interviews. The sample includes 16 s-generation migrants, 6 UK-born students with migrant parents and 1 UK-born student, selected to represent a cohort of over 300 post-16 learners. The aim is to better understand the educational success and upward mobility of G2 migrants, who are reported to outperform their native-born peers, particularly in advanced economies. Using data drawn from a wider study, this paper finds that the participants’ socio-economic backgrounds, family values and schooling have in different ways contributed powerfully to their progress in a new country. The achievements of all 23 participants in public examinations and in securing university places confirm the idea that migrants, especially G2 migrants, do well in education despite many hindrances. Participants seem set for professional careers and upward mobility in relation to their parents. There are also signs the labour market is more open for recent migrants, despite persisting inequalities. The news for native students seems to be less encouraging, with growing inequality more powerful than school reform. The policy lesson is that there can be little progress with social mobility policy without serious efforts to close the gaps in society that hinder so many young people.

Abstract

The study examines the relationship between the innovative use of learning environments and student achievement by integrating data from the Trends in Mathematics and Science Study (TIMSS) and the OECD School User Survey (SUS) conducted in Russia. The research utilizes self-assessment questionnaires completed by students, teachers, and school leaders to analyze how classroom design, spatial flexibility and school climate interact with teaching methods and influence learning outcomes in mathematics and science. In addition, qualitative case evidence illustrates how teachers adapt and reconfigure spaces, demonstrating their agency in shaping learning environments. Findings indicate that (i) the learning environments in terms of physical and non-physical characteristics explain up to ¾ a year of learning effect in TIMSS 2019, (ii) despite growing interest in flexible and modern classroom designs, traditional classroom setups with fixed seating and teacher-centred instruction remain dominant in most schools and (iii) the effect of the modern teaching approaches by teachers, who utilize diverse teaching styles and apply spatial layout changes during teaching, reaches up to one and a half years of learning in TIMSS 2019. The actual implementation of flexible layouts and innovative teaching practices is still limited. However, students who experience more innovative teaching methods—such as collaborative group work, project-based learning, and team teaching—tend to have higher learning gains, and the effect is consistent regardless of socioeconomic backgrounds. The study concludes that modernizing school infrastructure and supporting teachers in using these spaces and applying innovative pedagogy are essential for improving educational outcomes. It emphasizes the need to prioritize these efforts in disadvantaged communities to ensure equitable access to quality education and to support the holistic development of all students.

Context and implications

Rationale for this study: While the share of infrastructure and equipment for schools is significant in overall investments in education worldwide, there is still limited robust evidence on how physical learning environments and teaching practices may influence student achievement.

Why the new findings matter: This study quantifies the impact of both physical environments and modern pedagogy on learning outcomes, demonstrating measurable gains even after accounting for students’ socioeconomic status.

The study has the following implications:

For governments and policymakers: The findings show that upgrading school infrastructure and supporting innovative teaching can lead to learning gains of up to one and a half years of schooling. This highlights the importance of modernizing facilities and investing in teacher professional development.

For researchers: The study demonstrates the value of combining international assessment data with user surveys to guide evidence-based investments in education. It lays the ground for future research expanding such data series and involving more countries.

For all stakeholders: Prioritizing these improvements in disadvantaged communities is crucial to ensure equitable access to quality education and to support the holistic development of all students.

Background

Omega-3, a long-chain fatty acid critical for brain structure and function, has been argued to be effective in reducing antisocial behavior, but findings are variable. This meta-analysis examines 25 randomized controlled trials (RCTs) with a total of 2,889 participants.

Methods

This study was registered with PROSPERO (CRD42021256959). Two separate analyses were conducted where the unit of analysis was independent studies and independent laboratories.

Results

Significant effect sizes (p < .001) were observed for both analyses (g = .13, .14 respectively) and in the direction of omega-3 supplementation reducing antisocial behavior. There was equivocal evidence of publication bias. Study quality was good. Stratified analyses were nonsignificant for age, gender, dose, sample size, and study quality, although the effect size was larger for unmedicated participants. Combining these data with a recent meta-analysis on aggressive behavior produced an overall effect size of g = .22 for externalizing behavior based on 36 nonoverlapping RCTs.

Conclusions

Given that omega-3 supplementation is relatively cheap, easy to implement, transfers well from the research environment to community settings, and has additional health benefits, it is suggested that it may be considered as an adjunctive intervention for the treatment of antisocial and externalizing behavior.

ABSTRACT

Several studies suggest that parent–child relationship quality interacts with child fearful temperament in predicting early stages of moral development. However, less is known about these interactive effects in relation to developmentally advanced moral constructs that are salient in adolescence, such as moral reasoning. This longitudinal study examined the prospective association between mother–child attachment security in early childhood and moral reasoning in adolescence, and whether this association was moderated by child temperamental fearfulness. The sample consisted of 93 Canadian mother–child dyads. When children were aged 15 and 25 months, mother–child attachment security was assessed using an observational measure at home. Mothers reported on children’s temperamental fearfulness when they were aged 3 years. When participants were aged 16 years, their moral reasoning was assessed using a visual task presenting everyday socio-moral conflicts. A moderation analysis conducted in a structural equation modeling framework suggested that mother–child attachment security predicted more mature moral reasoning. Furthermore, a significant interaction was found, revealing that the association between mother–child attachment security and adolescent moral reasoning was positive and significant at low and mean levels of child fearfulness, but not at high levels. Consistent with a dual-risk pattern, the adolescents who showed the least mature moral reasoning were those who, as toddlers, had the most insecure attachments to their mother and the least fearful temperaments. These findings highlight that secure mother–child attachment in toddlerhood has implications for moral reasoning into adolescence and in particular, could protect temperamentally fearless children against poor moral development.
