<st>Background</st>

Cold weather remains a serious health threat in the UK and elsewhere, particularly for older adults. The Winter Fuel Payment has been a key government strategy to mitigate health risks linked to cold homes in the UK, but recent policy shifts have raised questions about whether income-based eligibility criteria effectively identify those most at risk.

<st>Methods</st>

We analysed cold-related mortality in adults aged ≥75 across 324 local authority districts in England (2007–2019) using distributed lag non-linear models in a spatial Bayesian framework. Multivariate meta-regression was used to evaluate modification of cold effects by deprivation, income-based pension credit uptake, home energy efficiency and fuel poverty.

<st>Results</st>

Areas in the highest quartile of fuel poverty had significantly greater cold-related mortality risk than those in the lowest quartile, with a 15.3% versus 13.1% increase in mortality risk at the first compared with the 50th percentile of wintertime temperature, ie, an absolute difference of 2.2% (p<0.001). This effect was stronger than the corresponding differences for energy efficiency (1.7%, p=0.04), income as indicated by pension credit uptake (0.6%, p=0.39) and deprivation-based measures, for which differences were minimal. Overall, an estimated 17% of cold-related deaths among people aged ≥75 were attributable to fuel poverty.

<st>Conclusion</st>

Fuel poverty, an indicator designed to capture both low-income and housing energy efficiency, is a stronger predictor of cold-related mortality than income (as indicated by pension credit update) or deprivation-based indicators alone. Winter energy support schemes should consider fuel poverty metrics in their targeting to more effectively reduce health risks associated with cold homes and improve equity.

ABSTRACT

Background

Stroke is a leading cause of disability and mortality in the Philippines, where access to formal rehabilitation services is limited. There is a lack of research on the lived experiences of people with stroke and their household carers across diverse urban and rural settings.

Objective

To explore the experiences, challenges and support needs of people with stroke and their household carers throughout the Philippines, and to inform the co-design of a community-based stroke support programme.

Methods

A descriptive qualitative design was used. Semi-structured interviews were conducted with 24 people with stroke and 20 household carers across six sites in Luzon, Visayas and Mindanao. Interviews were enriched by auto-photography and visual elicitation. Data were analysed thematically using an interpretivist approach, through collaborative analysis by UK- and Philippine-based researchers.

Results

Four key themes were identified: (1) Multidimensional burden of stroke for people with stroke and household carers, including physical, psychological, social and financial impacts; (2) Cultural management and coping practices, emphasising the central role of family, use of traditional therapies and adaptive strategies, rooted in cultural values such as ‘pagtitiis’ (resilience and endurance) and ‘utang na loob’ (reciprocal obligation); (3) Knowledge and awareness of stroke and rehabilitation, revealing significant gaps and reliance on personal experience and informal information sources; and (4) Stroke care and rehabilitation needs, challenges, and recommendations, showing limited service availability and geographical and financial barriers, particularly in rural areas, alongside a strong desire for accessible, community-based support.

Conclusion

Culturally relevant, gender-sensitive and accessible community-based stroke support and education programmes are urgently needed in the Philippines to address unmet needs and inequities in immediate and longer-term care.

Patient or Public Contribution

A PPIE group consisting of people with stroke and other physical disabilities and carers reviewed the study protocol, interview topic guides and participant-facing documents and provided feedback on initial themes.

ABSTRACT

Parental beliefs and practices regarding motor development vary across cultures. However, there is a gap in cross-cultural studies focusing on minority groups. This mixed-methods study explores the cultural construction of infant motor development within the Druze-Israeli (Druze-IL) minority group. Using questionnaires, we measured beliefs, practices, physical settings, and infant motor skills among 83 Druze-IL mothers comparing them with a matched group of 83 Jewish-IL mothers, representing Israel’s majority group. Moreover, qualitative insights were gleaned from semi-structured interviews with eight Druze-IL mothers. Results indicated that Druze-IL mothers prioritised stimulating motor development and valued expert guidance, while also perceiving it as a natural process. For example, they were less supportive of the prone position and preferred the upright position compared to Jewish-IL mothers. This preference was congruent with Druze infants’ superior skills in upright postures compared to Jewish-IL infants. Interviews highlighted that Druze mothers balance traditional customs with expert advice, stressing the importance of attuning to their infants’ needs to determine the suitability of various practices. The findings enrich our understanding of the socio-cultural construction of motor development and illustrate how Druze mothers negotiate influences from both minority and majority cultures in shaping their beliefs, practices, and ultimately, their infants’ motor skills.

ABSTRACT

Objective

While high caloric realimentation (HCR) is increasingly recommended for quick weight restoration of patients with anorexia nervosa (AN), the development of AN-specific psychopathology and somatic symptoms during HCR have so far insufficiently been studied.

Method

Patients hospitalised for the treatment of AN received oral realimentation starting with 2000 kcal under close medical monitoring and a substitution regimen. Weekly over the initial 6 weeks of treatment, assessments included body weight, drive for thinnesss, body dissatisfaction, depression, and somatic symptoms.

Results

In 46 patients (mean age 27.57 years, 2 males, 44 females), the body mass index increased from 12.6 (SD 1.1) at baseline to 14.4 (SD 1.0) kg/m2 (p < 0.001) at week 6 (d = 2.57). Drive for thinnesss did not change (28.9 [SD 10.2] to 27.7 [SD 10.8]; p = 0.102), while body dissatisfaction increased slightly from 37.3 (SD 7.8) to 39.1 (SD 9.2; p = 0.019; d = 0.41). Depression and somatic symptoms decreased significantly (p < 0.001) with large effect sizes (d = 1.09, d = 0.90, respectively). No case of refeeding syndrome was encountered.

Discussion

HCR was medically safe and associated with a decrease in depressive and somatic symptoms at only slight increase in body dissatisfaction. Fast realimentation brings patients with extreme AN quickly out of a weight range dangerous to their health.

ABSTRACT

Individuals’ belief in a just world is related to a complexity of personal and social consequences. Understanding the development of justice belief can provide valuable information about its determinants and functions across the lifespan. The current study aims to investigate age-related differences in belief in a just world across adulthood and how the age differences are moderated by cultural values. Responses from 81,543 individuals across 57 societies were analysed. Individual-level information was obtained from the sixth wave of the World Values Survey, and society-level individualism and flexibility scores were obtained from Minkov and Kaasa’s work on the two-dimensional model of cultural values. Hierarchical linear modelling was conducted to analyse the data and revealed a robust positive association between age and the belief in a just world. Moreover, society-level flexibility, but not individualism, strengthened the positive association between age and the belief in a just world. A positive main effect of flexibility on the belief in a just world was also found. The study revealed age-related differences in justice belief across societies. The findings suggest that the development of the belief in a just world is jointly influenced by individuals’ personal motivational needs for justice and the cultural context.

ABSTRACT

Background

Resilience, the ability to adapt and thrive under adversity, is essential for nursing students vulnerable to stress, burnout, and psychological distress. Strengthening resilience may protect well-being and enhance patient care.

Methods

This feasibility study employed a one-group pretest–posttest design. A face-to-face stress management and resilience training (SMART) programme was delivered to 40 nursing undergraduates at a self-financing institution in Hong Kong. Resilience and stress were measured at baseline (T0), 2 weeks (T1), and 3 months (T2) using the Connor-Davidson Resilience Scale-10 (CD-RISC-10) and Perceived Stress Scale-10 (PSS-10). A post-training satisfaction survey was also conducted.

Results

Thirty-one participants completed all assessments. CD-RISC-10 scores increased significantly from 22.00 (SD = 4.11) at T0 to 27.81 (SD = 7.39) at T1 and 26.42 (SD = 6.45) at T2. Improvements were significant between T0–T1 (p < 0.001) and T0–T2 (p = 0.004). PSS-10 scores declined slightly (18.61 at T0 to 17.65 at T2) but without statistical significance (p = 0.123). Satisfaction ratings were high (mean 5.38–5.60/6), particularly for trainer performance and content.

Conclusion

The SMART programme was feasible, well-received, and effective in improving resilience among Hong Kong nursing undergraduates. Findings support its potential integration into undergraduate nursing curricula.
