Overall, the main conclusion of our paper is that there is nothing systematically wrong with US immigration policy regarding the fiscal effects of immigrants. There is nothing unsustainable about the US immigration system. We could have scaled immigration as it existed without burdening government budgets. For years, nativists in Congress and the administration have wrongly claimed that immigrants are behind the growth in debt and that the US immigration system allows foreigners to take advantage of Americans’ generosity. Our data completely repudiates this view. Immigrants are subsidizing the US government. The best way to balance the budget is to reduce spending—particularly on wealthy retirees—but rather than hinder our efforts to control deficits, immigrants are helping.

ABSTRACT

Background

As researchers strive to conduct culturally competent and equitable research, there remains a lack of actionable guidance across the research lifecycle. Increasingly, funding bodies require evidence of such approaches, yet a clear, practical direction for effective research practice is often lacking.

Aim

To develop a practical framework, grounded in Meleis’ cultural competence criteria, to guide all stages of the research lifecycle.

Methods

Databases PsycINFO, ERIC, PubMed, Web of Science Core Collection and Google Scholar were searched (August–October 2025) for English-language publications applying Meleis’ framework in research contexts. Narrative and thematic analysis synthesised identified applications into draft recommendations and measures across five research stages. A modified Delphi process with 31 experts (community leads, researchers, policymakers and translators) in two survey rounds and a consensus workshop was then used to refine and finalise items, producing a co-developed framework of actionable, measurable recommendations for culturally competent research with minoritised and/or diverse groups.

Results

The review identified eight studies, yielding 41 applications of Meleis’ criteria. These were distilled into 29 draft recommendations and measures, which were further refined through Delphi consensus to 25 recommendations and 25 measures, offering practice-ready guidance across research focus, recruitment, measurement, analysis and dissemination. Contextuality was rated critical in four stages, highlighting how cultural identities shape engagement. Language was strongly prioritised in focus, recruitment and measurement, with emphasis on diverse, well-funded interpretation and transcription strategies. Empowerment and reciprocation were noted as underused but essential for legitimacy and impact, particularly in focus and dissemination. Continuous cultural humility training was endorsed, reinforcing critiques of competence as a static skill. During consensus discussions, recommendations initially framed as optional (‘could’) were reframed as compulsory (‘should’). The final framework is available as an accessible online resource.

Conclusions

Using consensus, an evidence-based, culturally competent research framework was co-designed; it comprised 25 recommendations that are actionable, measurable and adaptable by researchers, policymakers, community leads and others. This framework represents a crucial step towards fostering equitable and impactful research practice.

Patient or Public Contribution

A patient and public involvement group of three patients from minoritised backgrounds and two health professionals met twice to review findings, refine components and provide feedback on language, accessibility and usability.

<st>Introduction</st>

The debate over conscientious objection (CO) in healthcare remains deeply polarised, often framed as a clash between professional obligations and individual moral integrity. In their consequentialist defence of permitting CO, Steve Clarke proposes a region-based register of non-objecting professionals as a mechanism to minimise reliance on referrals—what they aptly term a ‘fight without fighting’ approach. This commentary supports Clarke’s core argument but seeks to strengthen it by addressing a potential vulnerability: the risk that such registers could exert social coercion in close-knit or ideologically homogeneous communities, discouraging professionals from listing themselves. I argue that this risk does not undermine the model but rather underscores the need for robust privacy protections and anonymous access protocols, thereby enhancing the system’s resilience and appeal from a consequentialist standpoint.

With the increasing incidence and persistent prevalence of syphilis, it is necessary to reevaluate the clinical utility of three syphilis algorithms.

<st>Methods</st>

Cases with concurrent Treponema pallidum-electrochemiluminescence immunoassay (TP-ECLIA), T. pallidum particle agglutination (TPPA) and toluidine red unheated serum test (TRUST) results from a large hospital in China were collected. The diagnostic performance of three syphilis algorithms—the traditional algorithm (start with TRUST and confirmation by TPPA), reverse algorithm 1 (start with TP-ECLIA and confirmation by TRUST with TPPA for TRUST-negative cases) and reverse algorithm 2 (start with TP-ECLIA and confirmation by TPPA with TRUST for TPPA-positive cases)—was evaluated using kappa coefficient analysis and Bayesian latent class model (BLCM), along with assessing their capabilities, discrepancies and limitations in identifying discordant results for T. pallidum antibodies.

<st>Results</st>

High agreement was observed between the two reverse algorithms. BLCM revealed sensitivities of 49.1%, 98.9% and 98.5% and specificities of 99.6%, 99.2% and 100.0% for the traditional algorithm, reverse algorithm 1 and reverse algorithm 2. The traditional algorithm identified 11 biological false-positive cases but missed 1177 TPPA+/TRUST– syphilis diagnoses. Discordant treponemal test results (TP-ECLIA+/TPPA–/±) were common (18.2%) and associated with significantly lower TP-ECLIA cut-off index values. The reverse algorithm 1 identified 500 treponemal test-discordant cases, while the reverse algorithm 2 identified 517 cases. Besides, 17 discordant cases were identified among untested components of the algorithms.

<st>Conclusions</st>

The reverse algorithm 2 is recommended, as the confirmation of TP-ECLIA by TPPA reduces the underdiagnosis of previously treated syphilis compared with the traditional algorithm and compensates for the limited specificity of TP-ECLIA, thereby representing an improvement over reverse algorithm 1.

Abstract

In this piece, we consider the practice of psychoanalysis ‘otherwise’ through the legacies and lessons of free clinics. Here, we challenge the ‘straight line’ of an institutionalised, exclusive historiography, arguing that prohibitive fees have enclosed psychoanalysis within bourgeois quarters – yet, this is not the whole story. Against a backdrop of biomedical dominance, we assert the renewed relevance of psychotherapeutic approaches that work with singularity, and the dynamic unconscious as a guarantor of creativity and liveable future. Our research collective, FREEPSY, investigates free clinics as fundamental experiments of political implication. We identify an ‘infrastructural turn’ in psy-practice, where clinics operate as micro-economies that reinvent rules on value, setting and the social bond. Through radical listening and a creative relationship to the institution, these collectives de-individualise care. We conceptualise this as a ‘mental health commons’, positioning mental health under a biopsychosocial matrix that is not individualising, but is a micropolitical matter of social (re)production. The ‘free’ in free clinic becomes an exclamation—a call to action and a collective interpellation for practising otherwise. We argue for a new relevance of psychoanalysis in catastrophic times by opening space for that which does not fit, and honouring the unconscious.

ABSTRACT

Background and Aims

Although the global plan aimed to eliminate mother-to-child transmission of HIV by 2015, an estimated 130,000 children were newly infected, and 84,000 children died from HIV related causes in 2022. In Ethiopia, 3200 children became newly infected and 1,900 children died from HIV related causes in 2022. A new target has now been set for 2030. Vertical transmission remains the predominant route of pediatric HIV infection. This study aimed to assess HIV vertical transmission and its predictors among HIV-exposed infants in Northwest Ethiopia.

Methods

A retrospective cohort study was conducted among 480 mother–infant pairs enrolled in the PMTCT program. Data were analyzed using R version 4.3.2. To account for missing values, multiple imputations were performed prior to bivariate and multivariable logistic regression analyses. Associations with a p-value < 0.05 were considered statistically significant.

Results

The rate of vertical transmission of HIV was 3.6%. Mothers residing outside the catchment area (AOR = 4.74; 95% CI: 1.17–20.63) and those not receiving antiretroviral therapy (AOR = 6.82; 95% CI: 1.33–38.73) had significantly higher odds of vertical HIV transmission. Conversely, maternal HIV diagnosis prior to pregnancy (AOR = 0.08; 95% CI: 0.009–0.84), receiving Nevirapine prophylaxis (AOR = 0.14; 95% CI: 0.03–0.65), and delivery at a health facility (AOR = 0.08; 95% CI: 0.01–0.66) were significantly protective against mother-to-child transmission.

Conclusion

The rate of vertical HIV transmission was found to be high. Therefore, we recommend further PMTCT service expansion; focus on maternal HIV diagnosis in the preconception period; improve provision of infant Nevirapine prophylaxis and maternal ART help to minimize HIV vertical transmission. Reduce home delivery also helps to decrease HIV vertical transmission.

ABSTRACT

Background

The link between a therapist’s motivational interviewing skills and the subsequent response of individuals with mild intellectual disabilities to borderline intellectual functioning (MID-BIF) is unknown. This study examines this sequential relationship and describes change talk (CT) in individuals with MID-BIF during substance use disorder (SUD) treatment.

Method

In this study, 35 treatment sessions were sequentially coded using the Motivational Interviewing Sequential Code for Observing Process Exchanges. Observed and expected frequencies of transitions, transitional probabilities, odds ratios and 95% confidence intervals were calculated in Generalized Sequential Querier software. Furthermore, frequencies of CT in IBM SPSS statistics were calculated.

Results

Questions and reflections of CT and two-sided questions were followed by CT. Questions and reflections of counter change talk (CCT) and two-sided questions were followed by CCT. Individuals expressed all kinds of CT utterances.

Conclusions

Questions and reflections of CT are powerful skills in evoking CT in individuals with MID-BIF during SUD treatment.

The purpose of this special issue of Canadian Psychology is to celebrate the 40th anniversary of the Canadian Code of Ethics for Psychologists (the Code), which was first adopted by the Canadian Psychological Association in 1986. Although many aspects and applications of the Code have changed since it was first adopted, the ethical principles providing the moral foundation of the Code have remained the same, the Code’s organization around these ethical principles has been maintained, and the inclusion of a decision-making model to help individual psychologists resolve ethical dilemmas has been preserved. The continuing relevance, versatility, and wide-ranging application of the Code in today’s world are clearly evident from the 12 articles comprising this special issue. The articles describe the application of the Code to old and new ethical issues related to research (e.g., patient partnership, integration of data technologies, knowledge mobilization) and practice (e.g., pronoun charting, dual relationships in rural or remote communities, psychedelic-assisted psychotherapy, technology in psychological service delivery), the mapping of the literature on the Code, the role of the Code in guiding university graduate-level teaching of ethics, and the conceptual representation of different worldviews in the Code (e.g., the concept of “peoples” in the Code and its implications for ethical decision making; the alignment between the ethical principles, values, and standards as defined in the Code and the worldview of Indigenous peoples in Canada). As guest editors, we believe this collection serves as an important resource to psychologists and an inspiration for future work. (PsycInfo Database Record (c) 2026 APA, all rights reserved)

The Canadian Code of Ethics for Psychologists (or CPA Code), first published in 1986, has established itself as one of the most comprehensive and influential professional codes of ethics. Over the past four decades, a broad body of scholarly literature has focused on the development, structure, and practical applications of the CPA Code. Despite this, there are no existing scoping reviews that have focused on the CPA Code. Our goal was to conduct a scoping review summarizing the existing literature on the CPA Code. This review was conducted in accordance with the Joanna Briggs Institute scoping review methodological guidelines, and findings are reported using the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. APA PsycInfo, Web of Science, and Embase were systematically searched from inception to February 2024 for English or French language peer-reviewed articles that analyzed or applied the CPA Code in any context and with any population. Of the 139 search results screened, 97 articles met inclusion criteria. Narrative reviews constituted the majority of articles, followed by commentaries, empirical studies, and theoretical analyses. Overall, the CPA Code literature focuses on a wide range of ethical issues related to psychological practice. Many studies cursorily mentioned ethical principles or standards and their applicability without an in-depth analysis. There is a need for additional empirically supported and cross-cultural studies to validate the CPA Code’s structure and applicability across diverse populations. Future research should also focus on identifying areas for potential CPA Code improvements to reflect the evolving nature of our discipline. (PsycInfo Database Record (c) 2026 APA, all rights reserved)

Patient partnership (also called patient and public involvement, citizen or community engagement, community-based participatory research, etc.) is the meaningful and active engagement of diverse individuals with personal experience of a health issue and informal caregivers, including family and friends, on research teams. Patient partnership has rapidly become the norm for psychological and health research in Canada. While patient partnership can significantly benefit patient partners, researchers and the research itself, it remains a relatively new practice and can be harmful when undertaken poorly. Notably, the Canadian Code of Ethics for Psychologists, fourth edition (the ‘Code’), was developed largely before widespread patient partnership practice and neglects to name or define ‘patient partners’ as a group. While we purport that patient partnership is an ethical imperative for psychologists, the role of patient partners must be distinguished from others with whom psychologists and researchers interact (e.g. research participants, clients, employees, trainees, volunteers, clinical advisors). Our main purpose was to discuss key ethical standards from the Code pertinent to patient partnership including informed consent, privacy and confidentiality, fair treatment, general caring, competence and self-knowledge, risk, harms, benefits, objectivity, straightforwardness, openness, conflict of interest and extended responsibility. We also provide guidance regarding challenges to ethical practice in patient partnership, specifically risks posed by patient partners as an unnamed group in the Code; issues of power and vulnerability; challenges of multiple roles of patient partners; considerations of equity, diversity, inclusion and the need for an intersectional trauma-informed approach; and guidance for building an evidence base. (PsycInfo Database Record (c) 2026 APA, all rights reserved)

The pervasive influence of digital data in contemporary society presents research psychologists with significant ethical challenges that have yet to be fully recognized or addressed. The rapid evolution of data technologies and integration into research practices has outpaced the guidance provided by existing ethical frameworks and regulations, leaving researchers vulnerable to unethical decision making about data. This is important to recognize because data is now imbued with substantial financial value and enables relations with many powerful entities, like governments and corporations. Accordingly, decision making about data can have far-reaching and harmful consequences for participants and society. As we approach the Canadian Code of Ethics for Psychologists’ 40th anniversary, we highlight the need for small updates to its ethical standards with respect to data practices in psychological research. We examine two common data practices that have largely escaped thorough ethical scrutiny among psychologists: the use of Amazon’s Mechanical Turk for data collection and the creation and expansion of microtargeting, including recruitment for psychological research. We read these examples and psychologists’ reactions to them against the current version of the Code. We close by offering specific recommendations for expanding the Code’s standards, though also considering the role of policy, guidelines, and position papers. (PsycInfo Database Record (c) 2026 APA, all rights reserved)
