Indigenous children in Canada have long been overrepresented in foster care, often placed in non-Indigenous foster homes, reflecting the legacy of colonial policies and ongoing child welfare practices that separate them from their families and cultures. In 2022, among Indigenous people aged 15 and older with a history of foster care during childhood, 78% of First Nations people living off reserve, 89% of Métis and 43% of Inuit reported having been placed in non-Indigenous, rather than Indigenous, foster homes. While placements in Indigenous foster homes appear to have increased over time, the majority of Indigenous foster children continue to be placed in non-Indigenous foster homes.

Jan 23rd 2026, 12:16

ABSTRACT

Background

This study aimed to explore the experiences of people diagnosed with opioid use disorder (OUD) and recently involved with the criminal-legal system (CLS) as they received addiction treatment services from a Mobile Medications for Opioid Use Disorder (MOUD) programme in Chicago. By assessing perceptions and satisfaction with receiving addiction treatment and additional wraparound services, including factors influencing care-seeking decisions, this research provides insight to tailor care and services to this population of people who have involvement with the CLS. Insights from this population are critical to increasing their access to MOUD and other services, given that people with CLS involvement are at disproportionately higher risk of overdose than people without such involvement.

Methods

Semi-structured interviews were conducted with people with OUD engaged in mobile MOUD services who had CLS involvement in the past 90 days (arrested, booked, or charged; released from prison, jail, or electronic monitoring; or currently on probation, parole, or supervised release). Eligibility for participation was determined by an outreach specialist with knowledge of past service utilisation and access to the medical record. Questions were based on Consolidated Framework for Implementation Research (CFIR) domains. Interviews were recorded, transcribed, deidentified, and inductively analyzed using Dedoose qualitative analysis software. The primary phenomenon of interest was a description of perceived benefits, barriers, and facilitators to accessing mobile MOUD services by CLS-involved individuals.

Results

A total of 13 interviews were conducted. The mean age of the participants was 48.5 years (SD 11.4). Over half of participants identified as Black or African American (61.5%) and male (61.5%). In the 30 days prior to interview, seven (53.8%) participants reported stable housing, five (38.5%) were living outdoors, and one (7.7%) had been incarcerated. Five themes and eight sub-themes were identified. Participants viewed the mobile unit services as essential to their communities, citing proximity to an open-air drug market and limited services in the area. Participants overwhelmingly praised the non-judgmental, approachable, and trustworthy environment compared to other healthcare settings. Law enforcement interactions ranged from positive to neutral, with some noting CLS referrals to mobile medical unit care. Key facilitators included onsite MOUD dispensing, walk-in appointments, and comprehensive medical services. Word-of-mouth, co-utilisation of mobile services with partners, friends, and/or family, and intrinsic self-motivation were identified as facilitators to mobile unit access. Barriers to accessing care included incarceration, transportation, and schedule/service awareness.

Conclusions

By exploring the perspectives of people with opioid use disorder and involvement with the CLS, the study contributes to the design and tailoring of an established mobile MOUD programme to meet their collective needs, including broader implementation of services that support re-entry to the community, like peer recovery and recovery-oriented support groups.

Patient Contribution

This study directly involved people with opioid use disorder who were recently involved in the criminal-legal system, a population historically marginalised in healthcare settings, enabling a client-informed approach to service improvement. By centreing their voices through qualitative interviews, the research provides insight into how structural barriers such as incarceration, housing instability, stigma, and limited service availability affect their ability to access treatment. Results from this qualitative study will be discussed with members of the Community Outreach Intervention Projects (COIP) organisation who have lived experience with substance use disorders and living experience with the neighbourhoods these services are offered in. The findings inform actionable strategies to adapt mobile healthcare services to better meet the unique needs of this population, including facilitating access immediately post-release.

ABSTRACT

Developed by the United Nations Children’s Fund (UNICEF) and collaborators, the Children’s Climate Risk Index (CCRI) is an important global framework to assess and reduce the impact of climate risks on children. Reframing climate change as a child rights issue, the CCRI functions as an evidence-based policy instrument to translate the normative principles of the Convention on the Rights of the Child into national and international actions. This article analyses the conceptual, methodological and policy features of the CCRI, alongside its integration with national and international policies and actions, including UNICEF’s initiatives. It highlights the CCRI’s potential to guide disaster risk reduction, social protection and education policies that empower children’s resilience to climate risks. Some limitations of the CCRI, such as restricted institutional inclusiveness, limited child participation and data gaps, are also identified. Based on these findings, this article proposes future directions for strengthening the CCRI and calls for stronger scientific and policy efforts to advance child-centred climate knowledge and governance.

Background

Evidence on the association between loneliness and psychotic experiences in adolescents remains limited. Moreover, loneliness has typically been assessed at a single time point, which fails to capture its dynamic nature. We hypothesized that persistent loneliness, assessed across repeated measures, would be associated with psychotic experiences and other mental health problems.

Methods

Using longitudinal data from 3,171 participants in the Tokyo Teen Cohort, we applied the g-formula. We analyzed how loneliness patterns at ages 12 and 14 were associated with psychotic experiences, depression, anxiety, and diminished well-being at age 16, accounting for time-fixed and time-varying confounders. Missing data were handled using multiple imputation by chained equations.

Results

Persistent loneliness was associated with increased risk and greater severity of psychotic experiences (RD 7.1%, 95% CI: 0.8–14.3; RR 2.44, 95% CI: 1.16–4.11; β 0.28, 95% CI: 0.10–0.48). Incident loneliness at age 14 showed similar associations. No association was found for adolescents whose loneliness had remitted by age 14 (RD −1.3%, 95% CI: −3.6 to 1.2; RR 0.73, 95% CI: 0.31–1.26; β 0.01, 95% CI: −0.04 to 0.08). Sensitivity analyses using marginal structural models yielded results that were largely unchanged. Findings were generally similar for other mental health problems. Associations were consistent across genders, although the association with well-being appeared particularly important for girls.

Conclusions

The dynamics of loneliness are associated with a wide range of mental health problems in adolescents. The risk may not be permanent and could be mitigated if loneliness remits. Further research examining interventions that target loneliness is warranted.

Abstract

Background and Objective

Advances in information and communication technologies offer increased flexibility for employed parents but also blur work–family boundaries. Research is needed to understand how flexible working arrangements, such as after-hours telework, may affect relationships and well-being within the family. This study thus examined associations between adolescents’ perceptions of parental after-hours telework and psychological adjustment, focusing on the mediating role of parental support and technoference.

Methods

Participants were 381 adolescents (46.5% girls; 78.1% identified as Québécois) aged 15 to 18 years, who completed a survey about parental after-hours telework, parental support, family technoference, and psychological adjustment (i.e., self-esteem, life satisfaction, externalizing and internalizing symptoms). Structural equation modeling was used to test hypothesized associations, with path analyses assessing mediating roles of support and technoference for mothers and fathers separately.

Results

Paternal after-hours telework was associated with lower adolescent self-esteem and life satisfaction, and higher levels of externalized and internalized symptoms. Maternal after-hours telework was associated only with increased externalized symptoms. Path analyses indicated that lower paternal support and higher maternal technoference partially mediated these associations.

Conclusion

Findings reveal distinct patterns linking maternal and paternal after-hours telework to adolescents’ psychological adjustment, highlighting the mediating influence of reduced parental support and increased technoference.

Implications
