Abstract

Background and aims

Gypsy and Traveller communities in the United Kingdom (UK) face substantial health challenges. Smoking tobacco and drinking alcohol likely contribute to health disparities, but there is little national data on the prevalence or heaviness of smoking and drinking among these communities. We aimed to estimate the prevalence and heaviness of smoking and drinking among the UK Gypsy and Traveller communities compared with people from other UK ethnic groups.

Design/setting

Observational study using data collected between 2013 and 2025 in a series of monthly cross-sectional surveys of representative samples of the adult population in England.

Participants

Adults aged 18+, between 2013 and 2025 (total n = 226 339; Gypsy or Traveller n = 213).

Measurements

Marginal means were derived from regression models and used to estimate the prevalence of current smoking and drinking (both of which includes daily and non-daily), and the heaviness of smoking (cigarettes per day) and drinking [Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) score, units per week and estimated weekly alcohol consumption] by ethnicity; adjusted for age, gender and survey year.

Findings

Current smoking prevalence was markedly higher among Gypsy and Travellers [33.0%, 95% confidence interval (CI) = 26.3–39.8%, n = 81] than among the “Other White” ethnic group (18.7%, 95% CI = 18.5–18.9%; P < 0.001), and exceeded estimates observed across other ethnic groups (range: 9.4–19.9%; all P < 0.001). Among those who smoked, Gypsy and Travellers reported smoking more cigarettes per day (geometric mean = 12.5, 95% CI = 8.9–17.6) than the “Other White” group (geometric mean = 9.0, 95% CI = 8.8–9.1; P = 0.59), with other ethnic groups ranging from 6.8–9.7, although not all comparisons reached significance. The proportion reporting any current alcohol consumption was lower among Gypsy and Travellers (61.5%, 95% CI = 53.8–69.2%, n = 126) than ‘Other White’ ethnicities (77.1%, 95% CI = 76.8–77.3%; P < 0.001). Prevalence of high risk drinking was similar among Gypsy and Travellers (11.3%, 95% CI = 6.7–15.8%) and “Other White” ethnic groups (10.7%, 95% CI = 10.5–10.9%; P = 0.808) but exceeded estimates observed across all other ethnic groups (range: 1.3–7.1%; all P < 0.05). Prevalence of possible dependence was higher among Gypsy and Travellers (3.5%, 95% CI = 0.4–6.6%) compared with the “Other White” (1.3%, 95% CI = 1.2–1.3%; P = 0.027) and all other ethnic groups (range: 0.3–0.8%; all P < 0.05).

Conclusions

People from Gypsy and Traveller communities in the United Kingdom appear to be more likely to smoke compared with other UK ethnic groups, and those who smoke and/or drink do so at more harmful levels compared with other UK ethnic groups.

ABSTRACT

This study presents an 11-year content analysis of 1514 counseling faculty job postings advertised on the Counselor Education and Supervision Network (CESNET) from 2014 to 2024. Using a deductive content analysis design, we systematically examined trends across six dimensions: annual volume, academic rank, position type, geographic region, Council for Accreditation of Counseling and Related Educational Programs (CACREP) accreditation status, and work modality. Overall, the counseling faculty job market demonstrated marked volatility, with an early surge likely tied to the implementation of the CACREP 2016 standards, followed by pandemic-related contraction in 2020 and partially recovered thereafter. Assistant professor roles consistently dominated, while senior-level positions were sparse and often absent in recent years. Tenure-track positions remained the primary hiring pathway, though non-tenure track and clinical roles grew substantially. Geographic hiring was heavily concentrated in the Southern and North Central Association for Counselor Education and Supervision (ACES) regions, and the Rocky Mountain region remained consistently underrepresented. The majority of the postings originated from CACREP-accredited programs, and transparency in reporting accreditation status increased over time. Explicit in-person postings rose over the past decade, while flexible in-person and/or remote roles peaked in 2021. The proportion of postings omitting modality declined. Collectively, these trends reflect a faculty labor market that remains anchored in traditional hiring yet is increasingly diversified, raising implications for counselor education practice and research.

ABSTRACT

The full-scale Russian invasion and the ongoing war have a significant impact on the Ukrainian higher education system, which is primarily related to material losses caused by damage and destruction of universities and infrastructure, decreased funding, the departure of some students and university teachers abroad, decreased research productivity, and educational losses due to studying in difficult conditions. This study compares data from the pre-war year (2021) with the period following the onset of the full-scale invasion (2022–2024) across affected and unaffected institutions in order to evaluate the war’s effects on Ukrainian universities using a Difference-in-Differences approach. The results indicate significant losses in conflict-affected regions, with universities experiencing declines in public funding, student numbers, increased faculty attrition, and reduced academic productivity. The study suggests cluster-based strategies for the post-war recovery of Ukrainian universities, given the diversity of institutional profiles, needs, and capacities.

Abstract

This systematic review and meta-analysis aimed to evaluate the impact of narrative interventions on self-efficacy and self-management behaviors in patients with chronic diseases. We systematically searched EMBASE, the Cochrane Library, CINAHL, PsycINFO, China National Knowledge Infrastructure (CNKI), and PubMed from conception to 2025, supplemented by manual searches of reference lists. Randomized controlled trials (RCTs) assessing the effects of narrative interventions on self-efficacy or self-management in adults with chronic diseases were included. Data were pooled using random-effects meta-analysis. A total of 34 RCTs involving 4584 participants were included. The meta-analysis showed that narrative-based interventions significantly enhanced patients’ self-efficacy (22 studies; SMD = 0.66, 95% CI: 0.35–0.96, p < .001; I
<sup>
2
 = 59.6%) and self-management behaviors (12 studies; SMD = 1.73, 95% CI: 1.03–2.42, p < .001; I
<sup>
2
 = 85.1%). Meta-regression indicated a positive regulatory trend of national income level on self-efficacy (p = .055), although the effect was not statistically significant, and no significant moderating effect was observed for self-management. Other variables—including age, disease type, intervention type, duration, and publication period—showed no significant moderating impact on either outcome (p > .05). Subgroup analysis further revealed that the improvement in self-efficacy was significantly greater in upper- to middle-income countries (SMD = 1.04) than in high-income countries (SMD = 0.39; subgroup difference p = .019), and the effect was significant among patients with heart disease (SMD = 1.16), cancer (SMD = 0.98) and stroke (SMD = 0.71). For self-management, the effect varied significantly across subgroups defined by intervention type, duration, publication period and disease type, with the largest effects observed among patients with neurological diseases (SMD = 3.16) and cancer (SMD = 2.73). The results of this systematic review and meta-analysis suggest that narrative-based interventions may improve self-efficacy and self-management behaviors among patients with chronic diseases. However, considerable heterogeneity remains, and further high-quality studies are needed to confirm these findings.

Abstract

For over 10 years, schools have been responsible for providing career support to all young people. In order for schools to be able to deliver career support in the classroom, the career leader role was introduced as part of the Education Act 2011. With increasing numbers of disabled students attending mainstream school, a key challenge encountered is ensuring that the support provided is inclusive and accessible to all young people. This article provides an overview of a case study which brought together eight educational professionals working across mainstream and specialist provisions. It was hoped that by providing space for these educational practitioners to share practice, they could both reflect on their own experiences and identify ways to develop more inclusive practice when providing career support to young disabled people. As with many of the professionals working within the current English education system, the career leaders demonstrated their commitment to ensuring all young people have opportunities to successfully transition from school; however, they find themselves constrained by a system that is not working. This article therefore highlights some key recommendations for addressing the barriers that are encountered when supporting young people to successfully transition from school.

ABSTRACT

Informal rules are an underexplored component of collaborative governance, yet they play a critical role in digital transformation projects. This study examines their functions and constraints through a longitudinal case study of the Hui-Tian City Brain project in Beijing. Drawing on government documents, media reports, and interviews with officials, IT enterprises, and civil servants, we trace the emergence and evolution of the informal ‘product recommendation meeting’ rule. This rule streamlined deliberation by reducing information asymmetries, lowering coordination costs, and facilitating joint action across sectors. However, its effectiveness was constrained by the high transaction costs of repeated face-to-face meetings and the limited, invitation-based caucus structure of collaboration. The findings show that informal rules can both enable and restrict collaboration in digital transformation. We argue for greater attention to informal institutional arrangements in understanding and designing collaborative governance.

ABSTRACT

Mental imagery has been identified as a key feature of the onset, maintenance and treatment of psychological disorders. Research on the role of mental imagery in body dysmorphic disorder (BDD), a condition hallmarked by negative sensory appraisals of the self, has been increasingly recognised in theoretical perspectives and psychological interventions. However, the scope and implications of this work have not yet been reviewed. This systematic review sought to identify the characteristics and proposed mechanisms of imagery in BDD, synthesising qualitative and quantitative data using Meta-Integration. Quality was assessed using the Mixed Methods Appraisal Tool. Thirty-seven studies were identified among 33 publications. Study quality was mixed with significant methodological heterogeneity. Mental imagery in BDD is consistently reported to be vivid, emotionally intense, recurrent and important in the maintenance and potentially the onset of BDD. These findings concur with theoretical frameworks of BDD (and other related conditions) which highlight the causal role of imagery and encourage the use of imagery-based interventions. Crucial areas for future work include stronger causal tests, unpacking mechanisms, attention to individual differences and intersectionality and exploring the potential for imagery-based approaches for innovations in treatment and prevention across the lifespan, particularly in adolescence when BDD first develops.

ABSTRACT

Objective

Over the past 60 years, the proportion of highly processed foods rich in refined carbohydrates and added fats in the food environment has increased, coinciding with a rise in binge eating. While past research has focused on individual vulnerabilities for binge eating, emerging evidence suggests highly processed foods may exacerbate these vulnerabilities. This Research Forum addresses a critical research gap by synthesizing the foods reported in binge-eating episodes across 41 studies and identifying limitations in current food-reporting practices.

Method

Systematic searches were conducted in PubMed, Google Scholar, and PsycINFO through February 2025. Eligible studies reported specific foods consumed during binge-eating episodes among human participants. Foods were coded as highly processed based on the presence of refined carbohydrates and/or added fats.

Results

All 41 (100%) eligible studies reported at least one highly processed food in binge-eating episodes. Only 31.7% of studies (n = 13) reported minimally processed food consumption in a binge-eating episode. Of the food types reported across all studies (n = 404), 70.3% were classified as highly processed and 14.9% as minimally processed. However, vague food descriptions limited the ability to categorize many foods. The most frequently reported foods were cake, ice cream, cookies, and chocolate.

Discussion

Across published studies, highly processed foods were highly prevalent in binge-eating episodes, but interpretations are constrained by vague and incomplete food reporting. We outline recommendations for standardized food-reporting practices to improve the characterization of foods reported in binge-eating occasions to strengthen the clinical relevance of future research.
