Abstract

Sexual dysfunction in young people is on the rise. This trend has dramatic implications for key state interests, including fertility rates, economic growth, and overall well-being. The serious consequences associated with poor sexual health in childbearing-age citizens call for creative solutions. One such solution is Surrogate Partner Therapy (“SPT”), a highly effective form of treatment for people suffering from severe cases of psychogenic sexual dysfunction. SPT establishes a triadic relationship among the patient, a licensed mental health professional, and a trained surrogate partner who may use intimate contact to instill patients with the skills necessary to form healthy connections. However, because its unorthodox methods likely violate state anti-prostitution laws, SPT is rarely utilized. This article argues that, given SPT’s promising potential to address the rise of psychosexual ailments in young people, state governments should create a limited medical exception to their prohibitions on prostitution.

ABSTRACT

Three hundred and eighteen in-service teachers from Greece, Cyprus, Romania and Hungary participated in a professional training program aimed at promoting their metacognitive and epistemic thinking and practices, with argumentation as a prominent teaching practice. Pre-program survey results revealed that teachers’ personal and teaching epistemic practices were underdeveloped, with particularly low engagement in the latter. Personal epistemic practices predicted teaching ones. Cultural differences emerged, with Greek and Cypriot teachers reporting higher engagement in both epistemic practices than Romanian and Hungarian teachers. In-depth interviews with six Greek and Cypriot teachers who implemented argumentation post-program indicated that argument-based practices improved students’ epistemic understanding, particularly in appreciating multiple perspectives, evaluating sources and using evidence. Teachers acknowledged receiving limited preparation for promoting students’ epistemic understanding and argumentation skills. These findings highlight the need for higher education programs to develop pre- and in-service teachers’ epistemic practices. Implications for higher education and policymakers are discussed.

Background: African adolescents and young adults account for a disproportionate number of new HIV infections. There is an urgent need to identify scalable and cost-effective behavioral HIV prevention strategies for this population. Using a condom at first sex is associated with a higher likelihood of consistent use later. Tumaini (“Hope for the Future” in Swahili; Emory University) is a choose-your-own-adventure smartphone game that has been shown to reduce the risk of unprotected first sex by end line in a 45-month randomized controlled trial in western Kenya. Objective: This study aimed to assess the impact of Tumaini on proximal outcomes related to condom use at first sex (specifically, behavioral intentions, self-efficacy, attitudes, and knowledge) longitudinally across mid-adolescence in the above trial. Methods: Adolescent participants (n=996, mean baseline age 14, SD 0.56 years) were randomized 1:1 to receive either a smartphone loaded with Tumaini or an attention-control math game for 5 to 7 weeks at 3 time points (mean age 14.0, SD 0.56; 15.3, SD 0.55; and 16.0, SD 0.56 years, respectively). They completed a behavioral survey at 13 time points, through mean age 17.7 (SD 0.56) years. Using generalized estimating equations and controlling for age at baseline, we modeled mean scores (overall and stratified by gender) on a range of condom-related survey items over time to assess mean differences at specific time points. We applied appropriate Bonferroni corrections to inferences about cross-arm differences in mean changes relative to baseline at 4 time points (after each intervention period and at end line; α=.05/4) and within-arm mean changes relative to baseline at each of the 12 post-baseline time points (α=.05/12). Analyses were conducted as intent-to-treat. Results: At end line, 97.8% (n=974) of the sample had been retained. Participants in both arms dedicated a mean total of >30 hours to their assigned game. There was significant improvement across all condom-related proximal outcomes in the intervention arm relative to the control arm immediately after initial intervention exposure. For almost all outcomes, a significant cross-arm difference was also present at end line and for most outcomes at the 2 intervening comparison time points. Some outcomes saw stronger intervention effects on female participants (eg, self-efficacy to refuse unprotected sex) or male participants (eg, knowledge that condoms are an effective way to prevent HIV). In each arm, intention to use a condom at first sex was consistently higher among male participants; however, female intervention-arm scores overtook male control-arm scores following initial intervention exposure. Conclusions: Tumaini significantly improved theory-based proximal outcomes related to condom use, with effects sustained 45 months post initial exposure and 16 months post most recent exposure. Adolescents benefited from even short-term exposure, though repeated exposure generally sustained and reinforced intervention effects. As access to smartphones increases, Tumaini has potential for high scalability and impact on condom-related outcomes. Trial Registration: ClinicalTrials.gov NCT04437667; https://clinicaltrials.gov/study/NCT04437667

ABSTRACT

A survey by the IfD Allensbach Institute for Public Opinion Research yielded that 60% of German deputies to different parliaments (members of the German National Parliament, the State Parliaments and the European Parliament) consider the social conditions in Germany to be just, while 28% of the population do so. Also, 56% of the population, but 28% of deputies, consider their country unjust. Based on the Justice Motive Theory and research, the current study aimed to explore whether German politicians and the voting public differ in their just-world beliefs and perceptions of justice. These beliefs and perceptions of 116 members of the 18th German National Parliament and 998 citizens (a representative sample of the German population) were analysed by latent structural equation modelling, including randomly drawn subsamples of 116 citizens each. The politicians indicated significantly lower beliefs in a just world, an unjust world, immanent justice, ultimate justice, perceived performance and equality distributive-justice principles than the citizens. However, the citizens indicated a significantly lower belief in justice according to an individual’s needs than the participating members of the German national parliament. The findings offer insights into how distributive justice principles are perceived across societal roles.

Objectives

Individuals with post-traumatic stress disorder related to childhood maltreatment (PTSD-CM) show limited response to treatment on pre-post-symptom measures. While a nuanced understanding of nonresponse is crucial for improving treatment, quantitative measures may not fully capture clinically meaningful processes of change from the patients’ perspectives. We therefore explored how individuals with PTSD-CM who showed no or limited numerical improvement after trauma-focused psychodynamic psychotherapy (TF-PDT) experienced change.

Design

This qualitative study was embedded in a large randomized controlled trial on PTSD-CM (DRKS00021142).

Method

From 75 qualitative post-treatment interviews, we purposively sampled eight TF-PDT recipients meeting criteria for numerical nonresponse, defined as current PTSD diagnosis and less than 50% reduction in the Clinician-Administered PTSD Scale for DSM-5 (CAPS-5). Client Change Interviews were analysed using Critical-Constructivist Grounded Theory.

Results

We derived two clusters (‘Progress—A Double-Edged Sword’, ‘When Building Trust Collides with the Therapeutic Framework’) and one overarching core category (‘Unfinished Business’), which captured a dialectic process. Within a responsive therapeutic relationship, patients began confronting their relational trauma, experiencing relief, increased emotional regulation and hope. However, this very engagement surfaced emotional challenges that exceeded what could be addressed within the limited therapeutic framework, leaving the process meaningful but unfinished.

Conclusion

What is often labelled as numerical nonresponse in PTSD-CM may reflect a dynamic interplay of emerging improvement and emotional distress that needs more time to unfold. Moreover, the results underscore the value of integrating qualitative, patient-reported outcomes into treatment evaluation and relationship-focused treatment tailored to patients’ individual needs.

ABSTRACT

Background

Thailand faces rapid demographic ageing and limited formal support for persons with intellectual disabilities. As this population lives longer, ageing family caregivers must prepare for future care.

Methods

Semi-structured interviews were conducted with nine caregivers of adults with intellectual disabilities in four Thai provinces. The adults with intellectual disabilities were aged over 40 years. Data were analysed using Braun and Clarke’s thematic analysis.

Results

The analysis identified key themes reflecting reliance on family-based care, present-focused coping, financial insecurity, limited planning knowledge, caregiver self-care and the need for respite support.

Conclusions

Most families lacked clear future care plans due to limited services, financial constraints and cultural values. While a present-focused mindset provided comfort, it often delayed planning. The findings highlight the need for stronger financial support, employment opportunities, planning resources and respite services.

ABSTRACT

Objective

This study examines how parental housework time and labor market participation during early childhood are associated with adolescents’ housework participation.

Background

Prior research shows that parental time use is associated with adolescents’ housework in gendered ways. Drawing on gender performance theory, new home economics, and the cumulative disadvantage framework, this study assesses whether adolescents’ housework participation is associated not only with contemporaneous parental behavior but also with parental behavior during early childhood.

Method

Using longitudinal data from the China Family Panel Study (N = 1637), regression-with-residuals models estimate associations between parents’ housework time and labor market participation during preschool (ages 0–5), early school years (ages 6–9), and adolescence (ages 10–15) and adolescents’ housework participation.

Results

Mothers’ housework hours during preschool were positively associated with girls’ adolescent housework participation but not boys’. Fathers’ housework participation during adolescence showed a stronger contemporaneous association with boys’ housework time. Fathers’ labor market participation during adolescence was associated with higher housework participation among girls only. Mothers’ labor market participation during early school years was also positively associated with girls’ adolescent housework participation.

Conclusion

Girls’ housework socialization appears to begin in early childhood, whereas boys’ housework participation is more strongly linked to contemporaneous parental behavior. Interventions aimed at reducing domestic gender inequality may benefit from targeting the preschool years.

ABSTRACT

Many features of personality psychopathology are shared among patients with Anorexia Nervosa (AN), yet the impact of these shared features is not fully understood, and the impact of having more personality psychopathology features during adolescent onset of AN, especially among adolescents admitted to an inpatient eating disorder unit, has not been explored. This study evaluated how personality psychopathology, utilizing a measure of borderline personality disorder (BPD) symptom severity, has a potential predictive value in understanding risk of readmission for an eating disorder over time. A total of 256 adolescents were admitted for medical stability to an inpatient unit for the first time. Adolescents completed the Borderline Personality Features Scale for Children-11 (BPFSC-11) within the first 14 days of admission; demographic variables, anxiety and mood comorbidities, and readmission status were obtained through chart review. Higher scores on the BPFSC-11 were significantly associated with being readmitted within 1 year of discharge from the inpatient unit. These findings highlight how personality psychopathology features at onset of severe AN in adolescence could indicate a higher risk for recovery complications, including repeated inpatient admission. These findings may underscore the role of supplementary therapeutic interventions alongside family-based therapy, including dialectical behavior therapy.

Who counts as white in the United States? This has been a demographic puzzle since US citizenship was first limited to “free white persons” in 1790. Over time, immigrants from various world regions began to comprise the white population, creating a numerical majority and what is now the widely used reference category for measuring racial inequality. This review traces the evolving composition of the white racial category from the 1960s to the present, highlighting increased immigration from Eastern Europe and the Middle East as key sources of diversity within the federally defined white population. I organize the review into three sections. The first section reviews the historical construction of the white racial category in relation to the federal classification system used by the US Census. The second section examines compositional changes in the global origins of white immigrants since the 1965 Immigration and Nationality Act. It highlights how geopolitical events have shaped divergent migration and resettlement experiences for immigrants from Eastern Europe and the Middle East—groups who are legally classified as white but often racialized as “not quite white.” The third section reviews research on immigration and health to illustrate how compositional changes in the white population affect our understanding of US health disparities. The conclusion considers the implications of treating whites as a single, monolithic group and calls for greater analytic attention to within-group diversity among whites in studies of inequality.

ABSTRACT

Introduction

The travel industry has a responsibility to accommodate the needs of all its customers, including those with obesity. It is not known to what extent airlines communicate accessibility policies to passengers with obesity. We sought to assess the adequacy and content of information provided by international commercial airlines regarding the carriage of passengers with obesity.

Methods

A descriptive cross-sectional analysis was conducted of policies relating to passengers with obesity that were available on publicly accessible websites of the 50 busiest global commercial airlines. Variables of interest included fleet description, passenger weight-related terminology, seating information, use of seatbelt extenders, special assistance information, use of artificial intelligence chatbots for customer service, and availability of customer information in the English language.

Results

A diverse range of terms was used to describe travelers with obesity, with “customers requiring extra personal space” or “an extra seat for personal comfort” being the most frequently used descriptors. Seating information relating to larger passengers was provided by 86% (n = 43) of airlines. Passenger guidance on the use of seatbelt extenders was available on 70% (n = 35) of airline websites. Special assistance information was provided by all airlines.

Conclusion

While leading commercial airlines provide obesity policy information on their websites, there is considerable variation between carriers in the degree of information provided. There is a need for more transparent and standardized accessibility information for airline passengers with obesity.
