Abstract

Objective

Generalised anxiety disorder (GAD) is characterised by uncontrollable worries. Cognitive behaviour therapy (CBT) is an effective treatment for GAD; however, manualised treatments rarely include interventions to target core beliefs. This study aimed to (1) explore the core beliefs about the self that are endorsed by individuals with GAD, (2) determine whether these beliefs are related to the severity of GAD symptoms, (3) examine if core beliefs about the self shift in response to CBT for GAD and (4) determine whether changes in core beliefs about the self are related to reductions in symptom severity.

Method

A secondary data analysis of a randomised controlled trial with a repeated measures design examining the efficacy of videoconferencing-delivered CBT for individuals with GAD. Seventy-seven participants were included in the study (M
age = 37.14; 84.4% female).

Results

The results indicated that core beliefs about the self were commonly endorsed in GAD; however, there was no significant relationship between core beliefs about the self and baseline GAD symptom severity. There was a significant reduction in the severity of core beliefs about the self from pre-treatment to post-treatment (p < .001; Cohen’s d = .50) and pre-treatment to follow-up (p < .001; Cohen’s d = .51) despite the treatment not explicitly targeting these beliefs. There was also a significant relationship between changes in core beliefs about the self and GAD symptom severity at post-treatment and follow-up.

Conclusions

These findings provide insight into the role of core beliefs about the self in the treatment of GAD.

Abstract

There is growing interest in novel approaches to treating posttraumatic stress disorder (PTSD), including the use of psychedelic substances combined with psychotherapy, often referred to as psychedelic-assisted therapy, to better meet the needs of patients who do not prefer or respond to traditional evidence-based treatments. Among these treatments, 3,4-methylenedioxymethamphetamine (MDMA)–assisted psychotherapy (MDMA-AT) has shown promising outcomes in recent randomized controlled trials, with high response and remission rates. However, the U.S. Food and Drug Administration declined to approve MDMA-AT for marketable use in August 2024 due to concerns about insufficient evidence. This paper reviews the current state of the scientific literature on MDMA-AT for PTSD, including putative mechanisms of action; key strengths and limitations of methodologies used to date; gaps in the evidence; and clinical, ethical, and regulatory considerations. Key limitations to be addressed in future studies include challenges with blinding, a lack of active comparator conditions, a lack of head-to-head comparisons of different models, inadequate safety monitoring, and limited sample generalizability. We describe emerging research that integrates MDMA with established trauma-focused therapies, such as prolonged exposure therapy and cognitive processing therapy, to leverage MDMA’s effects on known cognitive behavioral mechanisms of action and support future implementation. Future research directions for advancing knowledge and consideration for the dissemination of MDMA-AT are discussed.

ABSTRACT

Transnational families are becoming more common, although the long-term impact of early childhood separations from biological parents remains unclear. “Satellite babies” (SBs) are Chinese infants born in North America, sent to China for caregiving, and then reunited with their parents. This interpretative phenomenological analysis (IPA) examined the lived experiences of 10 adult SBs to address gaps in attachment theory (AT) and inform culturally sensitive therapeutic practice. IPA’s iterative, in-depth approach to thematic data analysis revealed four group experiential themes (GETs): ambiguous loss, fear in relationships then and now, “how I got through it,” and change is possible. Results support key tenets from AT and ambiguous loss theory (ALT), including sensitive periods of attachment, adult attachment dimensions, and the “psychological family.” Results support negative developmental outcomes while highlighting protective factors like social support. Recommendations for practice include a call for MFTs to include ALT in culturally sensitive assessment and treatment of Chinese SB families.

ABSTRACT

This article examines the capability approach (CA) as an ethical framework for social work practice with minor refugees. Drawing on the foundational work of Amartya Sen and Martha Nussbaum, as well as its recent application to child welfare contexts, the article argues that conventional rights–based and deficit-oriented approaches are insufficient to address the complex, multidimensional vulnerabilities that refugee children face in transit and in host societies. The CA, in conjunction with the UN Convention on the Rights of the Child (UNCRC), provides a normative foundation that shifts the analytical focus from formal entitlements to the actual freedoms and real opportunities available to children in precarious circumstances. Central to this argument is the concept of age-appropriate participation: Refugee children must be understood not as passive recipients of protection but as agents whose evolving capacities demand structurally supported opportunities for meaningful self-expression and codetermination. The article further explores the practical implications of this theoretical reorientation for professional social work, including capability-sensitive assessment, participatory intervention design and the transformation of institutional structures that reproduce marginalization. It concludes by reflecting on the systemic obstacles that prevent capability-oriented practice and identifies directions for further research and professional development.

Abstract

Pupils in England who, for some reason, are not able to attend school often find themselves in Alternative Provision (AP). These are special arrangements designed to address their specific needs and help them return to mainstream schooling. AP involves a financial agreement between the provider and the organisation that commissions the provision and thus operates as an explicit form of education market in which educational services are traded directly for financial recompense. We report on a case study of one AP provider in the South-West of England undertaken over a one-year period, which generated data through individual, semi-structured interviews and observations. These are analysed using a Foucauldian framework to illuminate how truths are constructed, which then govern practices. We demonstrate how market arrangements affect the work of teachers and managers in the school, facing pressures which significantly distort the educational provision, but which satisfy the accountability that comes with the clear and specific market agreement about provision and its cost. We then use this instrumental account of one AP case to draw out more generally how markets operate through forms of accountability that change pedagogical and professional relationships. Specifically, these accountability pressures give teachers’ work its moral and political form, signalling what the ‘right’ practices are for teachers to undertake and altering the epistemological shape of teaching and learning. We conclude by briefly considering some of the implications for teachers and students.
