<p><p><sec><st>Objectives</st></sec></p>
<p>Prevalences of <i>Trichomonas vaginalis</i> (TV) in Europe are low and consequently marginally tested by Dutch sexually transmitted infections (STI) clinics. We routinely tested all patients on all anatomical sites. We aimed to assess trends, prevalences among key populations and at different anatomical sites, as well as predictors of being TV positive to inform future testing practices and guideline development.</p>
<p><sec><st>Methods</st></sec></p>
<p>A retrospective cross-sectional study was performed between 2014 and 2022, using STI clinic data. Trends and positivity rates among key populations and anatomical locations were assessed using descriptive analyses. Sociodemographics and sexual behaviour variables were analysed to assess predictors of being TV positive, using multivariable regression analysis.</p>
<p><sec><st>Results</st></sec></p>
<p>A total of 37 577 consultations were performed in which patients were tested for TV; 16 075 among women and 21 502 among men. Overall TV positivity was 0.9% (145/16 075) among women and 0.3% (54/21 502) among men, and highest among women with a Netherlands Antillean ethnic background with 5.2% (11/212). TV positivity was lowest at the pharyngeal anatomical site (0.1%), followed by anorectal site (0.2%) and the urogenital anatomical site (0.5%).</p>
<p>The following predictors of being TV positive were tested significant for men: being older (≥40 years, OR=5.1), having a Surinamese (OR=6.8) or Netherlands Antillean (OR=7.4) ethnic background and being gonorrhoea positive (OR=2.1). For women, the following predictors tested significant: being older (≥40 years, OR=3.3), having an Eastern European (OR=3.1), Western European (OR=2.2), Netherlands Antillean (OR=7.8) or Mid and South African (OR=4.0) ethnic background, having STI symptoms (OR=2.6) and being gonorrhoea positive (OR=2.6).</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Based on our results of 9 years of screening in an STI clinic setting, routine universal testing for TV is unnecessary. Testing patients, particularly women, is only required in symptomatic cases and might be considered in cases with an Eastern European, Netherlands Antillean or Mid and South African ethnic background.</p>

<p><p><span class="paragraphSection">Infants and children with prenatal substance exposure (PSE) experience high rates of child protection system (CPS) involvement.<sup>1</sup> Reporting to CPS is often initiated around the time of birth due to concerns about future risk of abuse and neglect, but reporting mandates vary by jurisdiction, state, and/or country and interpretation of these mandates can further vary by hospital and provider.<sup>2–5</sup> Few studies have been able to look at cumulative risk of CPS reports, investigations, and substantiated abuse and neglect following PSE throughout childhood because data systems involving administrative claims, CPS data, and substance use records are rarely linked or tracked longitudinally. In this issue of <span>Pediatrics</span>, Powell and colleagues were able to link population-level health, substance use, and CPS data spanning a 12-year period to compare childhood CPS involvement among children with PSE compared with those without in New South Wales (NSW), Australia.<sup>6</sup> They found, unsurprisingly, that by age 12 years, when compared with children without documented exposure, children with documented PSE had significantly higher rates of CPS reports that were screened in for investigation (90% vs 18%), CPS cases that were substantiated (61% vs 5%) and out-of-home placement episodes (39% vs 1%).</span></p>

<h2>Background</h2>
<p>Although individuals with psychosis represent a minority of homicide perpetrators, they are overrepresented in ‘overkill’ cases, yet little is known about the characteristics of this specific group.</p>
<h2>Aims</h2>
<p>To describe the characteristics of people with psychosis who not only kill another person but also use far more force than necessary to do so.</p>
<h2>Methods</h2>
<p>A 10-year cross-sectional clinical chart review was conducted at two major forensic psychiatry centres in Türkiye found 183 homicide perpetrators with psychotic illness whose index offences met overkill criteria, operationalised as the infliction of violence beyond that necessary to cause death. Data on background, clinical and homicide-related factors were analysed and an exploratory cluster analysis was performed.</p>
<h2>Results</h2>
<p>Almost all (91%) perpetrators were men, with a mean age of 38 years and long-standing illness with impaired psychosocial functioning and prior psychiatric contact. Most usually the victim was a relative, killed in a private indoor setting with a sharp weapon, easily to hand, when the perpetrator was experiencing active psychotic symptoms. Cluster analysis confirmed two distinct subgroups: this psychosis-dominant/domestic type, but also a smaller externalising/premeditated type, these perpetrators characterised by more planning and much greater likelihood of prior involvement with crime and/or substance use and targeting peers rather than family.</p>
<h2>Conclusion</h2>
<p>The profile of overkill perpetrators with psychotic illness largely resembles that of people with psychosis who kill without excessive violence, but the identification of two distinct subgroups highlights important heterogeneity within this population. Given that the perpetrators had almost all been known to mental health services at some point, these findings should help focus assessments and guide risk management approaches, which, particularly for the domestic attacks, may include environmental strategies.</p>

<p><h2>ABSTRACT</h2>
<h2>Introduction</h2>
<p>China’s one-child policy was implemented in 1980 primarily through mandatory intrauterine devices, tubal surgery, or abortion for “unauthorized pregnancies.” While it was replaced in 2015, it affected millions of persons and its effects on reproductive decision making are not well known.</p>
<h2>Methods</h2>
<p>We designed, validated, and performed a cross-sectional survey of reproductive age Chinese-born or first-generation women of Chinese descent to describe the policy’s impact on reproductive decision-making. Descriptive statistics and multivariate logistic regression were used to identify self-reported policy impact on contraceptive utilization and childbearing choices, and demographic associations.</p>
<h2>Results</h2>
<p>Between June 1 and October 31, 2021, 1098 people accessed the survey, and 838 were eligible. A total of 588 responded to questions pertaining to the primary outcome, yielding a response rate of 70.2%. Approximately 42% of participants lived under the policy and were affected by it in some way. 17.3% of participants stated their contraceptive utilization was affected and 23.3% stated their childbearing choices were affected. Those with low acculturation scores (OR = 2.27, 95% CI 1.35–3.85, <i>p</i> = 0.002) and those living in the United States for < 21 years (OR = 2.25, 95% CI 1.09–4.67, <i>p</i> < 0.01) were more likely to report their contraceptive plans were affected. 72.8% of participants self-reported high reproductive autonomy, whereas acculturation was mixed.</p>
<h2>Conclusions</h2>
<p>Although the one-child policy has had a large effect on people of the Chinese diaspora, its impact on reproductive decision-making may decrease with the duration of time in the United States and increasing acculturation.</p>
