Abstract

Background and aims

Whether gambling marketing has a causal effect on harm is of regulatory interest. Direct marketing offers (emails, push notifications and text messages) are frequently received by people with active gambling accounts, but they can opt out. This study aimed to test whether opting out of direct marketing reduces betting and short-term gambling harms in a real-world gambling environment.

Design

Stratified randomised field experiment with a between-participants design, embedded in a 14-day ecological momentary assessment (EMA).

Setting

Nationwide, Australia (July–August 2023).

Participants

Participants (n = 227; 61.7% men; mean age = 45), 52.0% of whom scored in the moderate-risk or problem range on the Problem Gambling Severity Index (PGSI), were regular Australian sports and race bettors recruited from online panels who agreed in principle to opt out of receiving direct marketing from all wagering operators with whom they held accounts.

Intervention

Participants were stratified by PGSI risk category, age group and gender and then randomly allocated either to an opt-out condition (n = 96), in which they opted out of direct marketing from their wagering operators and provided proof, or to a control condition (n = 131) that continued to receive direct marketing as usual.

Measurements

Seven EMA surveys were administered every 48 hours over a 14-day period during a high-volume betting season. Outcomes were self-reported number of bets placed, gambling expenditure (in Australian dollars, AUD) and short-term gambling harms in the previous 48 hours, assessed with a 10-item adapted Short Gambling Harm Screen (SGHS).

Findings

The opt-out group placed 23% fewer bets [B = −0.11, 95% confidence interval (CI) = −0.20, −0.03, P = 0.011], spent 39% less money (B = −0.53, 95% CI = −0.84, −0.21, P = 0.001) and reported 67% fewer short-term gambling harms (B = −0.22, 95% CI = −0.36, −0.07, P = 0.004) compared with the controls.

Conclusions

Opting out of receiving direct marketing from wagering operators appears to be associated with statistically significantly fewer bets made, amount spent gambling and short-term gambling harms.

<st>Setting</st>

The study was conducted in three regions of Tanzania: Kagera, Tabora and Dar es Salaam across 543 public and private health facilities.

<st>Participants</st>

A total of 10 147 pregnant and postpartum AGYW living with HIV attending PMTCT services between 1 January 2018 and 31 December 2020 were included in this study and followed prospectively until they were censored at the last appointment date or 31 December 2023, whichever was earlier.

<st>Primary outcome measures</st>

The primary outcome was time to HIV care attrition, defined as death, discontinuation of antiretroviral treatment (ART) or loss to follow-up (LTFU). LTFU was defined as failure to attend a scheduled clinic appointment and being absent from care for ≥90 consecutive days following a missed appointment among non-transfers. Kaplan-Meier analyses were used to estimate time to first attrition. The Anderson-Gill proportional hazard model estimated the risk factors for repeated care interruptions, adjusted for baseline characteristics and stratified by ART status at PMTCT enrolment.

<st>Results</st>

A total of 3259 attrition events were observed, of which 79% occurred within the first year, with the median time to first attrition of 4 months (IQR: 1–8), 96.3% were due to LTFU. Over two-thirds of first-year attrition occurred among AGYW newly started on ART at PMTCT enrolment, who had more than twice the attrition rate of those already on ART (28.6 vs 11.2 per 100-person-years). Of AGYW lost to follow-up, 44.8% returned to care and 20.9% experienced subsequent attrition. Among AGYW new on ART, attrition was higher in those enrolled late in their third trimester (adjusted HR (aHR) 1.20; 95% CI 1.01 to 1.42) versus those in the first trimester and lower during the postpartum period (aHR 0.58; 95% CI 0.43 to 0.79). In AGYW already on ART, attrition rate was higher among adolescents 18–19 years (aHR 1.37; 95% CI 1.13 to 1.66) and those enrolled late; during the second (aHR 1.41; 95% CI 1.16 to 1.72) and third trimesters (aHR 1.57; 95% CI 1.23 to 2.00) or post partum (aHR 1.36; 95% CI 1.09 to 1.70) compared with the first trimester. AGYW with early-stage HIV, on second-line regimens and attending facilities with fewer AGYW, had a lower attrition rate in contrast to comparison groups.

<st>Conclusion</st>

AGYW newly started on ART at PMTCT enrolment are more likely to have early and recurring dropout. Given the cyclical nature of HIV care engagement, tailored and repeated interventions are needed to support continuous retention and re-engagement for pregnant and postpartum AGYW with HIV.

Data were organised into four themes. Participants described two orientation tendencies within the group, both with associated subthemes: (1) seeking connection (shame and isolation spurred interest in the group, active digital participation and engagement facilitated a sense of connection, value of an alternative); and (2) seeking to learn (searching for solutions, cameras enabled the modulation of engagement but inhibited group ownership, a useful addition to care). Participants likewise described (3) voices’ experiences in the group; and (4) the impact of the group (impact on voices, impact on sense of self and hope for the future).

Conclusions

Despite their differing expectations for the group, experiences in the group and relationship with the online medium of the group, both the participants seeking connection and those seeking to learn reported benefits from group attendance in terms of self-acceptance and hope for the future. Further research is needed to understand how to incorporate this survivor-led approach into the NHS.

Of 258 patients contacted, 75 (29%) completed a survey and 54 (21%) were eligible for inclusion (83.3% female; mean age = 18.33 years), all of whom received telehealth services between March 2020 and December 2022. Measures assessed attitudes toward online and face-to-face counseling, satisfaction with online therapy, and perceived effectiveness of CBT interventions delivered online. Data were analyzed using t-tests, Pearson correlations, and chi-square tests.

Results

Participants rated online CBT as significantly more valuable than face-to-face CBT (t(53) = 2.64, p = 0.011), with no differences in discomfort between modalities. Most participants (82.4%) reported satisfaction with online CBT, though preferences were split regarding continuing online versus in-person treatment. Proportion of previous online sessions was negatively correlated with perceived value of online counseling (r = −0.43, p = 0.001). Participants rated most CBT interventions as equally effective online, with highest ratings for meal planning (74.0%) and cognitive restructuring (71.7%), and lowest for open weighing (35.7%).

Discussion

Participants valued online CBT during the pandemic and viewed most CBT components as acceptable when delivered virtually, though individual preferences varied. Interventions requiring physical presence (e.g., open weighing) were perceived as less suitable for online delivery. While these preliminary findings suggest potential benefits of offering flexible treatment modalities, the low response rate limits generalizability and necessitates further research to examine the long-term viability of online CBT for AYAs with EDs.

Abstract

This article theorises agency as a relational, improvisational practice through which masculinities are negotiated and reimagined in cross-border higher education. Using narrative inquiry, it traces the journey of Jiaming, a Chinese doctoral student at an elite UK university, as he navigates Confucian legacies of family-state masculinity while confronting racialised misrecognition and shifting enactments of nationalism. Drawing on the concept of figured worlds, the analysis shows how agency is enacted through situated acts of self-authoring that both draw on and rework available cultural resources. It demonstrates how cross-border mobility unsettles familiar gender norms while opening space for hybrid masculinities grounded in care, emotional openness, and peer solidarity, developing through horizontal negotiations rather than hierarchical positioning. Jiaming’s story illustrates transition not as linear adaptation or assimilation, but as an ongoing, embodied process of becoming. By reframing agency as a relational practice emerging at the intersection of self and structure, rather than an inner trait or deficit-compensating response, the article challenges binary views of mobility and calls on institutions to recognise students’ cultural resources and support inclusive spaces for identity negotiation.

ABSTRACT

Research on mind-mindedness is flourishing but remains limited both in geographical scope and by the scarcity of longitudinal designs. Framed within a larger study of the transition to siblinghood in China, we addressed these gaps by following 138 Chinese mothers (82% university educated) from late pregnancy for a second child (Time 1; M weeks = 32) to two months postpartum (Time 2; M interval = 3.40 months). At both time points mothers completed (a) the Five-Minute Speech Sample paradigm, with transcripts coded for the proportion of mental attributes and positive valence mental attributes (i.e., positive mind-mindedness), and (b) questionnaire measures of maternal mental health, child-mother relationship quality, and internalizing and externalizing problems in their firstborn child (M T1 age = 4.48 years, SD = 1.66; 38.4% boys). Mean mind-mindedness (not positive mind-mindedness) increased significantly across this period of family change. Cross-lagged analyses showed that, controlling for baseline child internalizing symptoms, expectant mothers’ positive mind-mindedness predicted fewer internalizing problems post-birth. Conversely, initial child internalizing symptoms were inversely associated with positive mind-mindedness post-birth. This bidirectional relationship highlights the dynamic nature of mind-mindedness and suggests that positive MM supports Chinese firstborns’ adjustment during the sibling transition.

ABSTRACT

Individuals not in romantic relationships (i.e., singles) represent an emerging focus in close relationship research. Gender has been recognized as an important demographic variable in understanding singles’ experiences, with some existing Western literature suggesting that single women report higher levels of well-being than single men. However, the cross-cultural generalizability of these findings remains unclear, such as in Eastern contexts like China. China presents a compelling case for examining these gender differences due to its unique tension between traditional Confucian values that link female worth to marriage and an evolving societal shift toward individualism. As such, the present study recruited 1629 Chinese-speaking never-married singles (M
age = 29.41 years) via Credamo, a nationally representative data platform, and assessed multiple dimensions of well-being. Results indicated that single women reported higher well-being than single men, showing greater life satisfaction, relationship status satisfaction, lower desire for a partner, and fear of being single, but no differences in family satisfaction. Further analyses revealed that these gender differences remained largely robust across age, urbanization, and geographical regions. Overall, these data suggest the potential that there are culturally universal patterns in the relation between gender and singles’ well-being, whereby women are happier single than men.
