As millions turn to ChatGPT and other AI chatbots for therapy-style advice, new research from Brown University raises a serious red flag: even when instructed to act like trained therapists, these systems routinely break core ethical standards of mental health care. In side-by-side evaluations with peer counselors and licensed psychologists, researchers uncovered 15 distinct ethical risks — from mishandling crisis situations and reinforcing harmful beliefs to showing biased responses and offering “deceptive empathy” that mimics care without real understanding.

ABSTRACT

Background

Data on psychiatric comorbidities in children with intellectual disability (ID) across subgroups remain limited. Thus, we aimed to investigate comorbidity prevalence by age, sex, ID severity, and socioeconomic status.

Methods

This multicentre, cross-sectional study included 1742 children with ID consecutively recruited from child psychiatry outpatient clinics. Participants were assessed using comprehensive psychiatric interviews, the Strengths and Difficulties Questionnaire, and Clinical Global Impression.

Results

Comorbid psychiatric disorders were present in 86.2% of the sample; 24.7% had one, and 61.5% had two or more. The most common diagnoses were ADHD (44.3%), anxiety disorders (40.0%), oppositional defiant disorder (36.7%), and conduct disorder (29.2%). Comorbidity rates were significantly higher in males, adolescents, those with severe ID, and those from higher-income families. Internalising disorders were more common in females; externalising disorders in males and adolescents.

Conclusions

Our findings suggest the integration of systematic, developmentally sensitive psychiatric screening into standard clinical care for children with ID.

ABSTRACT

This study reports on the development, factor structure, and validity indicators of the Prosocial Behavior in School Scale (PBSS) among 4264 sixth- to eighth-grade students in the United States. The final PBSS is a 12-item self-report measure of students’ prosocial behavior in a specific classroom. The PBSS was developed using student descriptions of authentic prosocial behaviors. Factor analysis suggested that responses to the PBSS were represented by two distinct but correlated factors–Responsive Prosocial Behavior (PB) and Principled PB. These factors were invariant across gender, grade levels (sixth to eighth), and ethno-racial groups, and demonstrated internal consistency. Small gender, ethno-racial, and grade-level differences emerged in frequency of self-reported prosocial behavior. Validity of the PBSS was supported by measures of antecedents and outcomes of prosocial behavior. Specifically, PBSS scores correlated positively with social connection (peer friendships and teacher–student relationships), in-class engagement over the school year, and teacher use of praise (but not use of induction). Implications for research and practice are discussed.

Background

Credible long-term outcomes from randomized trials evaluating the effectiveness and cost-effectiveness of preventive programs for mental health problems are needed. We compared long-term effects of the Mind My Mind (MMM) transdiagnostic cognitive behavioral therapy (CBT) program to management as usual (MAU).

Methods

The study was a pragmatic, multisite, randomized superiority trial (2017–2019) involving youths aged 6–16 years with anxiety, depressive symptoms, and/or behavioral disturbances, recruited through family self-referral. The MMM intervention included 9–13 weekly CBT sessions. The primary outcome was change from baseline in parent-reported impact of mental health problems at 3-year follow-up post-randomization using the Strengths and Difficulties Questionnaire (SDQ) impact scale. Register-based outcomes tracked youths’ psychiatric diagnoses in mental health services. To assess cost-effectiveness, we calculated the incremental costs and incremental quality-adjusted life years (QALYs). All primary analyses followed the intention-to-treat (ITT) approach. Clinical trials registration: ID NCT04804917.

Results

Among 396 youths randomized (baseline mean [SD] age, 10.3 [2.4] years; 52.0% boys; MMM n = 197, MAU n = 199), the 3-year follow-up (median 167 weeks; range 124–203 weeks) primary outcome data were available in 69.0% and 59.3%, respectively. The decrease in SDQ-impact-score from baseline to 3-year follow-up (4.12→1.79 points [MMM] and 4.21→1.85 [MAU]) was similar (between-group difference, 0.06 [95% CI −0.41 to 0.52]; p = .81). An equal proportion (25%) of youths in MMM and MAU were diagnosed with any mental disorder during follow-up (HR 1.01, 95% CI 0.68–1.50). Total costs over the intervention period were higher in the MMM group (incremental costs 3,014 Euros [95% CI: 2.174–3.855]). Cost-effectiveness analyses favored MMM: QALY net gain 0.121 (95% CI 0.045–0.196); the cost-effectiveness ratio was 24,789 Euro/QALY.

Conclusions

Although MMM was potentially cost-effective, the beneficial effects diminished over 3 years post-treatment. The findings highlight the need for strategies to sustain long-term effects.

ABSTRACT

Introduction

Homelessness is rising in Ireland, and people experiencing homelessness face disproportionately poor mental health outcomes due to intersecting structural and psychosocial factors. Access to mental healthcare remains limited, leaving generalist services to manage increasing need, particularly during crises.

Aim

To explore healthcare professionals’ experiences of managing mental health crisis episodes among people experiencing homelessness.

Method

A descriptive qualitative study informed by critical realism. We used semi-structured interviews with 14 healthcare professionals, including GPs, nurses, a psychotherapist, a hospital consultant, and service leaders. Reflexive Thematic Analysis was used to identify key patterns.

Results

Five themes were identified: (1) barriers to accessing mental health services; (2) the role of trust and therapeutic relationships; (3) trauma-informed and holistic approaches to care; (4) the need for stability in housing, healthcare, and substance use; and (5) systemic and policy failures.

Discussion

Generalist professionals are increasingly required to provide complex mental health support in a fragmented system that often excludes the most vulnerable.

Limitations

Findings reflect professional perspectives from a small sample and are not generalisable.

Implications

Trauma-informed, flexible, and integrated services are essential to support equitable care.

Recommendations

Develop inclusive, housing-linked mental health services that embrace harm reduction and reject rigid eligibility criteria.
