                        <p><p><sec><st>Objectives</st></sec></p>
<p>While health services leaders rely heavily on information gathered via environmental scans (ESs) to guide strategic decision-making, formal guidance on how to conduct these scans is notably absent. The purpose of this study was to determine the level of agreement on essential components of a definition and a methodological framework for ESs. The goals were to (1) advance our working definition to a concept definition for ESs and (2) develop a methodological framework to guide health service researchers conducting ESs.</p>
<p><br>
<sec><st>Design and setting</st></sec></p>
<p>We used a real-time, modified Delphi survey in a virtual platform setting to seek perspectives on statements related to ESs from individuals who were recruited based on having verifiable experience designing or conducting ESs in health services delivery research. Surveylet, an online software, was used to facilitate asynchronous data collection and to determine the level of agreement on the statements with an a priori threshold of 75% set for agreement on each statement.</p>

<sec><st>Participants</st></sec></p>
<p>21 panellists provided opinions on 59 statements related to a proposed ES definition and on 69 statements specific to components of a methodological framework for ESs.</p>
<p><br>
<sec><st>Results</st></sec></p>
<p>Panellists from four countries participated in the survey representing 2 to ≥11 years of experience with ESs and having completed 1 to ≥7 ESs. Agreement was achieved in 28 of the 59 statements related to the ES definition and for 51 of 69 statements related to a methodological framework.</p>
<p><br>
<sec><st>Conclusions</st></sec></p>
<p>The agreement on many elements deemed essential for a definition of ES support development of a proposed concept definition of ES in health service delivery research. As well, the agreement on components deemed necessary for a methodological framework will help in future development of such a framework to guide stakeholders in the planning and implementation of ESs. These results provide a starting point for a common understanding of ESs in the field of health services delivery research.</p>

<p>HIV pre-exposure prophylaxis (PrEP) programmes in Africa reach young women at risk of sexually transmitted infections (STIs). We evaluated curable STI prevalence, incidence and risk factors among women initiating PrEP.</p>
<p><sec><st>Methods</st></sec></p>
<p>From August to December 2022, sexually active women aged 16–30 years from 15 South African sites, and one site each in Eswatini, Kenya, Malawi, Uganda and Zambia were enrolled into the INSIGHT cohort and offered oral emtricitabine/tenofovir PrEP with follow-up at 1, 3 and 6 months. At each visit, STI symptoms were assessed and treatment provided based on syndromic management or diagnostic testing. Diagnostic tests included nucleic acid amplification for <i>Chlamydia trachomatis</i> and <i>Neisseria gonorrhoeae</i>, the rapid OSOM for <i>Trichomonas vaginalis</i> at enrolment and month 6, and serological testing for syphilis at enrolment using rapid plasma reagin with confirmatory <i>Treponema pallidum</i> particle agglutination. Prevalence and incidence of each STI were calculated, and predictors assessed using multivariable regression.</p>
<p><sec><st>Results</st></sec></p>
<p>Of 3087 participants offered daily oral PrEP with a median age of 23 (IQR 21–27), 3011 had STI results and 30.9% had one or more STIs, with 15.7% reporting symptoms. The prevalence of <i>C. trachomatis</i>, <i>N. gonorrhoeae</i>, <i>T. vaginalis</i> and syphilis was 20.8%, 6.8%, 6.0% and 4.4%, respectively. The incidence of one or more STIs (<i>C. trachomatis</i>, <i>N. gonorrhoeae</i> or <i>T. vaginalis</i>) was 49.3/100 person-years (95% CI 45.3 to 53.4) with 12.7% reporting symptoms. The incidence of <i>C. trachomatis</i> was 30.6/100 person-years (95% CI 27.5 to 33.7), <i>N. gonorrhoeae</i> 10.8/100 person-years (95% CI 9.0 to 12.6) and <i>T. vaginalis</i> 11.5/100 person-years (95% CI 9.7 to 13.4). An incident STI diagnosis was associated with low alcohol use (adjusted incidence rate ratio (aIRR) 1.3; 95% CI 1.0 to 1.6) and moderate alcohol use (aIRR 1.4; 95% CI 1.1 to 1.8), and having an STI diagnosed at enrolment (aIRR 1.8; 95% CI 1.5 to 2.1).</p>
<p><sec><st>Conclusion</st></sec></p>
<p>The high prevalence and incidence of STIs among African women initiating PrEP, most of whom did not report symptoms, highlights the need for aetiologic testing to detect STIs, guide treatment and reduce reproductive health sequelae and risk of transmission.</p>

<h2>Motivation</h2>
<p>How do people living in an insecure borderland beset by civil war and insurgency solve social problems and improve life when they are targeted by counterinsurgency forces, taxed by insurgents, and their villages are too insecure to get state or NGO services? Understanding their everyday governance helps reorient approaches to development in areas in or emerging from war.</p>
<h2>Purpose</h2>
<p>The article presents insights from residents of rural areas on either side of the national border between Somalia and Kenya, where protracted and violent conflict has been ongoing for over 30 years. It focuses on practices of everyday governance from a grounded perspective. What do residents’ solutions look like? Who do they engage with and how? What norms and connections do they privilege?</p>
<h2>Methods and approach</h2>
<p>Resident-led reflection began in early 2024 and continued for six months with 50 diverse borderland residents formed into small cross-border groups. Facilitators invited them to reflect on their daily lives using a storytelling method.</p>
<h2>Findings</h2>
<p>People and their organizations are consistently negotiating security, managing social relations, and forging cross-community collaborations. In these borderlands, where economic, military, and geopolitical interfaces are hardened by military and political frontiers, the role of the individual and their communities in solving governance problems is especially salient.</p>
<h2>Policy implications</h2>
<p>These findings challenge some core assumptions of contemporary governance interventions in areas of limited statehood that suggest that state building should begin from the top and concentrate on government. Governments and NGOs should stress-test their proposed and ongoing policies and programmes by asking how well they enhance the three processes that citizens are already working on: negotiations over security, the organization of community relations, and the networks of interconnected problem solving.</p>

                        <p><p><sec><st>Introduction</st></sec></p>
<p>Smoke-free policies are often regarded as an imposition on the economy which has hindered their adoption in countries like China. In reality, they increase workforce productivity, including via fewer sick days (absenteeism) and unsanctioned breaks (presenteeism), culminating in an increase in effective work hours. This paper measures the potential impact on productivity levels in China via these short-term channels following the introduction of a national smoke-free law for indoor public places.</p>
<p><sec><st>Methods</st></sec></p>
<p>We use existing systematic reviews to identify effect sizes for smoke-free policies on smoking behaviour in the workplace, with this evidence being supplemented by local studies. A baseline model of aggregate hours worked across China’s economy was developed using a 2018 survey of adult smoking, National Account Statistics and International Labour Organization data. The effect sizes were introduced to test the impact of the law on effective hours worked by those who quit smoking.</p>
<p><sec><st>Results</st></sec></p>
<p>A national smoke-free law would cause the smoking rate among employed adults to decrease by 5.3% in relative terms, representing 11.6 million fewer adults who smoke. This decrease in smoking would, in turn, increase total effective work hours across China by 0.07% and expand Gross Domestic Product by ¥90 billion per annum or by 0.10% on current levels.</p>
<p><sec><st>Conclusion</st></sec></p>
<p>The productivity gains from a national smoke-free law in China are potentially very sizeable even before considering the impact of long-term reductions in mortality and healthcare spending. As China’s working-age population declines, such a law would strongly support high-quality development.</p>

<p>In response to the ongoing risk of disasters triggered by natural hazards across Bangladesh, the Shelter Cluster Bangladesh undertook a comprehensive Shelter Vulnerability and Capacity Assessment to identify communities most exposed to these risks. This assessment employed a community-driven approach, engaging directly with residents to capture their perceptions, lived experiences, and practical recommendations informed by previous hazard events. Through this participatory process, the initiative sought to uncover the underlying vulnerabilities of households and assess the resilience capacities of communities frequently affected by cyclones, monsoon floods, flash floods, and other recurring natural hazards.</p>
<p>The objective of the assessment is to generate evidence that would contribute to the revision of national shelter standards and support humanitarian partners in designing more effective, contextually appropriate, and community-centered shelter interventions.</p>
<p>This extensive effort was made possible through the dedicated support of Shelter Cluster partners; their collaboration was essential to completing the assessment successfully. In total, 3,078 quantitative household survey responses were collected to ensure a comprehensive and representative understanding of the conditions, risks, and needs of disaster-prone households. This large-scale dataset forms a reliable foundation for evidence-based planning and targeted programming.</p>
<p>To enrich the quantitative findings with deeper community perspectives, the assessment also included 84 Key Informant Interviews (KIIs) with influential community members such as teachers, religious leaders, and elected representatives. Additionally, 24 Focus Group Discussions (FGDs) were conducted at the community level to explore shared experiences, challenges, and coping strategies in greater depth. Together, these complementary methods provided a holistic picture of community vulnerability and capacity in the face of natural hazards across Bangladesh.</p>

                        <p><h2>ABSTRACT</h2>
<h2>Background</h2>
<p>Although advance care planning (ACP) has been vigorously promoted by the Japanese government, it remains unfamiliar to the general public. This study explored citizens’ perspectives on ACP conversations in clinical settings using a newly developed handbook to support such discussions.</p>
<h2>Methods</h2>
<p>A qualitative study using four focus group interviews was conducted in March 2024 with 15 citizens aged 53–77, recruited through local networks. Participants included cancer survivors, family caregivers, and individuals with bereavement experiences and/or professional experience in healthcare. The interviews were transcribed verbatim and analyzed thematically.</p>
<h2>Results</h2>
<p>Four main themes emerged: (1) positive responses to ACP, (2) expectations for the broader promotion of ACP, (3) concerns regarding ACP and the use of the Handbook, and (4) real-life experiences related to ACP. Many participants expressed willingness to engage in ACP, and the Handbook was valued as a concrete tool for structuring conversations. Concerns were raised about whether healthcare professionals would have time and readiness to engage in ACP in busy hospital settings, suggesting a misalignment between citizens’ openness to ACP and professionals’ assumptions about patients’ reluctance.</p>
<h2>Conclusions</h2>
<p>This study highlights the perspectives of relatively healthy citizens, an area that remains understudied. A proactive segment of the public willing to engage in ACP was identified. To support this, practical tools such as the Handbook should be accompanied by institutional support and professional readiness. Encouraging dialogue among citizens themselves may further help foster broader social momentum for ACP in Japan.</p>

                        <p><h2>ABSTRACT</h2>
<h2>Background</h2>
<p>Raising a child with intellectual disabilities poses significant challenges. Despite the challenges faced by parents of children with intellectual disabilities, resilient parents can maintain a positive outlook and effectively navigate these challenges, whereas others struggle. This systematic review contributes to the literature by synthesising research on the factors influencing resilience among parents of children with intellectual disabilities.</p>
<h2>Methods</h2>
<p>Searches were conducted using databases MEDLINE, PsycINFO, EMBASE, CINAHL and SocINDEX. Reference lists of all included articles were searched. Quality assessments were performed using CASP.</p>
<h2>Results</h2>
<p>Of the 6382 studies identified, 27 were included in the review. A narrative synthesis of the included studies was performed. The study details were organised into two main factors: personal and contextual.</p>
<h2>Conclusion</h2>
<p>This systematic review highlights the need to promote the resilience of parents of children with intellectual disabilities. Evidence suggests that a combination of personal and contextual factors contributes to parental resilience.</p>
