                        <p><h2>Background</h2>
<p>Prospective and retrospective measures of childhood maltreatment often identify different individuals and are differentially associated with psychopathology. This study examines psycho-social factors that may explain discrepancies between these measures.</p>
<h2>Methods</h2>
<p>Data were drawn from the Environmental Risk Longitudinal Twin Study, a nationally representative birth cohort of 2,232 children born in 1994–1995 across England and Wales and followed to age 18 (93% retention). Childhood maltreatment was assessed through: (a) prospective assessments from caregivers, researchers, and clinicians at ages 5–12, and (b) retrospective self-reports at age 18 using the Childhood Trauma Questionnaire (for maltreatment occurring up to age 12). For the analyses, we focused on participants identified as maltreated from either measure (<i>n</i> = 290) and an a-priori selected array of potential explanatory variables assessed between ages 5–18. We conducted two sets of analyses: comparing individuals with only prospectively identified maltreatment to those identified by both prospective and retrospective measures to understand why some participants did not retrospectively report or recall maltreatment; and comparing individuals with only retrospective self-reports to those identified by both prospective and retrospective measures to understand why maltreatment had not been detected prospectively.</p>
<h2>Results</h2>
<p>Participants in the prospective-only group reported greater social support over the life course and lower psychopathology at age 18 compared to those identified through both prospective and retrospective measures. Individuals in the retrospective-only group had higher socioeconomic status, higher self-reported adult involvement at age 12, and less exposure to domestic violence compared to those identified through both prospective and retrospective measures.</p>
<h2>Conclusions</h2>
<p>Our findings suggest that perceptions of social support and better mental health may buffer retrospective recall of childhood maltreatment in those with prospective measures. Furthermore, more positive family functioning and socioeconomic factors may hamper prospective detection of childhood maltreatment in those who retrospectively report it.</p>

                        <p><h2>ABSTRACT</h2>
<p>Mental health therapists (MHTs) providing care during shared traumatic reality face distinct emotional and professional challenges, particularly when they are also personally affected. While a central concern is compassion fatigue, emotional exhaustion that arises from prolonged exposure to others’ trauma, this concept does not capture the dual nature of the MHT’s traumatic experience, both as a citizen of the region and as a helping professional exposed to the traumatic narratives of their clients. This study explores the shared traumatic reality experiences of MHTs working in a war zone, offering support to trauma survivors while navigating their own exposure to the same crisis. Through qualitative, semi-structured interviews with 20 therapists conducted between April and July 2024, three key themes emerged: (1) We’re All in the Same Boat—When Boundaries Blur, reflecting how shared traumatic reality erodes traditional therapeutic roles; (2) The Collapse of Everyday Concerns—When Nothing Feels Important Unless It’s Related to the War, illustrating how therapists deprioritised non-trauma issues and experienced emotional detachment; and (3) Therapy as Shared Emotional Support, highlighting a shift towards mutual validation rather than structured interventions. From these findings, we propose a model conceptualising the dual impact of shared traumatic reality on therapists: boundary dissolution can both increase distress and foster resilience, especially when cultural values of solidarity and mutual responsibility are present. Although rooted in one cultural context, this model underscores the importance of identifying community-level resilience factors that help MHTs maintain professional functioning during collective crises and when operating in war zones and other high-stress environments.</p>

<p>We first conducted a randomised controlled superiority trial to test whether the web-based Parents and Babies programme (Toi, Moi, Bébé; TMB) delivered with motivational telephone support (ie, coaching) was superior to the fully automated programme (ie, self-help). TMB incorporated classic and third-wave cognitive–behavioural therapy components and psychoeducation. Then, we tested whether TMB (both treatment modalities combined) was superior to usual care. The usual care comparison group was drawn from the CONCEPTION prospective pan-Canadian perinatal cohort (N=592).</p>

<sec><st>Setting</st></sec></p>
<p>A remote study based at Sainte-Justine Hospital Centre, Quebec, Canada.</p>

<sec><st>Participants</st></sec></p>
<p>Web-based intervention programme participants were women aged ≥14 years at 12–25 weeks’ gestation, with subclinical to moderate clinical Edinburgh Postnatal Depression Scale (EPDS) scores: 9–16. Exclusion criteria were psychosis and self-reported substance abuse. The usual care comparison group was pregnant women ≥18 years old. All participants were living in Canada at study inception.</p>

<sec><st>Main outcome measures</st></sec></p>
<p>The primary outcome was EPDS scores at 3 months post partum, accounting for baseline EPDS scores and depression events defined as EPDS ≥13 at 3 months post partum. The secondary outcomes were EPDS scores at 6 months post partum, depression events (EPDS≥13) at 6 months post partum, anxiety symptoms (Generalised Anxiety Disorder 7-item Scale, GAD-7) at 3 and 6 months post partum, accounting for baseline scores for the continuous outcomes; as well as the number of completed intervention modules and well-being scores (WHO 5-Item Well-being Index) at 3 months post partum.</p>
<p><br>
<sec><st>Results</st></sec></p>
<p>We randomised 510 participants to TMB self-help (n=255) or TMB with coaching (n=255); 211 and 214 participants, respectively, were included in the complete-case intention-to-treat analyses. At baseline, 91% lived with a partner, 71% were university graduates and 42% self-reported GAD-7≥10. Randomisation was successful. First, TMB with coaching was not superior to TMB self-help: at 3 months post partum, EPDS scores were TMB self-help (mean 8.0±4.3) vs TMB with coaching (mean 8.6±4.5); effect size was 0.01 (95% CI 0.00 to 0.03; p=0.16). Second, TMB (regardless of intervention arm) was superior to usual care: in adjusted regression models, EPDS scores were 6.2 units lower (per SD, 95% CI –8.2 to –4.3) in TMB (both treatment modalities combined) than in usual care; and proportions of depression events were 4.7 units lower (per SD on the logit scale, 95% CI –6.6 to –2.7) in TMB (combined) than in usual care. No other group differences were observed.</p>
<p><br>
<sec><st>Conclusions</st></sec></p>
<p>Our findings suggest that, in women with subclinical to moderate clinical antenatal depressive symptoms, receiving a web-based cognitive–behavioural therapy-based programme in addition to usual care can reduce depression postnatally.</p>

<h2>Objective</h2>
<p>This research validated the Italian version of the 40-item Caregiving Experiences Questionnaire (CEQ) by examining its factor structure and associations with maternal burnout and child dysregulation.</p>
<h2>Background</h2>
<p>Caregiving representations shape parental behavior and influence child development. Existing measures are resource intensive, limiting accessibility. The CEQ offers a practical self-report alternative, but its validation remains limited.</p>
<h2>Method</h2>
<p>Two studies involved independent samples of Italian cisgender, heterosexual, biological mothers. Study 1 assessed the CEQ’s factor structure, reliability, and measurement invariance across child gender, developmental stage, and maternal education (<i>N</i> = 891; <i>M</i><br>
<sub>age</sub> = 41.48, <i>SD</i> = 6.76). Study 2 used structural equation modeling to assess relations among caregiving, maternal burnout, and child dysregulation among 743 mothers (<i>M</i> = 42.31 years, <i>SD</i> = 6.64) of children mean aged 8.76 (<i>SD</i> = 4.99).</p>
<h2>Results</h2>
<p>A four-factor, 29-item model (Enjoyment, Helplessness, Heightened Caregiving, Role Reversal) showed strong psychometric properties and invariance across subgroups. Maternal burnout mediated the relations between enjoyment, heightened caregiving, and role reversal and child dysregulation. Helplessness exerted a direct effect.</p>
<h2>Conclusion</h2>
<p>The Italian CEQ is a valid and reliable tool for assessing maternal caregiving, relevant across developmental and demographic contexts.</p>
<h2>Implications</h2>
<p>Interventions targeting maternal caregiving and burnout may reduce child dysregulation.</p>

<h2>Background</h2>
<p>Given their modifiable nature, understanding the intrapersonal factors involved in the cessation of non-suicidal self-injury (NSSI) is needed to inform existing therapeutic approaches and explore novel avenues for supporting individuals with NSSI. This study aimed to examine the relative contributions of and interactions among intrapersonal factors (self-concept, emotion regulation, and cognitive flexibility) in distinguishing between ceased and current NSSI.</p>
<h2>Methods</h2>
<p>Two hundred and two adults aged 18–71 years (<i>M</i> = 31.50, SD = 11.23; 74.75% female) were surveyed to compare facets of self-concept, emotion regulation, and cognitive flexibility that contributed most to discriminating those who had ceased NSSI for at least 1 year from those who continued. These intrapersonal factors were also explored as potential indirect pathways between identity and NSSI status.</p>
<h2>Results</h2>
<p>Significant differences between ceased and current NSSI groups were observed across all intrapersonal factors, with moderate to large effects. Most differences persisted after accounting for psychological distress. Less identity fragmentation, greater emotional clarity, internal locus of control, self-liking, access to emotion regulation strategies, and impulse control contributed most to discriminating NSSI cessation after accounting for psychological distress. The relationship between lack of identity and NSSI status operated indirectly through lack of emotional clarity and self-liking.</p>
<h2>Conclusions</h2>
<p>Having a coherent sense of identity, emotional clarity, internal locus of control, self-liking, access to emotion regulation strategies, and managing impulsivity relate to NSSI cessation. These modifiable factors denote several promising intervention opportunities and should be considered in the design of NSSI-specific treatments.</p>

                        <p><p>The role of sexual transmission of viral, bacterial and parasitic pathogens in lower genital tract infections in women is well established. Although the sexual transmissibility of bacterial vaginosis (BV)-associated bacteria has historically been debated, recent evidence has substantially clarified this issue. A landmark randomised controlled trial by Vodstrcil <i>et al</i> provided compelling evidence that dual-antimicrobial treatment of male sexual partners significantly reduced BV recurrence in women, strongly supporting that sexual transmission of BV-associated pathogens occurs.<cross-ref type="bib" refid="R1">1</cross-ref> In this issue of the <i>Sexually Transmitted Infections</i> journal, the very latest extensive retrospective epidemiological study from the same Melbourne Sexual Health Center group (Sutton <i>et al</i>) further confirms the importance of sexual transmission in BV pathogenesis, particularly in recurrent disease.<cross-ref type="bib" refid="R2">2</cross-ref></p>

                        <p><p><sec><st>Objectives</st></sec></p>
<p>Long-acting injectable cabotegravir (LACAB) has proven to be superior to oral pre-exposure prophylaxis (PrEP); however, it represents an issue for PrEP management, given the relevant burden on clinic workload. This study aimed to describe (1) the proportion of users requiring LACAB for medical issues, (2) the proportion of users who wish to switch to LACAB and (3) the factors associated with prioritising LACAB based on a combination of medical and psychological needs.</p>
<p><sec><st>Methods</st></sec></p>
<p>Toxicity, adherence and efficacy issues were evaluated in PrEP users attending a community-based service. The same population underwent an online survey to measure their desire for and reasons for switching to LACAB. An adjusted model was developed to identify variables associated with both medical needs and interest in LACAB.</p>
<p><sec><st>Results</st></sec></p>
<p>A total of 1056 individuals were assessed for LACAB medical needs, and 27.8% were eligible, primarily due to adherence issues. Of the 419 survey respondents, 74.9% expressed interest in LACAB. Age older than 25 years, use of any other concomitant medication and reporting sexual encounters without PrEP or condom use were associated with both the medical need for LACAB and interest in this new preventive strategy.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>The need for and desire to use LACAB are significant. The health system needs to be implemented to address the issues posed by this novel approach.</p>

                        <p><p><sec><st>Background</st></sec></p>
<p>Female sex workers (FSWs) in Zimbabwe, where sexually transmitted infections (STIs) are managed syndromically, are at high risk of STIs and HIV.</p>
<p><sec><st>Methods</st></sec></p>
<p>We conducted a respondent-driven sampling (RDS) survey to assess the prevalence, engagement in HIV care and factors associated with STIs among FSWs in two cities of Zimbabwe. Participants self-completed an audio computer-assisted self-interview. Participants were offered HIV testing, and if positive, were tested for HIV viral load. Additionally, 45% were randomly selected and screened for syphilis, gonorrhoea (GC), chlamydia (CT) and trichomonas (TV). RDS-II weighted HIV and STI prevalence and 95% CIs were calculated. The HIV treatment cascade was determined for FSWs living with HIV. Factors associated with having at least one STI (GC/CT/TV) were determined using modified Poisson regression.</p>
<p><sec><st>Results</st></sec></p>
<p>We recruited 3006 FSWs from Harare and Bulawayo. HIV prevalence was 41.3% (95% CI 38.1% to 44.6%). Among FSWs living with HIV, 83.4% (95% CI 79.0% to 87.0%) reported being aware of their status. Of those aware, 97.3% (95% CI 94.4% to 98.8%) reported being on antiretroviral therapy (ART), and of those on ART, 93.1% (95% CI 89.3% to 95.7%) were virally suppressed. Overall, 90.1% (95% CI 86.6% to 92.7%) of FSWs living with HIV were virally suppressed. The prevalence of active syphilis was 4.4% (95% CI 3.2% to 6.0%), gonorrhoea was 13.4% (95% CI 11.3% to 15.9%), CT was 20.7% (95% CI 18.2% to 23.5%) and TV was 23.7% (95% CI 21.0% to 26.6%). Of the 598 FSWs with a current STI, 52.4% (95% CI 47.4% to 57.3%) did not have HIV infection. Factors associated with increased relative risk of having at least one STI, after adjusting for age and city, included being divorced/separated, living with HIV and reporting STI symptoms in the past year.</p>
<p><sec><st>Conclusion</st></sec></p>
<p>While there have been huge gains in identifying and treating FSWs living with HIV, many STIs remain undetected, and a significant proportion of HIV-negative FSWs are affected by these infections, suggesting an urgent need for improved STI management including access to cheap point of care diagnostics.</p>

                        <p><p>Syphilis is a sexually transmitted infection caused by the spirochaete <i>Treponema pallidum</i>. The standard treatment for early syphilis is a single dose of intramuscular benzathine penicillin G (BPG), while doxycycline for 14 days is an alternative.<cross-ref type="bib" refid="R1">1 2</cross-ref><cross-ref type="bib" refid="R2"></cross-ref> However, during global antibiotic shortages, doxycycline may be unavailable,<cross-ref type="bib" refid="R3">3</cross-ref> and no universally accepted alternative treatment exists for patients with syphilis with penicillin allergy. Although minocycline, another tetracycline agent with a similar antimicrobial spectrum, is widely used for syphilis treatment in Japan,<cross-ref type="bib" refid="R4">4</cross-ref> its efficacy remains uncertain.</p>
