                        <p><p><sec><st>Objectives</st></sec></p>
<p>Evidence suggests that home parenteral nutrition (HPN) can prolong survival and improve quality of life in selected patients with late-stage cancer. This study aims to identify clinical characteristics associated with the survival of Taiwanese patients with late-stage cancers receiving HPN through retrospective analysis.</p>
<p><sec><st>Methods</st></sec></p>
<p>A retrospective study of 410 Taiwanese patients with late-stage cancer unresponsive to at least three treatment lines was performed to compare survival outcomes between those receiving HPN and those receiving palliative care alone (2010–2024), matched by sex, cancer type and treatment history. Demographic, clinical, nutritional, inflammatory, performance status (PS) and do-not-resuscitate (DNR) status data were analysed using univariate and multivariate survival analyses.</p>
<p><sec><st>Results</st></sec></p>
<p>Compared with those in the palliative care group, patients in the HPN group were younger, with fewer having Eastern Cooperative Oncology Group PS (ECOG PS) score >2, fewer DNR consents and were more likely to be receiving ongoing cancer treatment. Median survival was significantly longer in the HPN group (77 vs 17 days, p<0.001). ECOG PS score >2, DNR consent and modified Glasgow Prognostic Score of 2 were independently associated with shorter survival, while the use of HPN was linked to reduced mortality risk. The negative impact of DNR consent on survival was more pronounced in patients aged <65 years.</p>
<p><sec><st>Conclusion</st></sec></p>
<p>HPN may prolong survival in selected Taiwanese patients with late-stage cancer, with PS, nutritional and inflammatory markers and DNR status serving as key factors for identifying those most likely to benefit.</p>

<p>This study aims to assess the prevalence of insomnia in patients with cancer and evaluate its association with overall survival, considering related symptoms such as anxiety and pain.</p>
<p><sec><st>Methods</st></sec></p>
<p>A prospective cohort study was conducted at Habib Bourguiba University Hospital in Sfax, Tunisia, involving adult patients with cancer. Data were collected through validated sleep assessment tools (Insomnia Symptom Questionnaire and Insomnia Severity Index). The impact of insomnia, alongside associated symptoms, on survival was assessed using Kaplan-Meier analysis and Cox regression.</p>
<p><sec><st>Results</st></sec></p>
<p>Among 100 patients, 78% reported insomnia, with pain and anxiety being the most common triggers. Insomnia was significantly associated with poorer overall survival (p<0.0001). Multivariate analysis revealed insomnia (HR=2.85, p=0.02) and metastatic disease (HR=3.21, p=0.01) as independent predictors of reduced survival.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Insomnia, along with related symptoms like anxiety and pain, is highly prevalent among patients with cancer and significantly associated with worse survival outcomes. This highlights the need for systematic screening and management of sleep disturbances, as well as a broader focus on symptom clusters in oncology care.</p>

                        <p><p><sec><st>Objectives</st></sec></p>
<p>To explore the level and nature of burden among primary caregivers of people with palliative care needs living in rural Nepal and to understand the sources of support available to them.</p>
<p><sec><st>Method</st></sec></p>
<p>An explanatory sequential mixed-method design was used. A house-to-house survey in two rural districts identified people with palliative care needs. Primary caregivers were invited to complete a structured questionnaire including a modified Caregivers’ Burden Scale in End-of-Life Care. A purposive subsample then took part in semi-structured interviews exploring caregiving experiences. Quantitative data were analysed descriptively; qualitative data were analysed thematically. Finally, both data sets were integrated and triangulated to gain deeper insights.</p>
<p><sec><st>Results</st></sec></p>
<p>52 caregivers of 58 people with palliative care needs participated; 42 (81%) were women, most commonly daughters-in-law (44%) and wives (23%). 83% reported high levels of emotional, physical and social burden, often feeling isolated and exhausted. Families provided most support, followed by neighbours. Only 38% reported any support from health professionals and 69% had little or no knowledge of the cared-for person’s clinical condition.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Primary caregivers in rural Nepal are predominantly women who experience multidimensional burden, with limited information and formal support. Palliative care services should recognise and respond to caregiver needs, building on family and community resources and empowering caregivers within primary care-based models of palliative care.</p>

                        <p><p><sec><st>Objectives</st></sec></p>
<p>The emergence of viral memes about oral nicotine products (ONPs) on TikTok is concerning, as popular content on a youth-centred platform may motivate youth use. We investigate when the viral emergence of (ONP) TikTok videos occurs in relation to accelerated growth of ONP sales.</p>
<p><sec><st>Methods</st></sec></p>
<p>We used NielsenIQ aggregated US sales data to identify sales growth inflection points from 1 January 2021 to 28 October 2023 and TikTok’s research application programming interface (API) to collect ONP-related videos posted during the same time frame. Video view counts were aggregated at the same 28-day intervals as sales data to identify viral spikes in views. Joinpoint regression identified the timing of any key inflection points in the sales data.</p>
<p><sec><st>Results</st></sec></p>
<p>ONP content first emerged in an initial viral spike in views between September 2022 and January 2023 (14.4% of views), followed by a second between July and October 2023 (80.9% of views). Joinpoint regression identified a single inflection point where the average percentage change across the 4- week period significantly increased from 2.94% to 6.22% in April 2023, following the first spike and preceding the second.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Following >3 years of steady market growth, a viral spike in TikTok views precedes a significant acceleration in ONP sales followed by a larger spike in views. Although further research is needed to determine whether TikTok views offer a reliable predictor of period-to-period sales, our findings suggest that spikes in specific content signalling effective penetration of youth subcultures may be indicative of acute market changes.</p>

                        <p><p><sec><st>Objective</st></sec></p>
<p>To review and synthesise European evidence identifying loopholes in ‘Tobacco Advertising, Promotion and Sponsorship’ (TAPS) regulations and actions undertaken by European countries to curb TAPS.</p>
<p><sec><st>Data sources</st></sec></p>
<p>We conducted a comprehensive search across seven databases (Medline/PubMed, Embase, Web of Science, Sociological abstracts, Global health, CINAHL and PsycInfo) along with grey literature from relevant tobacco control organisation websites. Searches were restricted by publication date (2005–2023), geographical setting (EU & European Free Trade Association countries) and language (English).</p>
<p><sec><st>Study selection</st></sec></p>
<p>All reviewers independently screened titles and abstracts using a purpose-designed Excel spreadsheet to identify sources addressing loopholes in national or supranational settings, deciding on inclusion accordingly.</p>
<p><sec><st>Data extraction</st></sec></p>
<p>Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews checklist, we extracted data based on the ‘Population, Concept and Context’ framework, covering tobacco product type, media communication channel and findings indicating TAPS loopholes. Two reviewers used an Excel sheet to extract data.</p>
<p><sec><st>Data synthesis</st></sec></p>
<p>Of 1477 sources, 140 were included in the review. Analysis identified five key loopholes: (a) smoking depictions and tobacco advertising in traditional mass media (films and television), (b) cross-border branding and sponsorships, (c) point-of-sale marketing, (d) e-cigarette and tobacco advertising and promotion on the internet and (e) promotion and advertising of tobacco and novel nicotine products on social media.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Curbing TAPS through comprehensive national bans and cross-border cooperation remains challenging because of the permeability of mass media, social media and other digital tools. Further research is necessary to develop strategies for effective implementation and enforcement to ensure compliance with TAPS regulations.</p>

                        <p><p><sec><st>Background</st></sec></p>
<p>We investigated whether ghost gun recovery rates are significantly associated with firearm mortality rates in the following year across California’s 58 counties from 2014 to 2023.</p>
<p><sec><st>Methods</st></sec></p>
<p>We obtained yearly county-level data on ghost guns recovered in California from The Trace’s Gun Violence Data Hub. County-level firearm death counts (total, suicide and homicide) were pulled from the Centers for Disease Control and Prevention’s Restricted-Use Vital Statistics Data. Spatiotemporal models quantified the covariate-adjusted associations between ghost gun recoveries per capita and firearm death rates (total, suicide and homicide) in the following year. Secondary analyses examined suicide and homicide models stratified by sex and race/ethnicity.</p>
<p><sec><st>Results </st></sec></p>
<p>For every 20 ghost guns recovered per 100 000 population, there was an associated 6.4% increase in firearm suicide rate (adjusted incidence rate ratio (aIRR): 1.064, 95% credible interval (CrI) 1.019 to 1.111) in the following year. We found no evidence of a significant ghost gun recovery association with total firearm death rate (aIRR: 1.036, 95% CrI 0.999 to 1.075) and firearm homicide rates (aIRR: 1.002, 95% CrI 0.946 to 1.064). Stratified models for firearm suicide rates suggested variations across sex and racial/ethnic groups, with significant positive associations observed for male (6.5% increase; aIRR: 1.065, 95% CrI 1.017 to 1.115), non-Hispanic white (6.2% increase; aIRR: 1.062, 95% CrI 1.005 to 1.122) and Hispanic (12.6% increase; aIRR: 1.126, 95% CrI 1.031 to 1.230) individuals. A different pattern emerged for firearm homicide death rates, where associations across demographic groups were not statistically significant.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Practitioners concentrating on suicide prevention efforts should be advised about the threat that ghost guns may present.</p>

<h2>Objective</h2>
<p>Transdiagnostic psychotherapies offer potential advantages to traditional, disorder-specific approaches, including coverage of comorbidity, dissemination and implementation, and patient access. Although initial investigations testing the efficacy and capability to be disseminated exist, very few studies have tested these interventions within group settings.</p>
<h2>Methods</h2>
<p>The present study involved a randomized clinical trial of Group Transdiagnostic Behavior Therapy (G-TBT) and matching group disorder-specific therapies (G-DST) in 243 veterans with major depressive disorder (MDD) and posttraumatic stress disorder (PTSD). Clinical symptom outcomes, patient satisfaction and participation, and access in terms of days between intake and the initiation of treatment were examined. G-DSTs were Cognitive Behavioral Therapy for Depression and Cognitive Processing Therapy for PTSD. Groups were initiated as soon as at least 6 participants were assigned.</p>
<h2>Results</h2>
<p>As expected, clinical outcomes for MDD and PTSD symptoms were consistently reduced in G-TBT within principal diagnostic groups, as well as associated symptoms and functional impairment. These outcomes were generally consistent with outcomes in the G-DSTs. While therapy satisfaction was similar across groups, significantly faster group assignment and less attrition were observed in G-TBT compared to G-DSTs.</p>
<h2>Conclusions</h2>
<p>Together, the present findings provide support for the efficacy of G-TBT and with comparable effects to well-established G-DSTs for MDD and PTSD, but with potentially improved access to care via transdiagnostic assignment compared to tradition disorder-specific assignment for group therapies.</p>

                        <p><h2>ABSTRACT</h2>
<p>The link between attitudes and social identity is complex, influencing perceptions, motivations, and actions. Social psychological research mainly focused on the role of attitude in identity formation, particularly in the contexts of social movements and collective action. However, there is a visible lack of research on attitude-identity interactions during violent conflicts and in non-American settings. Using the concepts of homogenization or diversification of identities and war attitudes, the present study explores how the ongoing war in Ukraine influences identities, attitudes, and interactions between the two. Based on an analysis of 50 interviews with opinion makers in Ukraine, this research demonstrates that the existence of a significant Russian-speaking minority in Ukraine, as well as other ethnolinguistic minorities, leads to contesting attitudes and meanings of identity as the population struggles to make sense of the war and the nation. As a result, distinct patterns of interconnections between national identity and attitudes toward the war emerge, demonstrating the diversification dynamics of identity and attitudes in this case. We identified ethnocultural, civic, and multicultural meanings of national identity, each connected to a specific set of attitudes toward the war, peace, and the future of Ukraine. Thus, while the war increased social cohesion in Ukraine, attitudes toward the conflict and the nation’s future differ significantly depending on the meaning of national identity.</p>

                        <p><p><sec><st>Objectives</st></sec></p>
<p>This study describes the prototype testing and clinical validation of the Fit-Frailty App, a fully guided, interactive mobile health (mHealth) app to assess frailty and sarcopenia. This multi-dimensional tool is freely available on the App Store and considers medical history, physical performance, cognition, nutrition, daily function and psychosocial domains. To guide management, a total frailty score and clinical summary of underlying “risk flags” are provided. Our objectives were to examine usability, feasibility, criterion and construct validity.</p>

<sec><st>Methods</st></sec></p>
<p>The primary outcome of the clinical validation study was criterion validity. A research nurse administered the Fit-Frailty App during a routine clinic appointment. Clinicians simultaneously completed a paper-based frailty index (FI) tool with similar items from a comprehensive geriatric assessment (FI-CGA). Total scores for both assessments were computed using the cumulative deficits frailty index scoring method. Intraclass and Pearson correlation coefficients and 95% CIs were calculated to examine criterion validity. Secondary outcomes were construct validity, feasibility (eg, completion rates, safety occurrences, resources) and usability (eg, ratings on ease of use, time to complete the app).</p>
<p><br>
<sec><st>Results</st></sec></p>
<p>In the clinical validation study (n=75, mean age 79.2, SD=7.0, 53% female), the mean total Fit-Frailty App score was 0.33 (SD=0.13) with 73% of our sample considered frail or severely frail. The app presented comparable results to FI-CGA (moderate to good validity; ICC=0.65, 95%CI=0.50–0.76) with a strong association between the measures (r=0.74, 95%CI=0.62–0.83). In our prototype and clinical cohorts, the app had a 100% completion rate with no safety occurrences and had high usability ratings.</p>
<p><br>
<sec><st>Conclusions</st></sec></p>
<p>The Fit-Frailty App is a feasible and valid tool that can be used in research and clinical settings to comprehensively assess frailty and sarcopenia by non-geriatricians and could assist with developing targeted interventions.</p>
