<p><h2>ABSTRACT</h2>
<h2>Objective</h2>
<p>To establish methods for comparing Department of Veterans Affairs (VA)-direct and VA-purchased “community care” (CC) for methadone medication for opioid use disorder (M-MOUD), and determine differences in access, quality, and cost.</p>
<h2>Study Setting and Design</h2>
<p>Three outcome measures were constructed: wait times (from clinician referral to appointment) for access, retention on M-MOUD (number of months, from one to six) for quality, and 6-month treatment costs (M-MOUD and related services). We used generalized linear models to estimate differences in each outcome for CC versus VA.</p>
<h2>Data Sources and Analytic Sample</h2>
<p>VA electronic health record and CC claims. Our study included VA patients referred by clinicians for M-MOUD between April 1, 2023–March 31, 2024 in VA (<i>n</i> = 389) and CC (<i>n</i> = 219) and, secondarily, VA walk-ins (<i>n</i> = 1830).</p>
<h2>Principal Findings</h2>
<p>Average unadjusted wait times from clinician referral to appointment were 15.73 days (standard deviation [SD] = 16.27) in VA and 19.03 days (SD = 19.12) in CC, while there was no wait time for Veterans seen as VA walk-ins. Average unadjusted M-MOUD retention was 2.07 months (SD = 1.65) in VA and 3.13 months (SD = 1.84) in CC. Average unadjusted 6-month costs were $7360 (SD = $9554) in VA and $4376 (SD = $2171) in CC. In adjusted models, CC had greater M-MOUD retention (1.07 months longer, <i>p</i> < 0.0001) and lower costs ($-1720, <i>p</i> < 0.05) compared to VA clinician referral; wait times did not statistically differ (<i>p</i> = 0.12). M-MOUD retention and cost patterns did not change when considering VA walk-ins.</p>
<h2>Conclusions</h2>
<p>An important option for expanding Veterans’ access to M-MOUD is through CC. In this group of Veterans receiving M-MOUD, CC retention was greater and costs were lower. However, CC lacks the walk-in option for same-day access. This signals tradeoffs to consider when assessing the balance between VA and CC and provides methods for comparing VA and CC treatment options.</p>

<p><p>This article develops a demanding, though non-exclusivist, version of aesthetic cognitivism and argues for its significance in contemporary medical education. Although the arts are increasingly used to foster empathy, reflection and professional identity formation, their educational value is often justified in instrumental terms, as an enrichment that supports competencies otherwise secured by biomedical training. Against this framing, I argue that aesthetic experience offers a distinctive mode of understanding that can make embodied, affective and existential dimensions of illness and dying experientially salient in ways that are difficult to cultivate reliably within the constraints of formal medical education.</p>
<p>Drawing on philosophical aesthetics from Plato and Aristotle to Dewey, Merleau-Ponty, and Berys Gaut, and situating the argument within medical humanities and narrative medicine, the article clarifies how aesthetic form can shape perception, attention and moral imagination without claiming epistemic exclusivity. As a case study, it examines Mike Nichols’s film <i>Wit</i>, showing how cinematic form enables learners to grasp aspects of vulnerability, dependency and end-of-life experience that resist straightforward translation into clinical description or ethical abstraction. Based on long-term pedagogical use of the film with medical students, the article concludes that arts-based engagement is best understood not as a supplementary add-on but as a distinctive contribution to professional formation that complements scientific knowledge and ethical reasoning.</p>

<p><p>This article reconstructs the historical development and evolving conceptual architecture of the medical humanities since its emergence as a university subject in the 1960s. Originating in late 1960s US ‘values programmes’, the medical humanities initially deployed critique—philosophical, theological, psychoanalytical and sociological—to interrogate medicine’s epistemic authority, ethical commitments and social power. Yet, from the 1970s onwards, critique operated in implicit conversation with systems theoretical approaches, particularly through the emergence of the biopsychosocial model and early engagements with phenomenology, cybernetics, anthropology and process philosophy. The subsequent rise of narrative methodologies in the 1980s and 1990s consolidated this synthesis by enabling scholars to conceptualise illness experience as an emergent property of complex, open biological and social systems. The article contends that renewing this synthesis is now essential for advancing the field’s transdisciplinary ambitions. Contemporary systems science—encompassing epidemiology, developmental research, social determinants of health and the ‘omics’ disciplines—provides a powerful framework for understanding how social experience becomes biologically embedded across the lifecourse. At the same time, critique remains indispensable for revealing the often-concealed values, power relations and institutional arrangements that shape health and illness. Integrating these orientations would reconnect the medical humanities with its diverse intellectual constituencies, address long-standing fragmentation and enable new engagements with topics such as childhood, inequality, embodiment and lifecourse health. The article concludes by proposing that a concise set of shared systems theoretical concepts could provide the durable conceptual infrastructure needed to sustain ambitious transdisciplinary dialogue across the field.</p>

<p><p><sec><st>Background</st></sec></p>
<p>Tobacco cessation services delivered via quitlines increase the likelihood of smoking cessation. Integrated media communication campaigns are a critical channel for promoting quitlines. This study used real-world data to explore the effect of various tobacco control advertising campaign strategies on generating quitline self-referrals, including among high prevalence communities.</p>
<p><sec><st>Methods</st></sec></p>
<p>Monthly counts of self-referrals to the Victorian quitline were examined during periods of Quit Victoria advertising campaign activity and inactivity between January 2018 and December 2023. Campaigns varied by spend, pairing of motivational plus capacity-building messages or not, and extent of repeated airings over the previous 6 years. Negative binomial regression models examined associations between different advertising campaign strategies and self-referrals, adjusted for periods surrounding tobacco tax increases, and time.</p>
<p><sec><st>Results</st></sec></p>
<p>Self-referral rates accelerated as funding increased. These accelerating effects were particularly evident among high prevalence groups. Greater increases were observed when messages with different behavioural goals (eg, motivational and capacity-building) were delivered together, as well as during advertising campaign periods with messages that had aired at least once before in the previous 6 years.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Findings reinforce the importance of well-funded advertising campaigns, especially for high prevalence groups. Effects of previously delivered campaign messages can build with repeated use and so can be effectively re-used over an extended period. Pairing creative concepts with distinct behaviour change objectives may also drive a greater number of self-referrals to quitlines.</p>

<p><h2>Abstract</h2>
<h2>Objective</h2>
<p>The human–animal bond (HAB) has been shown to provide a buffering effect for stress and adversity, particularly when individuals experience lower social support networks. This study aimed to explore the relationship between the HAB, perceived human social support and resilience by assessing whether the HAB could moderate the impact of social support as a protective factor for resilience. Additionally, whether the relationship between the HAB and human social support may be curvilinear was explored.</p>
<h2>Method</h2>
<p>A cross-sectional study of a large community sample of pet owners (<i>n</i> = 392) and non-owners (<i>n</i> = 146) provided information about their human social supports and resilience, and the strength of pet owners emotional bond to their companion animal.</p>
<h2>Results</h2>
<p>There was no difference in levels of resilience between pet owners and non-owners, but social support was positively associated with resilience for both. The HAB was not a significant moderator between levels of social support and resilience for owners. However, there was a significant curvilinear relationship between the HAB and perceived human social support.</p>
<h2>Conclusion</h2>
<p>The lack of evidence for HAB being a buffer between perceived human social support and resilience may partly be due to the curvilinear relationship between the strength of the HAB and perceived human social support. Extremely weak or strong HABs may be correlated with a reduced capacity to build resilience and process adversity. Therefore, this study highlights the complexities of the HAB and its relationship with human mental health, offering alternative considerations for future research.</p>

<p><h2>ABSTRACT</h2>
<p>This paper enacts a feminist-posthumanist reimagining of gender as ontological disturbance, using magical realism not as metaphor but as epistemological method. Rejecting representational logics and the managerial rationalities of organizational realism, we advance gender not as identity or role but as spectral interference—a transversal flicker that haunts the architectures of bureaucratic life. Drawing on Barad, Braidotti, Haraway, and Glissant, we engage magical realism as an enchanted mode of inquiry that disrupts binary logics, temporal linearity, and epistemic securitization. Through conceptual vignettes and speculative fabulations, we explore affective labor, invisible metrics, haunted infrastructures, and the spectrality of gendered power. Our argument foregrounds ontological mischief as feminist praxis: a refusal of institutional fixity and a commitment to worlding otherwise. In tracing gender’s recursive entanglement with race, class, and temporality, we call for a methodologically generative politics of fabulation that unsettles the realism of gendered social order and opens space for speculative reconfigurations of organizational life.</p>

<p><h2>ABSTRACT</h2>
<p>University rankings powerfully shape strategy and policy, yet they say little about what students pay. This paper introduces a simple, replicable benchmark that juxtaposes posted undergraduate fees with rank positions for 112 European universities listed in both ARWU and THE WUR (2018). Using a combined rank indicator and descriptive comparisons, we show a pronounced decoupling: low-fee institutions appear span the rank spectrum, whereas many high-fee providers are not clustered at the top. This pattern challenges the assumption that prestige reliably signals value and offers stakeholders a transparent way to read price alongside position. The contribution is methodological and policy-relevant: a tractable tool that applicants, institutions, and regulators can adopt to improve price transparency and temper ranking-driven governance. Although designed as an exploratory case, the approach scales readily to multiple years, programmes, and realised outcomes, paving the way for evidence-informed evaluations of value for money.</p>
