                        <p><p>Glucagon-like peptide-1 receptor agonists promote weight loss, suppressing appetite among other effects. Recent evidence suggests they may similarly placate desire when it comes to alcoholism and even problem gambling. This paper adds to the ethical consideration already given to the weight loss debates, by asking whether we are ready for such wider impacts, where such drugs and their descendants might be increasingly tuned to induce personal temperance and even societal docility. Aside from the money to be made by mass medicating, a more biddable populace could become a realisable dream, for authoritarians or degrowth environmental activists alike.</p>

                        <p><p><sec><st>Introduction</st></sec></p>
<p>The Netherlands implemented a supermarket tobacco sales ban on 1 July 2024. This study aimed to evaluate supermarkets’ compliance with the ban and potential unintended impacts on tobacco availability at tobacco points of sale (POS) in areas around supermarkets.</p>
<p><sec><st>Methods</st></sec></p>
<p>A longitudinal pre–post study was conducted in three Dutch cities: Amsterdam, Haarlem and Zwolle. In total, 114 supermarkets and their surrounding area (200 m radius buffer) were included. Data were collected in 2021 (pre-ban) and 2025 (post-ban). The study examined (1) supermarkets’ compliance with the tobacco sales ban and (2) change in the number of alternative POS, presence of alternative POS (yes/no) and distance to the closest alternative POS within the buffer. Poisson, logistic and linear generalised estimating equations models were applied.</p>
<p><sec><st>Results</st></sec></p>
<p>All included supermarkets complied with the ban. The number of alternative POS within 200 m of supermarkets was significantly lower post-ban, with an average reduction of 31% (incidence rate ratio (IRR)=0.69, 95% CI 0.61 to 0.77, p<0.001). The number of tobacco specialist shops increased, while other POS types (including other supermarkets) decreased. There was a non-significant reduction in the odds of an alternative POS being present around supermarkets (OR=0.86, 95% CI 0.65 to 1.13, p=0.283) and an increase in average distance from the supermarket to the closest alternative POS (<i>B</i>=6.42 m, 95% CI –0.36 to 13.2, p=0.063).</p>
<p><sec><st>Conclusion</st></sec></p>
<p>After implementing the supermarket tobacco sales ban, there were fewer POS of tobacco around supermarkets in three Dutch cities, but this change did not demonstrably reduce availability within 200 m of supermarkets.</p>

                        <p><p><sec><st>Objectives</st></sec></p>
<p>Rapid and accurate detection of antimicrobial resistance in <i>Neisseria gonorrhoeae</i> is key for appropriate treatment of gonorrhoea. Nucleic acid amplification tests are increasingly available for the detection of <i>N. gonorrhoeae</i> and antimicrobial resistance markers from clinical samples. This study evaluated the Allplex NG & DR assay for the detection of <i>N. gonorrhoeae</i> and resistance markers against ciprofloxacin (<i>gyrA</i> S91F) and azithromycin (23S rRNA A2059C and C2611T).</p>
<p><sec><st>Methods</st></sec></p>
<p>This was a retrospective diagnostic accuracy study performed in two centres. We evaluated assay inclusivity with a panel of laboratory strains, specificity with 35 <i>N</i>. <i>gonorrhoeae</i> negative clinical samples and sensitivity with 82 <i>N</i>. <i>gonorrhoeae</i> positive samples with a matched culture isolate to allow comparison of resistance calls. The Alinity <i>m</i> STI assay was considered gold standard for <i>N. gonorrhoeae</i> detection, and agar incorporation minimum inhibitory concentration testing of <i>N. gonorrhoeae</i> isolates was considered gold standard for resistance calling. Comparisons were also made to the <i>ResistancePlus</i> GC assay which detects <i>N. gonorrhoeae</i> and the S91F ciprofloxacin resistance marker.</p>
<p><sec><st>Results</st></sec></p>
<p>The Allplex NG & DR assay performed well in the inclusivity panel and had high sensitivity and specificity for the detection of <i>N. gonorrhoeae</i> in clinical samples (98.8% and 100%, respectively). For resistance detection, sensitivity and specificity were >97%. The Allplex NG & DR assay generated one ciprofloxacin resistance marker false negative due to the presence of the S91I <i>gyrA</i> mutation. Two azithromycin resistance marker false positive calls were identified due to the presence of the 23S rRNA C2611T mutation in phenotypically sensitive cases.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Our retrospective study shows the Allplex NG & DR assay performed well in the detection of <i>N. gonorrhoeae</i> and associated azithromycin and ciprofloxacin resistance mutations.</p>

                        <p><h2>ABSTRACT</h2>
<p>Street-level bureaucracy research argues that the local community shapes policy implementation. Yet, little is known about the impact of local political preferences on welfare policy application. This study extends this line of research examining how local political preferences in East Germany are linked to job centres’ delivery of active labour market programmes—sanctions, workfare programmes, wage subsidies, and training programmes. Additionally, we assess whether local political preferences are related to welfare recipients’ chances of labour market re-integration. We apply a fixed-effects approach to local elections data linked with job-centre-level registry data on welfare recipients. Our results suggest that left-wing and right-wing parties generally adhere to distinct welfare policy approaches, with higher far-left party vote shares being associated with fewer restrictive policies and higher vote shares for the Right with more employment uptake. Overall, the results suggest an important role of local political preferences in shaping welfare policy implementation through political and organisational steering.</p>

                        <p><p><sec><st>Objective</st></sec></p>
<p>Socioeconomic inequalities significantly impact access to healthcare services for individuals with type 2 diabetes mellitus (T2DM). This study investigates these inequalities in Iran, focusing on factors such as asset, sex, urban–rural residence, age, education, employment status, and marital status.</p>

<sec><st>Setting</st></sec></p>
<p>This study used data from the national ‘Diabetes Care (DiaCare)’ study, a population-based survey conducted from 2018 to 2020 in Iran.</p>
<p><br>
<sec><st>Methods</st></sec></p>
<p>Socioeconomic status (SES) was assessed using Principal Component Analysis (PCA) based on assets. Socioeconomic inequalities in access to physicians, pharmacies and laboratories were measured using the Concentration Index (CI) and Erreygers Corrected Concentration Index (ECI). Decomposition analysis was performed using a probit regression model to assess the contributions of various factors to the observed inequalities.</p>

<sec><st>Results</st></sec></p>
<p>Among 13 315 patients with T2DM, 5.8% lacked access to physicians, 6.8% to pharmacies and 8.7% to laboratories. The CI was positive and statistically significant for access to physicians (0.0614), pharmacies (0.0787) and laboratories (0.0875), indicating better access concentrated among higher SES individuals. Urban residents had the largest positive marginal effects on access to physicians (0.032), pharmacies (0.078) and laboratories (0.053), with percentage contributions of 13.21%, 23.23% and 17.39%, respectively. Higher asset quintiles showed substantial contributions to inequalities, with the highest quintile contributing 10.5% to physician access inequality, 9.68% to pharmacy access and 9.16% to laboratory access. Education level also positively impacted access, with high school education contributing 0.64% and college education 0.52% to access inequalities. Sex differences showed a negative marginal effect for women, indicating slightly lower access.</p>
<p><br>
<sec><st>Conclusion</st></sec></p>
<p>Socioeconomic factors, particularly asset, residence and education, significantly impact access to healthcare services for patients with T2DM in Iran. Policies should focus on reducing barriers to healthcare access, especially for lower SES and rural populations.</p>

                        <p><p>The new study included more than 11,000 adults aged 65 and up and was published on Thursday in <em>Geriatrics</em>. Over the course of 12 years, 45 percent of the participants saw a positive development in either their scores on a cognitive test or their walking speed—a critical measure of fitness. Many others saw no change in their cognitive skills over the study period. Notably, when the researchers averaged the participants’ scores, they saw an expected decline in ability as people aged. But on the individual level, that picture didn’t hold up for everyone.</p>

                        <p><p>Background: Best practice standards, in the form of clinical practice guidelines (CPGs) and clinical pathways (CPs), aim to standardize care and improve outcomes. However, variation in clinical practice exists, and not all deviations are inappropriate. Measuring adherence to best practice standards remains challenging due to limitations in representation methods and data fidelity. Objective: This scoping review aims to survey and synthesize the existing literature on the computable representation of guideline recommendations and to explore methods for detecting and quantifying deviations from best-practice standards. Methods: We followed the Arksey and O’Malley framework and PRISMA-ScR guidelines. Five databases (Ovid Medline, EMBASE, IEEE Xplore, Web of Science, and Scopus) were searched in November 2025. Studies were included if they either (1) described or modelled a computer representation of best practice standards, or (2) assessed adherence to such standards using patient data, including patient data derived from electronic medical records (eMR) or patient event logs. Titles, abstracts, and full texts were screened using Covidence. Data was extracted on representation, clinical context, data sources, adherence metrics, and modelling techniques. A narrative synthesis was conducted to identify themes. Results: Twenty-four studies were included. Most studies were published as conference proceedings (56%). Fourteen studies (58%) included measurement of adherence to best practice standards. Cardiovascular conditions were the most common focus (n=13, 54%). Data sources included Health Level Seven (HL7) messages, structured eMR data, event logs, and Fast Healthcare Interoperability Resources (FHIR) -transformed data. Best practice standards were formalized using Business Process Model and Notation (BPMN) (n=6, 25%), ontologies (n=7, 29%), FHIR (n=4, 17%), or hybrid approaches (n=4,17%). The most common method for adherence measurement was rule-based alignment of patient data with guideline components. Several studies incorporated weighted scoring to differentiate the severity of deviations. Process mining was used in a subset to detect sequence and timing variations. However, most models lacked contextual sensitivity and rarely incorporated patient-specific factors, such as comorbidities, patient acuity, or clinician rationale. Consequently, although deviations can be automatically identified, determining whether they were clinically warranted remained largely unresolved. Conclusions: Despite promising advances, challenges persist in representing best-practice standards in computer-interpretable formats and measuring adherence in a clinically meaningful way. Current approaches predominantly assess technical alignment rather than clinical relevance and are limited by data quality and standardization, thereby limiting real-world utility. This scoping review offers an innovative contribution by synthesizing evidence from two separate domains – the computable representation of best practice standards and the measurement of adherence. The findings emphasize the need for context-aware, standardized modelling and integration with clinical workflows that can distinguish warranted from unwarranted deviations. Developing such systems will be crucial to enabling scalable, transparent and real-time adherence monitoring – ultimately driving safer, patient-centered care delivery.</p>
