ABSTRACT

Many features of personality psychopathology are shared among patients with Anorexia Nervosa (AN), yet the impact of these shared features is not fully understood, and the impact of having more personality psychopathology features during adolescent onset of AN, especially among adolescents admitted to an inpatient eating disorder unit, has not been explored. This study evaluated how personality psychopathology, utilizing a measure of borderline personality disorder (BPD) symptom severity, has a potential predictive value in understanding risk of readmission for an eating disorder over time. A total of 256 adolescents were admitted for medical stability to an inpatient unit for the first time. Adolescents completed the Borderline Personality Features Scale for Children-11 (BPFSC-11) within the first 14 days of admission; demographic variables, anxiety and mood comorbidities, and readmission status were obtained through chart review. Higher scores on the BPFSC-11 were significantly associated with being readmitted within 1 year of discharge from the inpatient unit. These findings highlight how personality psychopathology features at onset of severe AN in adolescence could indicate a higher risk for recovery complications, including repeated inpatient admission. These findings may underscore the role of supplementary therapeutic interventions alongside family-based therapy, including dialectical behavior therapy.

Abstract

The International Trauma Questionnaire (ITQ) assesses posttraumatic stress disorder (PTSD) and the disturbances in self-organization (DSO) aspect of complex PTSD (CPTSD) per the ICD–11. This study examined the ITQ’s factor structure among violence- or abuse-exposed women in Northern Ireland, a region with a history of conflict (i.e., “the Troubles”), to validate its use in this unique sociopolitical context. The sample consisted of the 542 women who participated in the Violence Against Women and Girls Survey in Northern Ireland. All participants completed the ITQ and reported lifetime experiences of various forms of violence and abuse. Construct validity was assessed by testing four models of ITQ’s factor structure using confirmatory factor analysis (CFA). Regression analyses were performed on the factors from the two best-fitting CFA models to predict PTSD and DSO scores using age and types of violence as predictors. The two-factor second-order model and the correlated six-factor first-order model both showed good fit. Factor loadings indicated that all ITQ items adequately measured their intended constructs, λs = .697−.973. Regression analyses on both models revealed that physical violence, sexual violence, and digital abuse significantly predicted both PTSD and DSO scores, with stalking predictive of PTSD only, two-factor second-order model: βs = .118−.244, correlated model: βs = .101−.287. Findings suggest that the ITQ is a reliable and valid tool for assessing PTSD and DSO in women living in Northern Ireland. Associations with violence types showed partial differences and overlap, highlighting value in examining PTSD and CPTSD regarding specific trauma types.

ABSTRACT

Introduction

The Age-Friendly Health Systems (AFHS) initiative aims to improve care for older adults through the “4Ms” framework: What Matters, Medication, Mentation, and Mobility. Despite national momentum and evidence within individual M domains, limited evidence guides outpatient AFHS implementation of the 4Ms as a set. The objective of this systematic review was to summarize the evidence of the impact of AFHS implementation in outpatient settings.

Methods

We searched Medline, EMBASE, CINAHL, Cochrane, and clinicaltrials.gov from 2015 to November 22, 2024. Comparative studies that implemented all 4Ms in outpatient settings were included. Risk of bias was assessed using questions derived from the Cochrane Risk of Bias tool for RCTs and the Risk of Bias In Non-randomized Studies—of Intervention tool for other study designs, and results were summarized using GRADE methodology.

Results

Twelve US-based studies met inclusion criteria. Overall, implementing AFHS interventions was associated with improved process measures across all 4Ms, though the effectiveness of specific implementation strategies could not be determined. Outcome and structural measures were infrequently reported. Study heterogeneity and poor reporting limited generalizability.

Discussion

Findings underscore the urgent need for standardized, outcomes-oriented AFHS measurement before policy and payment reforms, such as CMS’s Age-Friendly Hospital Measure, are expanded into outpatient settings. To advance meaningful transformation, future research must prioritize implementation fidelity, outcome evaluation, and measures that reflect older adults’ values and lived experiences.

ABSTRACT

In this article, we explore the consequences of parents’ trust and distrust of social workers in child welfare services. We collected data via qualitative surveys administered in 2022, to which 29 Finnish social workers responded. We used thematic analysis to identify the key themes of the consequences of trust in our data. According to our results, social workers described the consequences of trust and distrust primarily as opposing phenomena. The consequences of trust and distrust were visible during interactions between a parent and social worker as well as in parents’ commitment to cooperating with a social worker. Parents’ trust in a social worker also affected whether their children’s situations changed for the better and if children felt safe around the professionals working with them. Our results demonstrate that parents’ trust of social workers serves as the basis for so-called ‘change work’ in child welfare services, which aims to improve a child’s and family’s situation. However, we also identified critical elements of trust and challenges to establishing trust created by the child welfare service context, such as the power asymmetry between social workers and parents. This article furthers our understanding of the concept of trust and its significance in the relationship between parents and social workers. It also offers practical insights into key areas that social workers should prioritize when working to build trust with parents.

ABSTRACT

Psychological well-being (PWB) is a core component of mental health influenced by personality; however, most prior research focused on subjective well-being and normal-range personality traits. Less is known about the longitudinal role of maladaptive personality traits in shaping eudaimonic well-being. This study examined prospective associations between maladaptive personality domains and subsequent PWB in the SUN cohort. Personality traits were assessed at the 16-year follow-up using the Personality Inventory for DSM-5 Abbreviated Form (PID-5-BF). PWB was evaluated 2 years later using Ryff’s 29-item scale. All domains were analyzed as continuous variables in multivariable linear regression models with mutual adjustment. Among 2080 participants (56.9% women, mean age 57 ± 10.9 years), higher levels of detachment (β: −2.43; 95% CI: −2.77, −2.09), negative affect (β: −1.09; 95% CI: −1.38, −0.80), psychoticism (β: −0.76; 95% CI: −1.21, −0.30) and disinhibition (β: −0.52; 95% CI: −0.93, −0.11) were prospectively associated with lower PWB, whereas antagonism showed a positive association (β: 0.76; 95% CI: 0.36, 1.17). Detachment was consistently associated with lower scores across all six PWB dimensions, negative affect and psychoticism showed selective inverse associations with several PWB dimensions, and disinhibition showed an inverse association for autonomy. Nevertheless, antagonism displayed positive associations with environmental mastery, purpose in life, and self-acceptance. In conclusion, maladaptive personality traits are prospectively associated with PWB, with domain-specific patterns, highlighting the importance of a multidimensional perspective when examining personality–PWB relationships.

ABSTRACT

Migration scholars often present democratic decline as an emigration driver. This effect is more pronounced among highly-skilled citizens who have the resources and capability to settle abroad. Yet, not much attention has been paid to why highly-skilled emigrants would opt to return to their autocratizing countries, even if they are concerned about the political path the country has taken. This article provides answers to this puzzle through 41 semi-structured interviews with highly-skilled Turkish citizens who were born and raised in Turkey and who, after living abroad, voluntarily returned after 2016, when Turkey’s autocratization reached its peak. In line with the existing literature, it finds that socio-economic and affective reasons play an important role in shaping return decisions. However, as a novel contribution to the literature, it finds that autocratization paradoxically acts as a motivating factor for some migrants who want to move back to contribute to the fight for democratisation. Building on this, the article conceptualises “return migration as voice” (a la Hirschman), a new analytical lens to approach highly-skilled migration flows in autocratizing countries.

ABSTRACT

Although metacognitive training (MCT) and cognitive-behavioural therapy (CBT) have been proven to be effective interventions for the treatment of symptoms of schizophrenia, still little is known about the effects on quality of life (QoL). This systematic review compares the effectiveness of MCT and CBT-based interventions for QoL in individuals with schizophrenia spectrum disorders (SSD). Three electronic databases were screened up to October 2025 for MCT and CBT studies reporting QoL outcomes for patients diagnosed with SSD. Data from 38 randomized controlled trials (RCTs) met the inclusion criteria, of which eight MCT and 30 studies included CBT-based interventions. When comparing MCT against control interventions, none of the eight MCT studies found significant differences in the overall score of QoL scales at posttest and at 1- or 6-month follow-up. Only one study has assessed the effects of MCT after 3 years, yet it demonstrated significant improvements in QoL. For CBT-based interventions, nine of 26 available between-group comparisons in overall QoL showed a significant improvement in QoL after CBT at the posttest, and five of 19 effects were observed at follow-up. This review provides modest evidence for improvements in QoL from CBT-based interventions and a small number of studies concerning QoL in MCT, emphasizing the need for further research that prioritizes QoL as a primary outcome as well as the need for research with longer follow-ups to capture the delayed effects of MCT and CBT on QoL. It awaits to be tested whether the recent extension of MCT (app, 2 modules on self-esteem and stigma) augments QoL.

Background

To examine whether positive self-evaluation and contextual variables—such as those created by family, peers and schools—serve as promotive factors or protective factors in the context of adversity.

Methods

Data were drawn from two waves of the youth survey of a population-representative cohort in Hong Kong. Adolescents aged 10–14 (n = 3475) completed questionnaires about positive factors, adverse experiences and mental health at Wave 1 from March 2009 to April 2011, and 2696 of them were followed up at Wave 2 from August 2011 to March 2014. Those who completed both waves were included in the analysis (n = 2171).

Results

Positive self-evaluation, high family cohesion, positive peer relationship and positive school experience were statistically significantly associated with better behavioural well-being at Wave 2, with incidence rate ratios (IRRs) of 1.01. Only positive self-evaluation (IRR: 1.02, 95% CI 1.01–1.03) and positive school experience (IRR: 1.01, 1.00–1.02) were associated with better emotional well-being in the fully adjusted models. We identified a statistically significant interaction between life stress and positive peer relationships in relation to behavioural well-being (p = .009). The association between positive peer relationships and behavioural well-being was only found among adolescents with ≥3 stressful events (IRR: 1.02, 1.01–1.04).

Conclusion

Adolescents who possessed a positive sense of self and were nurtured in positive family, peer and school environments tended to exhibit better behavioural or emotional well-being. Interventions should collaborate with families and schools to actively promote positive self-evaluation and environments, with a particular emphasis on fostering positive peer relationships to enhance the health and well-being of adolescents.
