Alarm swirling around Google’s AI health summaries, which produce misleading and sometimes fabricated advice, and the rise of ‘chatbot therapy’ presents a social problem as one of dangerous new technology. In fact, AI has simply made visible a crisis that was already there. People in the UK are turning to platforms for mental health advice, housing rights, welfare guidance and domestic abuse support because the institutions that were meant to provide those things have been systematically stripped back.

Abstract

Background and Aims

Sexual minority youth report significantly higher rates of substance use than heterosexual youth, yet a comprehensive and systematic evaluation and synthesis of the magnitude of this inequity has not been conducted. The purpose of this study was to conduct a meta-analysis to assess the magnitude of overall inequities in substance use between sexual minority and heterosexual youth and examine demographic and methodological moderating factors that might impact variability in substance use patterns.

Methods

A meta-analysis of studies that examined sexual orientation inequities in substance use. We conducted a comprehensive literature search across four electronic databases (PubMed, APA PsycINFO, Web of Science, ProQuest; January 2008–July 2024). Studies were eligible if they included a youth participant population (mean sample age of 25 years or younger), examined differences in substance use between sexual minority and heterosexual groups, and published between 2008 and 2024 in English. Primary outcomes were any measures of substance use; secondary outcome was age of substance use initiation. Following PRISMA guidelines, reviewers independently reviewed and extracted data. Analyses employed random-effects models, with robust variance estimation to account for dependency among multiple effect sizes within studies. Analyses examined continuous and dichotomous outcomes separately. Bivariate meta-regression examined moderators.

Results

Among 304 studies of 5 928 282 youth, sexual minority youth reported greater quantity and frequency of all assessed substances (i.e. alcohol, nicotine, cannabis, prescription drugs, powder cocaine, crack cocaine, meth/amphetamine, 3,4-methylenedioxymethamphetamine, heroin, and other substances) and engaged in more polysubstance use than heterosexual youth. For continuous outcomes, Hedges’ g ranged from 0.10 (95% confidence interval [CI], 0.05–0.15, I2 = 99.52, participants = 354 201, studies = 48) for alcohol to 0.40 (95% CI, 0.21–0.59, I2 = 100.00, participants = 30 679, studies = 5) for mixed/polysubstance use. Dichotomous outcomes showed consistently elevated odds ratios, ranging from 1.34 (95% CI: 1.24–1.46, I2 = 96.01, participants = 3 500 203, studies = 128) for alcohol to 4.63 (95% CI, 2.91–7.38, I2 = 86.35, participants = 391 827, studies = 10) for heroin use. Sexual minority youth also had earlier ages of initiation (all substance outcomes: odds ratio, 1.45; 95% CI, 1.04–2.03, I2 = 95.39, participants = 619 187, studies = 11). Moderation results indicated that inequities were larger for plurisexual youth (e.g. bisexual, pansexual), sexual minority girls and young women, and adolescents 18 years of age or younger. The magnitude of inequities was also larger for lifetime measures of use compared with measures of recent use.

Conclusions

Sexual minority youth — particularly those who are plurisexual, sexual minority girls and young women, and adolescents 18 years of age or younger — appear to engage in substance use at higher rates than heterosexual youth.

ABSTRACT

Bankruptcy and financial distress commonly precipitate suicidal crises, putting bankruptcy attorneys in potentially difficult situations with clients. This qualitative study explored attorneys’ experiences with clients’ risk for suicide and attorneys’ recommendations for suicide prevention. Bankruptcy attorneys and trustees (n = 11) were recruited via outreach to attorney groups (e.g., National Association of Consumer Bankruptcy Attorneys). Semi-structured interviews explored clients’ distress, responses to clients’ suicidal disclosures, and previous suicide prevention training. The research team established a codebook from transcribed interviews within Dedoose software, using peer debriefing and consensus building to iteratively identify themes. Nearly all attorneys had experiences with clients discussing suicide, but most did not know how to intervene. Common themes included clients’ co-occurring stressors (e.g., divorce, unemployment), destigmatizing the bankruptcy process, and navigating attorney-client privilege. Major themes for prevention were correcting misconceptions about and destigmatizing bankruptcy and suicide prevention training that is sensitive to the context of the legal profession.

During the rehabilitation year, they had on average 1.7 more days on sick leave (95 % CI 1.3 to 1.9), 2.3 more WAA days (95% CI 1.9 to 2.7) and 0.2 more days on disability benefits (95% CI 0.1 to 0.3). In the year after rehabilitation, they had 0.6 fewer days on sick leave (95% CI –0.8 to –0.3), but 3.7 more days on WAA (95% CI 3.1 to 4.2) and 0.6 more days on disability benefits (95% CI 0.4 to 0.8). Patterns were similar for the subgroup with rheumatic and musculoskeletal diseases.

<st>Conclusions</st>

People undergoing rehabilitation had more days on health-related benefits and a greater increase in long-term benefits, even after matching, indicating a higher disease and support burden than controls. Tailoring interventions and health-related benefits is an essential aspect of rehabilitation for people with complex work participation needs. Future research should include longer observation periods to explore long-term outcomes of rehabilitation.

Background

Early life adversities (ELAs) including experiences such as abuse, neglect, and household dysfunction are strongly linked to psychopathology; yet, the developmental pathways connecting ELA to externalizing and internalizing psychopathology remain unclear. While most research has focused on threat and negative affect, positive emotions may represent a critical but understudied mechanism linking ELA to mental health outcomes.

Methods

Using data from the Adolescent Brain Cognitive Development (ABCD) study, we examined positive affect trajectories across six timepoints spanning childhood through adolescence (ages 9–10 to 12–13). We employed person-centered trajectory-based clustering to identify distinct patterns in positive affect – independent of ELA exposure – followed by multinomial logistic regression to examine associations between cumulative ELA exposure and trajectory membership. Mediation analyses tested whether positive affect trajectories explained links between ELA and psychopathology outcomes.

Results

Four distinct positive affect trajectories emerged: High-Stable, Declining, Persistently Low, and Volatile (N = 7,457). Higher ELA scores significantly predicted membership in all non-high-stable trajectories, with the strongest association existing for the Persistently Low group (β = .321, p < .001). Mediation analyses revealed that Persistently Low trajectory group membership significantly mediated the relationship between ELA and internalizing problems (indirect effect = 0.030, 95% CI [0.012, 0.056], p = .007), but not externalizing problems (N = 3,927).

Conclusions

This study demonstrates that ELA shapes positive affect development through distinct, heterogeneous pathways rather than uniform effects, with persistently low positive affect representing a specific mechanism linking early adversity to later depression and anxiety. Findings suggest that targeting positive emotional experiences may be a promising intervention strategy for youth exposed to ELA.

A number of psychiatric hospitals in central China have been exposed over a scam in which they lure healthy people to become patients in a bid to defraud the public medical insurance scheme. According to an undercover report by the Beijing News, most of the patients in such privately run psychiatric institutions in Xiangyang, Hubei province, do not show any abnormal behaviour and receive little treatment.

ABSTRACT

Given the higher demands on parenting and the limited resources available, parents of autistic children experience high levels of parental stress. Chinese parents of autistic children experience elevated levels of parental stress. Prolonged and severe stress may lead to distress, which adversely affects their mental well-being. In this cross-sectional study, 112 Chinese primary caregivers of autistic children were invited to complete online questionnaires, including the Parental Stress Index, the Kessler Psychological Distress Scale, the Difficulties in Emotion Regulation Scale, the Experiences in Close Relationship Scale, and the Child–Parent Relationship Scale. Nine separate bootstrap moderation analyses were conducted. Parental stress significantly and positively predicted distress (ps < 0.001). The effect of parental stress on distress was moderated by the non-acceptance of emotional responses (B = −0.202, SE = 0.089, p = 0.026) and attachment anxiety (B = −0.176, SE = 0.081, p = 0.032). To conclude, there was a significant association between parental stress and distress. High acceptance of emotional responses and low attachment anxiety moderated the relationship, exerting protective effects.
