ABSTRACT

Objective

This study conducted a preliminary naturalistic effectiveness evaluation of Individualised Metacognitive Therapy (MCT+) delivered via videoconferencing for individuals with bipolar I disorder (BD-I) in a real-world clinical setting in Chile. It also explored how therapist characteristics—specifically mentalisation capacity—influence the therapeutic alliance in digital psychotherapy.

Methods

A longitudinal quantitative case series design was implemented across 14 therapist–patient dyads. Patients received 12 weekly sessions of MCT+ online. Standardised measures assessed anxiety (GAD-7), depression (PHQ-9), metacognitive beliefs (MCQ-30), psychological distress (CORE-10) and quality of life (WHOQOL-BREF). Therapeutic alliance was tracked session-by-session (WAI-S). Therapist mentalisation and attachment were evaluated at baseline (MASC-SP, RFQ-8, ECR-12). Changes in outcome measures were analysed using paired t-tests, effect sizes (Cohen’s d), correlations, reliable change indices (RCI) and hierarchical linear modelling (HLM).

Results

Across the intervention, there were significant decreases in anxiety (d = 0.64) and improvements in metacognitive beliefs (d = 0.37). Depression showed a modest improvement (d = 0.34), while changes in quality of life were negligible (d = −0.21). Hierarchical modelling indicated a significant interaction between automatic and controlled mentalisation (b = −0.45, p = 0.008), suggesting that flexible adjustment supported therapeutic alliance development. Attachment style showed no significant associations with the alliance (largest unadjusted effect: ρ = −0.54, p = 0.073; all adjusted ps > 0.99).

Conclusions

Digital MCT+ showed preliminary effectiveness in reducing anxiety and maladaptive metacognitive beliefs among individuals with BD-I, with more limited effects on depression and quality of life. Importantly, therapist mentalisation flexibility—the capacity to shift between automatic and controlled modes, or the mentalisation switch—emerged as a central mechanism for alliance building and engagement in digital contexts, highlighting a key target for clinical training and future research.

Abstract

Objectives

To examine cognitive beliefs associated with hoarding in the context of homelessness, focusing on fear of future material deprivation, and to compare housing-related adversities across groups with hoarding difficulties, homelessness or both.

Design

Cross-sectional, three-group comparative study.

Methods

Adults in the UK/Ireland were recruited between July 2023 and April 2024 through third-sector and clinical partners via online, postal and in-person routes. Participants were grouped as hoarding difficulties with homelessness in the past 10 years (H&H; n = 47), hoarding difficulties without homelessness (HD; n = 43) and homelessness without hoarding difficulties (HM; n = 39). Hoarding difficulties were identified by professional referral and eligibility screening, mapping onto DSM-5 criteria (self-report endorsement of all criteria). Primary outcomes were Beliefs About Hoarding Questionnaire–Revised (BAH) subscales (fear of material deprivation, harm avoidance, attachment disturbance). Secondary outcomes included negative housing experiences, hoarding severity (SI-R), depression (PHQ-8), anxiety (GAD-7), functioning (WSAS) and early material deprivation (EEMD). Mixed-model ANOVA tested group differences in BAH subscales, and Kruskal–Wallis tested housing experiences.

Results

A significant group-by-belief interaction emerged. Fear of material deprivation and attachment disturbance were higher in H&H and HD than in HM. Harm avoidance was highest in H&H. H&H reported more negative housing experiences than HD and did not differ from HM (H&H ≈ HM > HD). Exploratory analyses showed higher depression, anxiety and impairment in H&H. HD showed the greatest hoarding severity (SI-R total, clutter, discarding), whereas excessive acquisition was similar in H&H and HD.

Conclusions

Hoarding difficulties in homelessness contexts appear embedded in cumulative psychosocial vulnerability rather than explained by deprivation alone. Harm avoidance may be particularly salient when hoarding co-occurs with homelessness, suggesting potential targets for formulation and intervention in homelessness services.

Background: Mobile health (mHealth) apps can address health inequities and enhance access to care for individuals with immunocompromising conditions. Although hundreds of oncology apps exist, research on provider perspectives regarding their use in clinical care remains limited. Objective: This study aimed to describe oncology providers’ recommended apps, mHealth attitudes and beliefs, and perceived barriers to and facilitators of mHealth adoption. Exploratory aims examined differences based on provider type (medical vs psychosocial), provider age (<45 vs ≥45 years old), and patient population (pediatric vs adult). Methods: We conducted a cross-sectional survey administered via REDCap (Research Electronic Data Capture) to oncology providers across the United States between June and November 2024. Data were summarized using descriptive statistics. Pearson’s chi-square analyses examined exploratory group differences based on provider type, provider age, and patient age. Results: Of 188 respondents, the majority self-identified as female (150/188, 79.8%), White (161/188, 85.6%), and non-Hispanic/Latino (174/188, 92.6%). Nearly all providers (178/188, 94.7%) reported either recommending or using mHealth apps with their patients, with primary use for patient-provider communication (139/188, 73.9%). Providers perceived potential benefit across a broad spectrum of holistic care functions. Providers, on average, reported a growth mindset and confidence in their ability to learn mHealth tools and in its potential to improve care access. Key facilitators included alignment with patient needs, increased accessibility, and cost-effectiveness, while barriers included disparities in technology access, digital health literacy, and data security and privacy. Exploratory analyses showed some significant group differences by provider role, provider age, and patient age. Psychosocial providers were significantly more likely to recommend or use apps for pain management (χ21=14.34, P<.001, φ=0.28), mental health (χ21=50.54, P<.001, φ=0.53), and sleep health (χ21=25.47, P<.001, φ= 0.38). Psychosocial providers also perceived higher benefit for sleep health apps (χ21=6.40, P=.01, φ=0.19). Medical providers were significantly more likely to perceive medication management apps as potentially beneficial (χ21=10.93, P<.001, φ=0.25). Older providers (16/88, 18.2%) and adult care providers (8/32, 25%) were significantly more likely to recommend or use disease management apps compared to younger providers (5/100, 5%; χ21=8.20, P=.004, φ=0.21) and pediatric care providers (6/101, 5.9%; χ22=9.22, P=.01, Cramer V=0.22), respectively. Pediatric care providers (83/101, 82.2%) were more likely to recommend or use medical team communication apps compared to adult care providers (15/32, 46.9%; χ22=15.66, P<.001, Cramer V=0.29). Conclusions: Our study underscores the opportunity to develop inclusive mHealth solutions tailored to the diverse needs of individuals across the cancer care continuum, including those in active treatment and survivorship care. Engaging diverse medical and psychosocial providers is essential to inform clinical integration of mHealth technologies in oncology care.

ABSTRACT

Understanding the co-development of moral personality and beliefs about control over well-being is essential for clarifying how individuals sustain mental health. However, the theoretical linkage between moral personality and beliefs about the causation of well-being remains underexplored. Addressing this gap, the present study examined the reciprocal dynamics between honesty–humility and internal well-being locus of control (Internal WB-LOC). Drawing on four–wave longitudinal data from 1349 Chinese participants, we employed random intercept cross-lagged panel models to investigate these associations. Results demonstrated stable between–person effects, indicating that individuals with high levels of honesty–humility tend to exhibit greater Internal WB-LOC across time. Results also showed concurrent positive associations within individuals and cross-lagged effects, indicating that increases in levels of honesty–humility temporally precede increases in Internal WB-LOC, and vice versa. These findings integrate personality research with belief systems about well-being, suggesting a dynamic interplay in which internalised moral orientation and perceived agency over well-being are mutually reinforcing.

Abstract

Despite established evidence of a mutual relationship between peer victimization and internalizing symptoms, gender-specific variations remain underexplored, hindering the development of tailored interventions. This study examines gender-specific symptom networks linking peer victimization and internalizing symptoms in 10,374 Chinese adolescents (47.9% female). Peer victimization (verbal, physical, relational, and cyber) and internalizing symptoms (depression and anxiety) were analyzed using undirected and Bayesian network models to identify core and bridging symptoms. In the undirected network, both genders shared important nodes, including “public mocking,” “threats intimidation,” and “often fearful,” while “online harassment” was distinctive for females and “social isolated” for males. “Feel unloved” emerged as the key bridging node linking internalizing symptoms and peer victimization among females, whereas “fear of doing bad” served this bridging role among males. Directed acyclic graph from the Bayesian network indicated that internalizing symptoms and victimization symptoms formed a single top-down cluster among females, whereas these symptoms formed two parallel clusters among males. These findings underscore the importance of gender-specific interventions targeting core and bridging symptoms to disrupt the cycle of internalizing symptoms and peer victimization.
