<p>Emotional reactivity and emotion regulation play key roles in adolescent mental health. Positive parenting behavior has been shown to facilitate positive adolescent emotional development; however, relationships between parenting and adolescent neural emotion reactivity have not been thoroughly examined. The present study employed whole-brain analysis of fMRI data to examine associations between observed parental warmth (one type of positive parenting) and adolescent neural reactivity to negative and positive emotional stimuli among a suburban, mostly upper middle-class (53% birth-assigned girls, Mage = 12.73) sample. Given previous evidence that relationships between parenting processes and adolescent neural emotion reactivity vary by youth’s assigned sex, interactions between parental warmth and assigned sex were also examined. A main effect was found in which lower parental warmth was linked to blunted neural reactivity to negative emotional stimuli in the vACC, an emotion-processing region. Further, there was a significant interaction by assigned sex, such that lower parental warmth was linked to blunted negative emotion-related neural reactivity was more so for assigned boys than assigned girls in the emotion processing, executive functioning, and sensory processing regions. Findings suggest that in parenting environments with low warmth, youth, and especially assigned boys, may be more prone to blunted neural emotion reactivity. This research suggests the importance of parental warmth in youth emotional development and negative emotion-related neural reactivity.</p>

<div class="boxTitle">Abstract</div>
<p>Cultural competence is widely recognized as a core component of equity in healthcare. However, little is known about how healthcare professionals in Greece, a frontline country for displaced populations, develop and implement cultural competence in refugee healthcare. This study explores the experiences, training needs, challenges, and strategies of Greek healthcare professionals for delivering culturally competent care to refugees and asylum seekers. Twelve healthcare professionals from various Greek healthcare settings participated in semi-structured interviews. Data were analysed using reflexive thematic analysis within an interpretive description framework, appropriate for applied health research. Three themes were developed: (i) developing cultural competence through cross-cultural engagement; (ii) finding a balance: dilemmas and complexity in refugee healthcare; and (iii) caring in isolation: refugee care in unsupportive environments. Systemic constraints and patient–provider challenges limited participants’ efforts, despite their demonstrated adaptability and commitment, which sometimes resulted in emotional exhaustion or disengagement. Findings highlight the need for multilevel interventions, combining structural competence, advocacy, and reflective supervision, to support uninterrupted, culturally responsive care and promote healthcare providers’ well-being.</p>

                        <p><p>The US overdose crisis is driving a surge in HIV diagnoses among persons who inject drugs (PWID). Innovative approaches are needed to address this increase in cases. Although HIV self-testing (HIVST) was hailed as a potential “game-changer” upon initial approval by the Food and Drug Administration over a decade ago, this convenient testing modality has not reached its full potential to impact the HIV epidemic. Nevertheless, lessons regarding self-testing for infectious diseases from the COVID-19 pandemic present an opportunity to increase HIVST uptake and reach current US goals of Ending the HIV Epidemic (EHE) by 2030. In this commentary, we first discuss facilitators and barriers of HIVST for PWID. We then explore how lessons regarding self-testing during the COVID-19 pandemic can allow us to realize the potential of HIVST for PWID. We conclude by suggesting the future utilization of HIVST to address 2 EHE pillars, rapid diagnosis of HIV cases and cluster identification.</p>

<p>The BODY-REAL Study was a 2 × 2 factorial design RCT that assessed whether providing patients with (1) visual body fat MRI reports compared with weight/body mass index (BMI) alone and (2) direct-to-patient reports versus through providers would improve cardiometabolic risk among individuals at elevated cardiometabolic risk (BMI ≥ 25 kg/m2, Hb A1C ≥ 5.7% and additional risk factors). We found that MRI report recipients had lower fasting glucose and higher physical activity scores and that provider-mediated reporting was associated with greater medication adherence and perception of personal disease control but lower perception of disease risk. The study was underpowered to detect significant change in other cardiometabolic markers but demonstrated potential for direct-to-patient visualization of body at distribution to improve cardiometabolic risk markers and preventive behaviors, warranting additional sufficiently powered studies.

<h2>ABSTRACT</h2>
<h2>Objective</h2>
<p>Advanced imaging can inform risk assessment and preventive/therapeutic decision-making to enhance care. It is not known how quantification of body fat distribution using advanced imaging and communication of the result through a graphic report may impact cardiometabolic risk factors and attitudes/behaviors among persons living with overweight or obesity. The BODY-REAL study assessed whether providing patients with (1) visual body fat MRI reports compared with the weight/body mass index (BMI) alone and (2) direct-to-patient reports versus through providers would improve cardiometabolic risk among individuals at elevated cardiometabolic risk.</p>
<h2>Methods</h2>
<p>Participants were randomized in a 2 × 2 factorial design to receive a BMI or MRI body fat distribution report, provided direct-to-patient report or a report through their healthcare provider. Clinical and psychosocial outcomes were measured at 2 weeks, 3, and 6 months and compared using analysis of covariance and linear models with both fixed and random (time-updated) effects.</p>
<h2>Results</h2>
<p>29 participants (31% female, mean BMI 33.6 kg/m<sup>2</sup>) were randomized. After 6 months of follow-up, MRI report recipients had lower fasting glucose and higher physical activity scores (<i>p</i> < 0.01). Provider-mediated reporting demonstrated greater medication adherence (<i>p</i> = 0.01) and greater perception of personal control of diabetes (<i>p</i> = 0.02) but lower perception of diabetes-related risk (<i>p</i> = 0.003). There were no significant differences in BMI, blood pressure, cholesterol, glycosylated hemoglobin, or other risk perception across study arms.</p>
<h2>Conclusions</h2>
<p>This pilot feasibility study demonstrated potential for direct-to-patient visualization of body fat distribution to improve risk markers and preventive behaviors. Further investigation in a sufficiently powered study is warranted.</p>

                        <p><p>Objective: </p>
<p>To compare a sample of pediatric primary care patients diagnosed with autism spectrum disorder (ASD) with the general clinic population, as well as associations of child and caregiver characteristics with age of diagnosis.</p>
<p>Method: </p>
<p>Cross-sectional data for patients from 2 large, urban primary care practices diagnosed with ASD between March 1, 2018, and February 28, 2022, were collected from the medical record through extraction and chart review. The sample was compared with the total primary care population using χ2 analysis. Unadjusted bivariate linear regression and multivariate linear regression were used to evaluate associations of each variable with age of diagnosis.</p>
<p>Results: </p>
<p>Patients diagnosed with ASD were more likely to be male, Hispanic, publicly insured and medically complex than the general clinic population. There was also a higher maternal education level in the autism group. In unadjusted linear regression analyses, language delay, connection with Early Intervention (EI), earlier age of parental concern, earlier age of referral, having an M-CHAT completed, higher M-CHAT scores, and having a first-degree relative with ASD were associated with earlier age of diagnosis. In multivariate linear regression analysis, connection with EI, better continuity of care, higher M-CHAT scores and having commercial insurance were associated with earlier diagnosis. Race/ethnicity, language, and Social Vulnerability Index ≥ 90%ile were not significantly associated with age of ASD diagnosis.</p>
<p>Conclusion: </p>
<p>This study reflects shifting prevalence patterns of ASD, but a persistent disparity in age of diagnosis among publicly insured children. It highlights the importance of developmental monitoring, continuity of care, and navigation support to help families obtain ASD diagnoses promptly.</p>

                        <p><h2>ABSTRACT</h2>
<p>Relational savoring (RS) is a brief, accessible, relationship-based intervention guided by systemic principles. RS may benefit Asian American, Native Hawaiian, and Pacific Islander (AANHPI) families experiencing relational difficulties. Family ties are often protective for AANHPI groups, yet acculturation-related stress may impede families’ ability to leverage this asset. RS has evidenced effectiveness in increasing well-being and relationship quality, but has yet to be tested with AANHPI families. RS may align well with cultural values relevant to AANHPI communities, such as interconnectedness, while also overcoming barriers through its strengths-based approach. This paper outlines the decision-making process used to adapt RS for AANHPI families and emphasizes the role of community feedback. An iterative and rigorous approach is essential to determine whether adaptation is needed and to identify appropriate targets for change. Importantly, enhancing the cultural responsiveness of RS furthers our goal of promoting flourishing and addressing health disparities among underserved families.</p>

                        <p><p><imgsrc border="0" align="left" alt="image">Objective </imgsrc></p>
<p>To assess the impact of the implementation of an electronic medical record (EMR)–based sexually transmitted infection (STI) order set on human immunodeficiency virus (HIV) testing rates and the acceptability of status-neutral HIV Care in the emergency department (ED).</p>
<p>Methods </p>
<p>We developed and implemented an STI order set integrated into the EMR to streamline HIV testing and status-neutral care. The cohort included ED encounters from April 2022 to December 2023 in which patients were tested for chlamydia or gonorrhea. We measured the adoption and acceptability of the order set among clinicians through surveys and analyzed the impact on rates of HIV testing and referral to Infectious Diseases using regression models.</p>
<p>Results </p>
<p>Of 3922 ED encounters, 1161 (29.6%) included an HIV test. The introduction of the STI order set in February 2023 correlated with increased testing rates for all STIs, including a significant rise in HIV testing (P </p>
