<h2>Background</h2>
<p>Risks related to long-term opioid therapy for chronic pain are high and may increase over time with aging. Deprescribing may be a beneficial intervention for older adults prescribed chronic opioids.</p>
<h2>Methods</h2>
<p>Semi-structured interviews with hypothetical clinical cases of older adults prescribed opioids for chronic pain: (1) low-risk case: a patient prescribed low-dose opioids without concerns; (2) moderate-risk case: a patient with multimorbidity and concurrent benzodiazepine use prescribed moderate opioid doses; (3) high-risk case: a patient prescribed high-dose opioids with signs of an opioid use disorder (OUD). PCPs were asked, in an open-ended fashion, to discuss whether they would initiate a deprescribing conversation, how they would approach deprescribing, and how they would approach a patient who declined recommendations to deprescribe.</p>
<h2>Participants and Setting</h2>
<p>PCPs from a Massachusetts health system.</p>
<h2>Results</h2>
<p>18 PCPs participated (56% female, 78% academic). More than half of PCPs would initiate a deprescribing conversation across the three cases. PCPs’ approach to deprescribing and mitigating risks differed based on clinical risk. In low and moderate-risk cases, PCPs emphasized a patient-directed taper plan and education on opioid risks. In the high-risk case, some PCPs were uncertain about initiating a deprescribing conversation due to concerns about the patient’s mental health and the risk of illicit opioid use. Naloxone was infrequently recommended across the three cases, but in the high-risk case, approximately half of PCPs suggested medications for OUD.</p>
<h2>Conclusions</h2>
<p>PCPs reported that they would often initiate opioid deprescribing conversations with older adults, but were less confident in managing older adults with signs of OUD. PCPs require additional support to implement successful conversations on opioid deprescribing with older adults.</p>

                        <p><p><sec><st>Background</st></sec></p>
<p>Patients who consult their GP about psychological complaints, such as feeling anxious or depressed, are often initially given a psychological symptom diagnosis. However, it remains unclear whether these symptoms will develop into psychiatric conditions, which is crucial for informing patients about their prognosis and guiding GPs in their management.</p>
<p><br>
<sec><st>Aims</st></sec></p>
<p>To explore the course of psychological symptom diagnoses and compare GPs’ management strategies for (a) psychological symptom diagnoses that persisted for more than a year and (b) those that changed into psychiatric conditions during the first year of care.</p>
<p><br>
<sec><st>Methods</st></sec></p>
<p>We performed a retrospective cohort study using the Family-Medicine Network database. We included all episodes of care (EoC) that started with a psychological symptom diagnosis between 2008 and 2021. We performed negative binomial analyses and logistic regression analyses to compare management strategies and number of contacts between EoC that changed into psychiatric conditions and persistent psychological symptoms (>1 year) during the first year of care.</p>
<p><br>
<sec><st>Results</st></sec></p>
<p>Out of the 14 633 EoC that started with a psychological symptom diagnosis, 79.4% resolved within 1 year, 12.8% persisted as psychological symptoms and 7.8% changed into psychiatric conditions. In EoC that changed into psychiatric conditions, as compared with EoC that persisted as psychological symptoms, we observed a significantly higher number of contacts with the GP (RR=1.76, 95% CI 1.63 to 1.91) as well as an increased total number of interventions (RR=1.71, 95% CI 1.58 to 1.84).</p>
<p><br>
<sec><st>Conclusion</st></sec></p>
<p>Most psychological symptoms remain for only a short period of time and only a few persist or change into psychiatric conditions. Future research should investigate the factors that influence patients’ decisions to seek further help from their GPs as well as those that contribute to the transition from psychological symptom diagnoses to a psychiatric condition.</p>

<p>People with intellectual disability experience barriers in accessing healthcare. A virtual reality (VR) application that includes an intelligent agent powered by generative artificial intelligence (AI) may support preparation for healthcare visits in this population.</p>
<h2>Objective</h2>
<p>To co-design, develop and evaluate the feasibility and usability of an AI-based VR application to improve healthcare preparedness for people with intellectual disability.</p>
<h2>Methods</h2>
<p>Ten adults with intellectual disability completed an AI-VR experience simulating a general practitioner visit across three sequential scenes: checking in with a receptionist, waiting in a clinic waiting room, and consulting with a doctor. Participants interacted verbally with AI avatars. Semi-structured interviews followed each scene.</p>
<h2>Results</h2>
<p>Participants valued the avatars’ clear, patient communication and described the system as supportive for learning healthcare content and practising communication and self-advocacy skills. Usability issues were identified.</p>
<h2>Conclusion</h2>
<p>AI-VR appears feasible and acceptable for healthcare preparation in people with intellectual disability. Refinements to system usability are needed to support independent use and broader implementation.</p>

<h2>Introduction</h2>
<p>Substance use among young people is a growing public health concern in South Africa (SA), with implications for short- and long-term mental health, education and development. A lack of use data is among several challenges impeding progress to support young people. To address this gap, this study aimed to synthesise published prevalence data on substance use among adolescents under 19 years of age in SA.</p>
<h2>Methods</h2>
<p>We conducted a systematic review and meta-analysis following PRISMA guidelines. Searches were run across PubMed, PsycINFO, Web of Science and Scielo. Studies reporting original SA prevalence data in community or school-based samples were included. The Joanna Briggs Institute checklist for prevalence studies was used to assess study quality. Meta-analyses were performed in R, using random effects models and heterogeneity was assessed using <i>I</i><br>
<sup>2</sup> statistics and meta-regression.</p>
<h2>Results</h2>
<p>Thirty publications met inclusion criteria, and these reported 202 prevalence estimates (<i>n</i> = 120,041, mean age 16.1 years). The most commonly reported substances used were alcohol (35.09%, 95% CI 23.83–48.30), tobacco (25.47%, 95% CI 14.56–33.01) and cannabis (10.47%, 95% CI 5.98–17.71) with lifetime prevalence of any substance use of 13.05%, 95% CI 9.65–17.42. Similarly, 12-month data indicated high exposure levels: alcohol 17.45% (95% CI 11.39–25.78), tobacco 11.57% (95% CI 8.92–14.88) and cannabis 6.66% (95% CI 4.82–9.13). However, substantial heterogeneity across studies was detected.</p>
<h2>Discussion and Conclusions</h2>
<p>High exposure to substances in SA, especially alcohol, tobacco and cannabis, underscores the urgent need for nationally representative surveillance and evidence-based prevention efforts tailored to adolescents, to avoid progression of substance use into substance use disorder.</p>

<p>We examined English-speaking preschoolers’ and adults’ attention to emotional prosody in an unfamiliar language when asked to: (a) match emotional prosody with emotional faces; and (b) use emotional prosody to identify a speaker’s intended referent. In Experiment 1, 4-year-olds (<i>N</i> = 36, <i>M</i> = 4.16 years; 18 females) and adults (<i>N</i> = 38, <i>M</i> = 21.18 years; 26 females) matched happy and sad Polish utterances to a corresponding emotional face, as evidenced through pointing decisions. In Experiment 2, adults (<i>N</i> = 36, <i>M</i> = 20.17 years; 31 females), but not 4-year-olds (<i>N</i> = 36, <i>M</i> = 4.11 years; 18 females), matched the same emotional utterances to objects whose properties signalled an association with happiness or sadness (e.g., intact vs. broken toy). These findings demonstrate that 4-year-olds and adults can recognise emotional prosody in an unfamiliar language, however, only adults are successful at extending this information to other kinds of emotion-relevant decisions.</p>

<p>Access to timely and appropriate information is a key component of quality cancer care.</p>
<h2>Aims</h2>
<p>This study aimed to fill an important gap regarding the information experiences of women with endometrial cancer from diagnosis to treatment.</p>
<h2>Methods</h2>
<p>This study is based on 24 interviews with women diagnosed with endometrial cancer in Queensland, Australia in 2022–2023. Reflexive thematic analysis was used to provide an in-depth exploration of women’s accounts of information across their diagnosis and treatment journeys.</p>
<h2>Results</h2>
<p>The researchers identified three implicit and socially constructed <i>assumptions</i> women negotiated when it came to information. These included: (1) the physical body is of central importance; (2) more information is better; and (3) medical knowledge is most credible. These assumptions were not always upheld or accepted; at times, they were rejected by women and ultimately shaped how women made sense of, and spoke about, their experiences of information.</p>
<h2>Conclusions</h2>
<p>Together, these findings point to the need for women to be supported to engage with contextually sensitive information regarding their cancer. In particular, health professionals should be actively involved in ensuring women understand their cancer (including treatment side-effects and risk of recurrence), are supported with psycho-social impacts, and that alternative and holistic sources of information are acknowledged alongside medical information. These findings point to the need for health professionals to reflect on their own communication practices and how they may reinforce assumptions that shape women’s experiences of endometrial cancer.</p>
