<p>Fear extinction is foundational to exposure therapy; therefore, the study of strategies to optimize fear extinction is relevant for clinical practice. This article provides a critical review of translational research on pharmacological enhancement of fear extinction, concentrating mostly on <span class="jp-small">d</span>-cycloserine, the agent with the most extensive evidence base across levels of analyses. Despite early promise, results across preclinical, human laboratory, and clinical trials have been mixed. We identify factors that may account for these inconsistent findings, including differences in study design, selection of subjects, sample size, and measurement approaches. We emphasize the need for a rigorous mechanistic research agenda that both assesses extinction processes—acquisition, consolidation, and retrieval—as distinct mechanistic targets and examines the relation between changes in these fear extinction processes and clinical outcomes. Finally, we discuss opportunities to advance translational research in this area by leveraging extant collaborative infrastructures to improve the quality, the efficiency, and ultimately the availability of effective clinical strategies.</p>

<p>In this career-review-as-memoir, I interweave the deeply personal/familial roots of my abiding interests in developmental psychopathology, clinical trials, and reduction of mental illness stigma and discrimination with an overview of my variegated research on youth and young adults. I also discuss mentoring, teaching, lab-building, leadership, collaboration, synthesis, and serendipitous ideas. Lived experience and personal interest can and should inform discovery phases of scientific efforts, whereas objectivity and disinterest are essential for justification aspects. My long-term aim has been to bridge science and humanization. Although progress in clinical psychology is apparent, even passing recognition of current mental health challenges—especially for adolescents/young adults—provides an urgent call for (<span class="jp-italic">a</span>) integration of genetic/biological risk with contextual and cultural factors; (<span class="jp-italic">b</span>) provision of supportive settings, plus evidence-based treatments, for individuals and families in need; and (<span class="jp-italic">c</span>) recruiting and mentoring new generations of scholar–clinicians who will continue these essential efforts.</p>

<p>A substantial literature on the decline in support for liberal trade policies and globalization has emerged. Most of these accounts, however, do not adequately address the puzzling issue of why it took so long for governments to respond to rising discontent stemming from trade globalization’s economic effects. The literature we review suggests that government actions during this era were shaped by neoliberal principles that established powerful ideological and institutional constraints. To promote trade and growth, political leaders and policymakers were committed to reducing government’s footprint in the economy. That commitment inhibited both their interest in and ability to respond to globalization’s impact, fostering an environment ripe for antiglobalist political entrepreneurs. We argue that the neoliberal cast of the contemporary era of trade globalization, together with the associated rules-based multilateral regime that restricted member states’ flexibility, may have sown the seeds of the backlash against it.</p>

<p>Racism is an underlying cause of health inequities and is entrenched in health systems, disproportionately affecting marginalized groups. Advancing health equity requires reimagining health systems to uproot racism from health-related policymaking. Racism, in its systemic, cultural, and interpersonal forms, remains a significant threat to health equity, a barrier to reform, and a public health crisis. This review draws lessons from US social movements—including tobacco control, sexual and gender minority rights, criminal justice reform, civil rights, and reproductive justice—to identify effective strategies for change. Drawing on key theories, typologies, and insights from the literature, we examine how organizing, messaging, and mobilization have shaped narratives, have fostered public will, and have driven policy reform. Prior movements can serve as a guide for the development and implementation of a social change movement aimed at addressing racism in public health.</p>
