<p><p>Health inequities are unnecessary, avoidable and unjust differences in health across social groups. Addressing them is a priority for governments and health systems worldwide, requiring not only specific interventions targeting inequity but also embedding equity across all decision-making. Systematic reviews of interventions underpin health decision-making and could, therefore, be a key mechanism to address inequities, but most reviews are limited in their approach to considering equity and often only conclude data for subgroup analyses are unavailable. While some guidance is available, it largely focuses on reviews of interventions specifically seeking to reduce inequities and is published in disparate literature. We describe approaches to incorporate an equity perspective relevant to all systematic reviews of interventions, even when equity is not the primary review focus.</p>
<p>Consideration of equity may be needed at all stages of the review process. Planning the review involves examining theory, using logic models, involving relevant people and organisations, and considering if additional sources of evidence are needed. Investigating the data requires examining the external validity of primary studies, including who was involved in the primary studies, and the reach of interventions. The synthesis process includes selecting appropriate analysis, considering the implications of reporting absolute or relative equity effects of the intervention, exploring and understanding mechanisms and assessing certainty of the evidence in relation to equity. Interpreting results involves linking theory with evidence and discussing implications and limitations. We hope this article helps review authors make best use of the available evidence to incorporate equity into systematic reviews.</p>

<p>During the recent COVID-19 pandemic, experiences of health care providers were labeled as moral injuries in numerous research articles. Prior to COVID, this construct was applied primarily to combat veterans. This conceptual review examines whether moral injury is an appropriate construct to describe health care providers’ experiences. Richardson et al. (2020) identified seven definitional themes that emerged from their review of 124 studies in military samples. The present conceptual review compares recent studies on health care providers to those seven definitional themes. Multiple reviews were conducted from 2021 to 2025 of PubMed, CINAHL, EBSCO, SCOPUS, and Web of Science using narrow search terms. Articles included were empirical studies that included three variables: COVID, health care providers, and moral injury. These searches yielded 46 qualitative and quantitative studies, and five reviews of empirical studies. Articles that did not contain these three variables were excluded. Richardson et al.’s (2020) definitional themes for moral injury included ethics, high-stress situation, orientation, betrayal, psychobehavioral wound, spiritual wound, and reconciliation. Health care providers experienced all of these. Betrayal was the strongest theme and reconciliation the weakest. Their experiences caused serious sequelae such as depression, anxiety, and posttraumatic stress disorder. In addition, their experiences led to an existential crisis, which was manifested as burnout. As a result, they considered suicide or leaving their jobs. In conclusion, moral injury is appropriate for describing the experiences of health care providers during COVID. <p><p>This briefing note examines refugee leadership in humanitarian coordination in Uganda, a country widely recognised for its progressive refugee policies and its leadership in promoting refugee participation. Uganda’s legal framework provides a strong foundation for inclusion, although gaps remain in practice due to barriers such as limited funding, information gaps, and language challenges. This can hinder meaningful engagement – especially for women, young people and people with disabilities. However, a number of well-established and steadily strengthening practices are evident, including the creation of RLO networks and platforms such as the Refugee Engagement Forum.<br>
To achieve a more effective and locally led response, this paper calls for inclusive coordination structures, direct funding and policy reforms that recognize and empower refugee leadership as a core part of humanitarian action.</p>

<p><h2>ABSTRACT</h2>
<p>In this article, I draw on ethnographic research conducted in Kabul to argue that threat production should be understood as a mechanism of racial capitalism. Based on 15 months of fieldwork, including confidential security reports, observations of segregation systems, and Afghan media accounts, the analysis shows how private security companies in U.S.-occupied Afghanistan profited from the racialized presumption of Afghan dangerousness. Through numerical rating systems, selective knowledge production, and curated media summaries, Afghans were depicted as inherently threatening. These representations were reinforced by segregation regimes that divided the city into “foreigner” and Afghan zones, alongside wage hierarchies that paid Afghan guards a fraction of their Western counterparts for identical labour. The construction of Afghans as perpetual threats generated continuous demand for security services, allowing private companies to profit by selling protection from dangers they themselves (re)produced. I suggest that this process demonstrates how racial capitalism adapts in conflict zones, creating new markets not through resource extraction or land seizure but through the continual manufacture of racialized insecurity. While grounded in Kabul, the analysis contributes to broader debates on empire, capitalism, and security by highlighting how the commodification of racialized danger is increasingly central to the global security economy.</p>
