                        <p><p><sec><st>Background</st></sec></p>
<p>Pharmacotherapy has a strategic role in palliative care (PC). Medications are prescribed in this context for various reasons, leading to polypharmacy, associated with several negative outcomes. Despite its clinical relevance, the scientific literature about pharmacological interventions in PC remains scarce and fragmented. This review aimed to explore and map the pharmacological interventions in PC.</p>
<p><sec><st>Methods</st></sec></p>
<p>A bibliometric review was conducted using combined data from Scopus and Web of Science. Inclusion criteria covered original articles and reviews, published between 2014 and 2024, that had terms related to PC present in the title and terms related to the use of medications present in the title, abstract or keywords. RStudio and VOSviewer were used to analyse data, focusing on publication trends, authorship, keywords and thematic evolution.</p>
<p><sec><st>Results</st></sec></p>
<p>A total of 2245 publications were included, mainly original articles (80%), although only 10% were clinical trials. Highest annual production was found in 2024, concentrating mainly in high-income countries. The term “symptom management” appears as a central term in keyword analysis; it also appears, along with “deprescribing” and “polypharmacy”, in trending topics evolution, growing in 2019. Data indicate that pharmacotherapy is not yet a consolidated theme itself, being addressed mainly in more general references.</p>
<p><sec><st>Conclusion</st></sec></p>
<p>Fragmentation hinders the advancement of key issues such as the identification and management of potentially inappropriate medications, therapeutic rationalisation at the end of life and the pharmacoeconomic impact of these prescriptions and interventions. The field of palliative pharmacotherapy should be considered as a possibility for future literature development.</p>

<div class="boxTitle">Abstract</div>
<p>Good communication skills are essential for effective social work practice, yet evidence-based communication approaches are rarely applied in child and family social work. Motivational interviewing (MI) has shown promise in enhancing practitioners’ skills and outcomes in child and family social work, but its use outside the UK remains understudied. This study evaluates improvements in social workers’ communication skills following MI training tailored to child and family social work in Finland. The training included coaching sessions and a self-practice programme informed by behaviour change science. Thirty-three Finnish social workers participated. Recordings of simulated and real service user meetings were analysed using the Social Work and Interviewing Motivationally (SWIM) coding tool. Of the seven measured communication skills, only empathy showed statistically significant improvement at the group level. Due to the small sample size, statistical power was limited. Notably, person-based analyses identified three subgroups: nine participants improved, four declined, and the rest showed mixed changes. Exploratory survey analyses suggested that differences in practice, self-efficacy, and intentions influenced outcomes. These findings underscore the need to move beyond group averages to examine individual responses to training. Such detailed analyses can help optimize interventions and improve the effectiveness of communication skill development in social work.</p>

                        <p><p>Studying patient experience has become increasingly important for providing alternative perspectives on healthcare and quality improvement. Understanding these experiences contributes to improving quality of care and aligning healthcare delivery with patients’ preferences and values. One method for exploring such experiences is the use of film. Examples such as Experience Based Co-Design (EBCD) and Database of Individual Patient Experiences exemplify how film can be employed to collect and present patient narratives. However, what remains largely absent in this work is the incorporation of cinematographic ideas and practices, an area that has been explored extensively within the field of visual anthropology.</p>
<p> To demonstrate how visual anthropology can inform the use of film in healthcare research, particularly studies that focus on patient experience, this paper highlights three lessons. First, attention to the multimodal qualities of film enables researchers to explore how senses, emotions, images and actions interplay within lived experience. Second, film provides opportunities to experiment with and represent the movement of time through montage, emphasising both the intensity of particular moments and the subjective experience of time. Third, the open-endedness of film fosters a wide range of possibilities for collaborative sense-making. Meaning in film is never fixed but continually in a process of becoming, offering researcher and practitioners new ways to think and rethink common practices and approaches to healthcare improvement.</p>

                        <p><p><sec><st>Background</st></sec></p>
<p>Transforming the scope of global health to become more equitable, especially in conditions such as sickle cell disease (SCD), has been the subject of increasing attention. It has garnered significant focus within the National Health Service (NHS) due to rising patient concerns about pain undertreatment, institutional racism and the need for culturally competent care. As much of SCD research originates from the USA, a gap exists in understanding the factors influencing pain expression and perception in SCD care in the NHS, especially from the perspectives of clinicians and medical trainees in the UK.</p>
<p><sec><st>Methods</st></sec></p>
<p>This study employed virtual semi-structured interviews with clinicians and medical students, through purposive and convenience sampling, to assess individual, social and cultural factors affecting pain perception and assessment in SCD care. The data were analysed using an abductive approach, combining inductive and reflexive thematic analysis with deductive refinement to align emerging themes with the research question. Considerations were made for the ethical framework of this study, including confidentiality and counselling available to participants, as well as for my positionality as a researcher with SCD.</p>
<p><sec><st>Results and discussion</st></sec></p>
<p>Four main themes emerged in exploring factors affecting pain perception in acute care, including (1) Knowledge gaps, (2) Cultural attitudes towards pain tolerance and (3) Biases misidentifying patients with SCD as ‘drug seekers’. The fourth and final theme, patient voices in education, proposed as a solution to addressing all of the above, underscores the importance of integrating experiences of patients with SCD into medical training to develop culturally competent care practices for improved pain management.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>As someone with lived experience of SCD who has held roles as a researcher and an advocate, this study has made me more driven to put patient voices within published literature because service provision, in any setting, which is not informed by patient experiences, will continue to miss the needs of the users.</p>

                        <p><p>This essay explores the enduring relevance of Isaiah Berlin’s fox and hedgehog metaphor, famously applied to Tolstoy, to contemporary psychiatric practice. Psychiatry, historically grounded in rich theoretical traditions, has seen its focus shift over time towards structured diagnostic systems that emphasise clarity and manageability. While these developments brought important clinical advances, they also risked reducing the complexity of mental suffering to narrow categories. Drawing on phenomenological psychiatry and recent psychotherapy research, this essay argues for a renewed emphasis on subjective experience, patient agency and co-constructed meaning. Tolstoy’s scepticism towards grand explanatory narratives and his sensitivity to the contingent, fragmented nature of human behaviour resonate deeply with these contemporary concerns. However, a fully Tolstoyan stance poses its own risks too: loss of patient agency, clinician disengagement and increased vulnerability to burnout.</p>
<p>The essay proposes that psychiatry must navigate the tension between coherence and complexity, resisting both rigid theoretical allegiance and total relativism. Rather than choosing between the hedgehog’s unifying vision and the fox’s multiplicity, clinicians are called to adopt a more flexible and ethically grounded position. The concluding metaphor of the sphinx suggests a practitioner capable of holding multiple perspectives, sustaining meaning and acting with discernment amid uncertainty. In this light, Tolstoy’s unfinished worldview may offer not only a literary insight but also a profound clinical ethic.</p>

<p>Cervical cancer is predominantly caused by infection with high-risk human papillomavirus (HPV). Prevention efforts rely on vaccination and cervical screening. This study aimed to determine HPV vaccination and in-date cervical screening rates among women seeking abortion to determine whether this care setting could provide an opportunity to deliver catch-up prevention services.</p>
<p><sec><st>Methods</st></sec></p>
<p>Retrospective database study examining HPV vaccination and cervical screening coverage among women accessing abortion in Edinburgh, Scotland, UK from October to December 2024. National Health Service electronic systems recording HPV vaccinations and the Scottish Cervical Call Recall System were examined together with clinical records to determine if an in-person clinical visit was made as part of care.</p>
<p><sec><st>Results</st></sec></p>
<p>Some 798 individuals presented for abortion over the study period (n=297, ≤25 years and n=501, >25 years). Of those aged ≤25 years reviewed for HPV vaccination, 161/297 (54.2%) were vaccinated, 37 (12.5%) were unvaccinated and 99 (33.3%) had no record of vaccination status. Of the 501 women of cervical screening age >25 years, 289 (57.7%) were up to date, 70 (14%) were overdue and 142 (28.3%) had never been screened. Of the 37 unvaccinated individuals, 29 (78.4%) attended the clinic in person as did 158 (74.5%) of the 212 overdue/never-screened women.</p>
<p><sec><st>Conclusions</st></sec></p>
<p>A significant proportion of those individuals who present for abortion are not vaccinated against HPV or up to date with cervical screening. Three-quarters of this group made an in-person visit, indicating an opportunity for delivering cervical cancer preventative services in the future. Future research is required to determine the feasibility of HPV vaccination and cervical screening in this setting.</p>

                        <p><h2>ABSTRACT</h2>
<h2>Objective</h2>
<p>This study provides an analysis of the epidemiology of vision/hearing impairments among frail older adults in New Zealand.</p>
<h2>Methods</h2>
<p>Data of older adults assessed using standardised interRAI Home Care and Long-Term Care Facility instruments between 1 January 2019 and 31 December 2020 underwent retrospective analyses of vision and hearing impairment. The use of vision and hearing examinations and aids is reported.</p>
<h2>Results</h2>
<p>We analysed 48,038 assessments comprising 65% females and a mean age of 82.2 (SD 8.4) years. Considering moderate difficulty or worse disability to have a meaningful impact on individuals, the prevalence was 19% for hearing and 10% for vision impairment. Dual sensory impairment as per the Deafblind Severity Index was present among 7% of our sample. Regular hearing examinations were undertaken by 31% of people living at home, dropping to 21% in care facilities, whereas regular vision testing decreased from 55% to 32% among those living in care facilities. Although 72% of the sample living in an aged residential care facility used visual aids, only 20% used hearing aids.</p>
<h2>Conclusions</h2>
<p>The majority in this cohort (~80%) reported adequate vision and ability to hear or minimal impairment, and half the cohort experienced dual sensory impairment. Access to hearing and vision assessments was less among those in aged residential care compared with those receiving support at home, and examinations and aids to overcome hearing impairment were less frequently used than aids for visual impairment. Further research is required to understand the reasons behind these observations.</p>

<p>Administrative burdens vary significantly across agencies and communities, and digital government initiatives are proposed as potential solutions to reduce administrative burdens. However, disadvantaged groups, particularly seniors, face multifaceted challenges that digital technologies alone cannot fully resolve. This research, based on in-depth interviews and grounded theory analyses, examines how frontline bureaucrats, citizens, and digital tools collectively contribute to administrative burdens. We use the case of the Old Age Allowance Policy (OAAP), which was introduced by the Chinese government to address the increasing demands of the rapidly aging population. We develop a theoretical framework conceptualizing administrative burdens as perverse effects of digital transformation, shaped by intricate interactions among citizens’ conditions, bureaucratic discretion, and technological limitations. In response to these challenges, our study highlights the crucial role of the government in fostering a supportive environment that enables citizens to adapt effectively to digital transformation.</p>
<h2>摘要</h2>
<p>不同机构和社区的行政负担差异巨大,数字政府举措被认为是减轻行政负担的潜在解决方案。然而,弱势群体(尤其是老年人)面临着多方面的挑战,单靠数字技术无法完全解决。本研究基于深度访谈和扎根理论分析,探讨了一线 人员、公民和数字工具如何共同加剧行政负担。我们以中国政府为应对快速老龄化人口日益增长的需求而推出的养老保险政策为例,将行政负担理解为为政府在进行数字化转型的过程中可能造成的负面效应,其形成机制包括公民自身条件、 街头官僚的自由裁量权和技术限制之间的复杂相互作用。为了应对这些挑战,我们强调了政府在营造支持性环境、使公民能够有效适应数字化转型方面的关键作用。</p>
<h2>RESUMEN</h2>
<p>Las cargas administrativas varían significativamente entre agencias y comunidades, y las iniciativas de gobierno digital se proponen como posibles soluciones para reducirlas. Sin embargo, los grupos desfavorecidos, en particular las personas mayores, enfrentan desafíos multifacéticos que las tecnologías digitales por sí solas no pueden resolver por completo. Esta investigación, basada en entrevistas en profundidad y análisis de teoría fundamentada, examina cómo los burócratas de primera línea, los ciudadanos y las herramientas digitales contribuyen colectivamente a las cargas administrativas. Utilizamos el caso de la Política de Subsidios para la Vejez (OAAP), implementada por el gobierno chino para abordar las crecientes demandas de una población que envejece rápidamente. Desarrollamos un marco teórico que conceptualiza las cargas administrativas como efectos perversos de la transformación digital, moldeados por intrincadas interacciones entre las condiciones de los ciudadanos, la discreción burocrática y las limitaciones tecnológicas. En respuesta a estos desafíos, nuestro estudio destaca el papel crucial del gobierno en el fomento de un entorno propicio que permita a los ciudadanos adaptarse eficazmente a la transformación digital.</p>

                        <p><h2>ABSTRACT</h2>
<p>There has been renewed interest in the role of parenthood in explaining women’s career outcomes in politics. Yet the empirical understanding of how and in what circumstances the parental status of women in legislatures affects their political career is relatively under-explored. Utilizing an original dataset covering the entire population of Turkish legislators, our study focuses on the tenure duration of those who served in parliament at least once between 1920 and 2023. Our exploratory analyses show that women in parliament tend to have fewer children, compared to men. Results from a series of negative binomial regression models indicate that women’s parliamentary career lasts shorter than that of men, especially among legislators with children. Specifically, women with two or more children have significantly shorter parliamentary careers compared to women without children, as well as to men regardless of parental status. Our results lend strong support to studies conceptualizing the parliament as a gendered workplace.</p>
