ABSTRACT

Substance use disorders (SUDs) are among the leading causes of morbidity and mortality globally. Identification of aetiological factors related to the severity of substance use can offer targets for therapeutic interventions. Role of impulsivity and emotion dysregulation in SUDs is well established. Based on the evidence that the adverse effects of impulsivity on behavioural and mental health outcomes can be mediated by emotion dysregulation, this study explored the possible mediating role of emotion dysregulation in the association between impulsivity and the severity of substance dependence. One hundred treatment-seeking substance users (monosubstance users and polysubstance users) were enrolled. Impulsivity, emotion dysregulation and severity of substance dependence were assessed using the Barratt Impulsiveness Scale-11, the Difficulties Emotion Regulation Scale and the Severity Dependence Scale, respectively. Correlation, mediation and 95% bias-corrected and accelerated (BCa CI) bootstrapped analyses were run. Emotion dysregulation was found to mediate the association between attention impulsivity (indirect effect [BCa CI]: 0.22 [0.11–0.36]), motor impulsivity (indirect effect [BCa CI]: 0.15 [0.08–0.24]), non-planning impulsivity (indirect effect [BCa CI]: 0.11 [0.04–0.18]) and severity of substance dependence. The direct effect of different facets of impulsivity on dependence severity was not significant after accounting for emotion dysregulation. Impulsivity was found to have a significant indirect effect on the severity of substance use through emotion dysregulation. Among substance users, emotion dysregulation may be involved in the association between impulsivity and severity of substance dependence. Emotion dysregulation could be considered a possible therapeutic target to reduce the severity of substance dependence among substance users with impulsive tendencies.

ABSTRACT

Climate change has and will continue to have a profound effect on both individuals and the communities in which they reside. A pressing question is how communities forced to confront climate-induced catastrophe—including both rapid and slow-onset hazards—undertake the decision of how and when to relocate, particularly given the multiple scales at which decision-making processes play out and the broadly felt social, economic, and environmental effects that arise from the resulting decisions. In the US, multiple communities have begun to face the challenge of relocation through the creation of formalized plans, plans that can themselves be used as windows into a complex and emergent governance process. But before the effectiveness of these processes in governing relocation can be assessed, we must first understand the structure under which relocation occurs. Through the example of three US communities that have already begun the process of relocation, we document the organizations involved and the policies and programs cited. Leveraging the Institutional Grammar, we next explore the institutional structure of the relocation process against theoretical expectations for polycentric governance. In doing so, we provide further examples of semi-automated analysis of institutional grammar using open-source software, contributing to an even wider realm of policy scholarship.

Education is a pillar of “social reproduction,” meaning it’s a social service necessary for maintaining daily life now and for future generations. When governments alter access to education and the way they deliver it, they shape everyday lives today and beyond. Since legal and regulatory changes shape how society is reproduced, it is possible to draw from these changes some ideas about the government’s social values. From this perspective, Ontario’s Doug Ford government is sending the message that education is about generating private wealth and social order.

Apr 20th 2026, 15:04

<st>Background:</st>

Although severe maternal morbidity (SMM), such as severe hemorrhage and sepsis, can occur from conception to 6 weeks postpartum, most surveillance studies in Canada are restricted to intrapartum cases, with rates approximating 17 per 1000 births. We aimed to characterize the frequency of SMM from conception to 6 weeks postpartum and describe its associations with maternal and clinical characteristics.

<st>Methods:</st>

We conducted a population-based cohort study of births at 20 or more weeks’ gestation, using administrative data in Ontario (2012 to 2021), and identified SMM using the Canadian Perinatal Surveillance System definition. We assessed the frequency of SMM overall, by period of occurrence (antepartum, intrapartum, and 6 weeks postpartum), and by SMM types. For each period, we described crude associations between characteristics identified a priori and SMM, using logistic regression.

<st>Results:</st>

Among 1 095 228 births, the SMM rate was 27.24 per 1000 births; 15.63%, 55.02%, and 29.34% of cases occurred antepartum, intrapartum, and postpartum, respectively. The most common SMM types were severe hemorrhage overall (6.10 per 1000 births), acute abdomen antepartum (1.94), severe hemorrhage intrapartum (5.28), and sepsis postpartum (4.69). Characteristics associated with SMM varied across periods. For example, whereas the frequency of intrapartum and postpartum SMM showed a U-shaped relationship with maternal age, antepartum SMM was associated with ages 15 to 19 years (v. 25 to 34 yr; odds ratio 2.22, 95% confidence interval 1.91 to 2.58) but not 40 to 49 years.

<st>Interpretation:</st>

In Ontario, SMM affects nearly 3% of pregnancies between conception and 6 weeks postpartum. Extending the usual observation window showed that approximately 40% of cases of SMM occurred outside the intrapartum period, which would support measures to strengthen recognition of SMM from conception through the postpartum period.

The report’s corresponding author said the findings reflect changes in epidemiology and resistance that are transforming Shigella from a primarily self-limiting infection that rarely required antibiotics into an emerging public health threat with limited treatment options.

ABSTRACT

The current paper offers guidance to practitioners working in some capacity with young people who are using abusive and harmful behaviours in their relationships. It highlights the significance of the working relationship between the practitioner and the young person and advocates for an approach that is holistic, supportive and judgement-free. The paper outlines literature evidencing the positive impact such an approach can have and is written for both specialist domestic abuse practitioners and practitioners working with young people across a range of sectors and organisations.

Abstract

Objectives

This study aimed to explore the impact of cystic fibrosis (CF) on the transition to motherhood using a strength-based narrative approach. It sought to elevate the voices of women with CF and examine how they navigate pregnancy and early motherhood in the context of chronic illness.

Design

A qualitative narrative study was conducted, led by a researcher with lived experience of CF and motherhood. The study adopted a co-constructed, interpretive epistemology to centre participant perspectives and challenge deficit-based clinical narratives.

Methods

Ten Australian women with CF who had given birth within the past 5 years participated in in-depth, semi-structured interviews. Narratives were analysed using holistic and categorical methods, including thematic coding and story mapping. Reflexive collaboration and participant validation were used to enhance trustworthiness.

Findings

Narratives consistently followed the ‘Hero’s Journey’ archetype, with participants portraying themselves as resilient protagonists. Key strengths included hope, leadership, discernment, child-focus and optimism. While healthcare providers were often described as supportive allies, antagonistic interactions—particularly during pre-pregnancy counselling—undermined trust and joy. Postpartum, women struggled to balance CF treatment with mothering roles, often prioritising their child over medical compliance. Breastfeeding emerged as a specific area of conflict between maternal values and clinical expectations.

Conclusions

Women with CF demonstrate significant strengths in navigating motherhood, yet face systemic challenges in receiving holistic, collaborative care. This study highlights the value of lived experience-led research and calls for healthcare teams to offer more tailored postpartum support and nuanced guidance around treatment, parenting and breastfeeding.

Abstract

Digital dating abuse (DDA), the use of technology to coerce, control, harass, threaten, monitor, or pressure a romantic partner, is a widespread form of dating violence among adolescents that is associated with adverse mental health outcomes. Previous research shows that adolescents misinterpret some DDA behaviors as normative relationship behaviors, increasing the risk for more severe DDA. These misinterpretations may stem from DDA occurring within positive (e.g., affection and joking) contexts, and who is experiencing and/or engaging in DDA. Yet, it remains unclear how the context in which DDA occurs (DDA only, DDA with affection, or no DDA) and roles (perpetrator, victim, or mutual) influence adolescents’ recognition of and emotional responses to DDA. Two between-subjects experiments using hypothetical text messages examined how DDA context and role influenced DDA recognition and anticipation of feeling upset among Hispanic and Latino high school adolescents (Study 1: N = 422, M
age = 15.76, SD = 1.02, 54.7% female; Study 2: N = 384, M
age = 16.52, SD = 0.98, 51.8% female). Across experiments, adolescents’ recognition of DDA behaviors and anticipation of feeling upset were significantly higher in both the DDA-with-affection and DDA-only contexts compared to the no-DDA context. Findings suggest that even when affectionate behaviors are present, DDA behaviors were still recognized by adolescents. There were no significant main effects of role on DDA recognition or anticipation of feeling upset. The findings of this work highlight the importance of addressing both overt and mixed-message forms of DDA in future adolescent prevention and intervention efforts.

ABSTRACT

Background and Aims

Mixed Depression (MxD), characterized by the co-occurrence of depressive and excitatory symptoms, is a prevalent yet often underdiagnosed presentation in bipolar disorders (BD), with significant implications for prognosis and treatment. Childhood trauma is a key environmental risk factor associated with a more severe course of BD, as it influences its onset, progression, and clinical presentation. To date, no studies have specifically investigated the relationship between lifetime childhood trauma and lifetime MxD in individuals with BD. The aim of our study was to address this gap in a large and well-characterized sample of patients with BD type I and type II.

Methods

A total of 376 individuals, 220 euthymic outpatients with BD (MxD = 100, non-MxD = 120) and 156 healthy controls (HC), were enrolled. Childhood trauma was assessed using the Childhood Trauma Questionnaire (CTQ). Lifetime MxD was identified according to the Koukopoulos criteria. The relationship between childhood trauma and MxD was evaluated using two different multivariate models.

Results

The first model (Wilks’ Lambda = 0.834, p < 0.001) revealed that BD patients with lifetime MxD reported significantly higher CTQ total scores compared to both non-MxD (p = 0.029) and HC (p < 0.001). When examining childhood trauma subtypes, BD patients with MxD reported significantly higher emotional abuse scores than those without MxD (p < 0.001). Furthermore, compared to HC, the MxD group showed significantly elevated scores in emotional abuse, emotional neglect, and physical abuse (all p < 0.001). The non-MxD group scored higher than HC on physical abuse (p = 0.008) and physical neglect (p < 0.001). Regression analyses confirmed specifically emotional abuse as a significant predictor of lifetime MxD.

Conclusions

The findings demonstrate a strong association between childhood trauma, particularly emotional abuse, and lifetime MxD. These results underscore the significance of childhood trauma as a key predictor of clinical outcomes in BD.

ABSTRACT

Objective

This brief report examines how often a technology called QDAS (qualitative data analysis software) was mentioned in family science (FS) research between 2011 and 2020 and how it was described. It also provides data about qualitative FS research trends and examines whether there is a relationship between QDAS use and grounded theory.

Background

QDAS is a technology used by qualitative FS scholars, but little is known about its prevalence or how it is used. Research in this area is needed because technology can influence qualitative research practices in unknown or unacknowledged ways such as methodological homogenization.

Method

Empirical articles from five FS journals were examined for whether they presented qualitative findings during 2011–2015 and 2016–2020. Identified articles were then examined for their QDAS use. A chi-square analysis compared articles mentioning QDAS with those not mentioning it for whether they were more likely to mention grounded theory.

Results

The percentage of qualitative research findings increased from 15% to 17% across the two time periods; in those articles, QDAS use increased from 25% to 41%. Few details were provided about how the programs were used, and a moderate relationship was found between QDAS and grounded theory.

Conclusion

QDAS use is increasing in FS, and more detail needs to be provided about how it is used. This information is increasingly important due to the incorporation of automatic features into QDAS programs such as Generative Artificial Intelligence tools.

Abstract

Objective

The current study examined whether Taiwanese intimate partners’ gender would determine their latent profiles of gender role attitudes, warmth toward partner, and perceived equity at home and in society.

Background

Taiwanese middle-aged adults have received traditional education rooted in Confucianism and encountered societal shifts in gender roles. How their gender in the context of East Asian cultures affects their gender role profiles is understudied.

Method

Using data collected in 2022 from the Taiwan Social Change Survey, a nationally representative data set, we conducted a latent profile analysis of Taiwanese intimate partners (M
age = 52.5, N = 1,024, 55% female) to explore potential determinants of their group membership of gender role profiles.

Results

Three latent groups were identified: The largest group (64%; mostly men) had the highest egalitarian gender role attitudes and warmth toward partner. The other two groups were a more traditional gender role profile (21%; mostly women) and an egalitarian gender role profile with the lowest perceived inequity at home and in society (16%; mostly women).

Conclusion

Taiwanese gender role attitudes have been more egalitarian over the past decade. Intimate partners’ gender predicts their gender role profiles as indicated by variables across bioecological levels such as individual gender role attitudes, warmth toward partner, and perceived equity in family and society.

Implications

The fact that a large proportion of Taiwanese men showed more egalitarian gender role attitudes is worth investigating. Practitioners promoting gender equity could pay more attention to clients’ gender role profiles.

ABSTRACT

Objective

The present study aims to assess the distribution of food addiction (FA) across different levels of food insecurity (FI) in Brazilian women of reproductive age living in poverty, and to investigate the association between FA and FI.

Method

This is a cross-sectional study conducted between October 2020 and May 2021 in 40 urban favelas and low-income communities. The FA was assessed using the modified Yale Food Addiction Scale 2.0 (mYFAS 2.0) and the FI was measured using the Brazilian Food Insecurity Scale (EBIA). A logistic regression model was used to examine the association between FA and FI, adjusting for age (in years), poverty status, race/skin colour, physical activity, nutritional status, and self-reported diagnosis of depression and anxiety. A total of 1863 adult women of reproductive age (20–44 years) were included.

Results

Among the women with FA, 30.7% experienced severe FI. A significant association was found between FA and severe FI, with the chances of FA being higher among women with severe FI compared to those with food security.

Conclusion

Severe FI is associated with higher chances of FA, particularly in socioeconomically vulnerable contexts.
