                        <p><p><strong>Background:</strong> Just-in-time adaptive interventions (JITAIs) use real-time monitoring to deliver personalized support at optimal moments, demonstrating potential for improving lifestyle behaviors in weight management. <strong>Objective:</strong> This study provides an overview of how JITAIs have been used or developed for weight management in adults with excess body weight. <strong>Methods:</strong> This scoping review followed Arksey and O’Malley’s 5-step framework and the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews) checklist to ensure methodological rigor. Eight electronic databases (PubMed, Cochrane Library, Embase, CINAHL, PsycINFO, IEEE Xplore, Scopus, and Web of Science) were searched from journal inception to November 13, 2024, along with gray literature and hand-searched references. Two independent reviewers conducted data extraction for all included studies. Descriptive statistics were used to summarize study characteristics, followed by a nonlinear, inductive qualitative content analysis of the extracted data to identify and synthesize recurring concepts and characteristics of JITAI-based weight management interventions. <strong>Results:</strong> Thirty-five studies on JITAIs for weight management were included, focusing on dietary behavior (25/35, 71.4%), physical activity (20/35, 57.1%), and self-weighing (17/35, 48.6%). Types of support included prompts (n=33), feedback (n=24), recommendations of coping strategies (n=7), and educational information (n=5). A total of 31.4% of studies used machine learning for decision-making, while the rest used rule-based algorithms. Retention rates varied from 74% to 100%, and compliance from 15.1% to 94.6%. Greater user engagement was associated with improved weight loss outcomes. Across interventions, significant improvements were observed in weight, waist circumference, BMI, and blood pressure, alongside increased physical activity, healthier dietary behaviors, and reductions in sedentary time. <strong>Conclusions:</strong> While JITAIs show potential for improving lifestyle habits by providing the right intervention at the right time and in the right setting, most studies lacked theoretical grounding and were not conceptualized as JITAIs. Furthermore, terminology and reporting were inconsistent, which hindered evaluation and comparison across studies. Nevertheless, most studies incorporated varied distal and proximal outcomes, behavioral theories, intervention delivery methods, and data acquisition methods, and demonstrated positive outcomes in weight, physical activity, and dietary behaviors. This review demonstrates JITAIs’ potential in weight management but highlights the field’s early stage of development. Future research should focus on improving reporting standards, optimizing JITAI components such as the integration of behavioral theories and machine learning, and enhancing user engagement and long-term effectiveness by incorporating passive sensing, personalization, and adaptive feedback mechanisms. <strong>Trial Registration:</strong></p>

                        <p><p><strong>Background:</strong> Digital therapeutics (DTx) for children and adolescents with mental health problems have been developed in the health care industry. Despite reports of side effects from DTx for children and adolescents, there have been no guidelines to address the prevention of DTx overdependence among young users. <strong>Objective:</strong> This study aimed to identify the requirements for guidelines to prevent DTx overdependence in children and adolescents and to develop and evaluate these guidelines. <strong>Methods:</strong> We conducted 2 phases. This study first involved a phase I survey to develop guidelines, including assessments of smartphone usage and mental health conditions. The second phase evaluated the guidelines’ effectiveness, reliability, necessity, and satisfaction using a visual analog scale through a randomized controlled trial. Participants—45 children and adolescents aged 9-16 years and 42 caregivers—were randomly assigned to the experimental and control groups. <strong>Results:</strong> Phase I revealed that blocking mobile applications and notifications (mean 8.5, SD 1.8) and parental monitoring (mean 8.5, SD 2.1) were effective preventive features. Caregivers, children, and adolescents expressed concerns about the side effects and overdependence of DTx and decreased effects due to nonindividualized guidelines in subjective responses to the phase I survey. Based on these insights, personalized guidelines for phase II were developed, in which overall mean visual analog scale scores for guideline evaluation were higher in the experimental group, except for necessity among caregivers (mean 8.5, SD 1.3 versus mean 8.7, SD 1.2). <strong>Conclusions:</strong> Both caregivers and children and adolescents demonstrated the need for guidelines to prevent overdependence on DTx distinct from smartphone usage. Tailored guidelines may be acceptable for use in real-world therapeutic protocols. Guidelines to prevent overdependence on DTx in children and adolescents and to achieve a balance between their benefits and risks need to be established. <strong>Trial Registration:</strong> Clinical Research Information Service (CRiS) of the Republic of Korea KCT0008893; https://cris.nih.go.kr/cris/search/detailSearch.do?seq=25609</p>

                        <p><p><sec><st>Objective</st></sec></p>
<p>Intrauterine device (IUD) user rates remain below 5% in low-income countries yet fertility after first-trimester abortions returns within 2 weeks. IUDs provide effective contraception. This study set out to explore the risk factors for IUD expulsion after medical management of first-trimester incomplete abortions.</p>
<p><sec><st>Design</st></sec></p>
<p>Prospective cohort study</p>
<p><sec><st>Setting</st></sec></p>
<p>Multicentre study at five public health facilities in central Uganda.</p>
<p><sec><st>Participants</st></sec></p>
<p>1050 women with first-trimester incomplete abortion managed with misoprostol, recruited on giving informed consent.</p>
<p><sec><st>Intervention</st></sec></p>
<p>After selecting either copper or levonorgestrel (LNG) IUDs, participants were randomised to early (within 1 week) or standard (at 2–4 weeks) insertion and assessed on IUD expulsion 6 months later.</p>
<p><sec><st>Main outcome measures</st></sec></p>
<p>Primary outcome was IUD expulsion rates at 6 months. Secondary outcomes were risk factors for IUD expulsions.</p>
<p><sec><st>Results</st></sec></p>
<p>Between 8 July 2023 and 31 May 2024, 532 (50.7%) participants chose LNG IUDs, 488 (46.5%) chose copper IUDs, while 30 (2.9%) participants chose not to use IUDs. The IUD expulsion rate was 4.6% (95% CI 3.48 to 6.07). IUD expulsion was significantly associated with low overall satisfaction with IUD insertion procedure and use (adjusted odds ratio (aOR)=7.99, 95% CI 4.83 to 13.22, p<0.001), anxiety during the IUD insertion (aOR=4.28, 95% CI 1.09 to 16.85, p=0.038), use of ultrasound at follow-up (aOR=8.41, 95% CI 4.56 to 15.5, p<0.001) and breastfeeding at the time of IUD insertion (aOR=1.48, 95% CI 0.26 to 4.98, p=0.042).</p>
<p><sec><st>Conclusion</st></sec></p>
<p>Offering IUD insertion immediately after medical management of first-trimester incomplete abortion is associated with low expulsion rates and should be offered as a safe choice.</p>

                        <p><p><sec><st>Background</st></sec></p>
<p>Recent research has shown that women often report that their concerns are not listened to in exchanges with clinicians when accessing reproductive healthcare, including contraceptive consultations. When people do not feel listened to, or have their concerns taken seriously, this can lead to disengagement with contraceptive services and distrust of healthcare professionals.</p>
<p><sec><st>Methods</st></sec></p>
<p>A participatory approach, with 18 volunteer researchers interviewing 52 friends and family members about information-seeking and contraceptive methods.</p>
<p><sec><st>Results</st></sec></p>
<p>Most participants were not satisfied with contraceptive consultations and felt that healthcare professionals did not give them all the information they wanted. Because of this, they found personal stories about contraception to be very important in their information-seeking and decision-making processes. Sharing these stories, both what they heard from others and their own experiences, helped them consider the potential side effects of different contraceptive methods. Participants thereby drew on vernacular knowledge (unofficial, informal and everyday culture of a group), some of which was accessed through social media. Vernacular knowledge also included experiential knowledge, as provided by friends and family members. This kind of knowledge was not covered in their contraceptive consultations.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>Patients’ understanding of side effects does not always align with medical evidence. Dismissing reported side effects can undermine patient experience, leading to disengagement or the seeking of counter-expertise. Practitioner knowledge and patient vernacular knowledge should be considered together when counselling about contraceptives. Our research suggests that a more prominent culture of active listening is required in clinician–patient exchanges that acknowledges and works with patient concerns and lived experiences.</p>

                        <p><p><sec><st>Introduction</st></sec></p>
<p>Physical pain is known to be a common feature of sexual and reproductive healthcare (SRH) encounters for women and other people with a uterus. Our aim was to understand experiences of painful procedures in SRH care and to identify priorities for change.</p>
<p><sec><st>Methods</st></sec></p>
<p>In January 2025, we conducted an online qualitative survey of people with lived experience of painful SRH procedures. Focused coding and thematic data analysis was supported by Nvivo 14.</p>
<p><sec><st>Results</st></sec></p>
<p>Analysis of the 149 qualitative responses highlighted four key thematic areas: how painful procedures felt at the time; how they made participants feel in the longer term; the impact on decisions around future procedures; and what, from participants’ perspectives, could be done differently. Our results illustrate the impacts of pain in the short and longer term, as well as ways in which painful experiences can shape future engagement with SRH care. Specifically, they highlight the distress, fear and distrust of health professionals and services which these experiences can generate.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>While best practice guidance exists for some potentially painful SRH procedures, women and other people with a uterus in the UK continue to report highly negative experiences. Trauma-informed approaches – recognising the role of past experiences and supporting compassionate, respectful patient interactions – could improve training and awareness among health professionals, and broader shifts in attitudes and practice.</p>
