<p><h2>ABSTRACT</h2>
<p>In 1985, Arber and Sawyer described the discretionary rationing power of general practice receptionists. Our paper revisits this territory. Much has changed in the intervening decades. Digitalisation has altered reception work. Increasing multimorbidity and rising chronic illness, combined with a dwindling workforce, restricted funding and systemic pressures on public services, have fuelled the ‘crisis’ in general practice. ‘Unacceptable’ delays in getting a general practitioner (GP) appointment are seen as evidence of this. Our focused ethnography in eight English NHS general practices highlights important shifts in receptionists’ management of GP access. We observed waiting and reception areas, interviewed 70 staff and 74 patients and examined practice documents. Arber and Sawyer’s dragon metaphor remains salient, but receptionists have new strategies of bureaucratic distancing and redirection to manage appointment requests. They are gatekeepers still and remain a target for hostility but mitigate this by using these strategies. Patients on a quest to obtain an appointment with a GP may still be thwarted and sometimes meet dragons at the desk, but they may more often find themselves allied with the receptionist. The barrier to getting a GP appointment has become the access system and its discorporate digital forms, rather than the receptionist or the GPs she protects.</p>

<p><h2>Abstract</h2>
<p>In Hungary there is no standardized tool to screen early social-emotional difficulties. This study aimed to evaluate the psychometric properties of the Hungarian version of the Ages and Stages Questionnaires: Social-Emotional Second Edition for 18-month-olds (ASQ:SE-2/18). Two studies were conducted. Study 1 involved translation, cultural adaptation, and validation in a convenient sample (<i>N</i> = 423). Study 2 used a nationally representative sample (<i>N</i> = 4918) to evaluate the factor structure, internal consistency, predictive validity and socioeconomic and demographic correlates. Study 1 supported the cultural adequacy of the Hungarian ASQ:SE-2/18, and a comparison with the Child Behavior Checklist 1.5–5 provided evidence for its validity. In Study 2, the two-factor model, consisting of Emotional Difficulty (<i>α</i> = .74) and Social Difficulty (<i>α</i> = .69), fit better than the single-factor model and showed acceptable internal consistency. Being at risk for social (odds ratio = 1.7) or emotional (odds ratio = 3.6) development at 18 months predicted socio-emotional difficulties at age 3 assessed by the Strengths and Difficulties Questionnaire. Socioeconomic disadvantages were correlated with higher levels of social-emotional risk. In conclusion, the study demonstrated the reliability of the Hungarian ASQ:SE-2/18 and offered evidence supporting its validity. Emotional difficulties at 18 months strongly predict later maladjustment, emphasizing the need for early screening and further tool development.</p>

<p>The prevalence of PTSD in individuals who suffer traumatic brain injury (TBI) or spinal cord injury (SCI) following road traffic accidents (RTAs) has not been comprehensively reviewed.</p>
<p>Methods: </p>
<p>A systematic search was conducted in Cochrane, PubMed, Scopus, CINAHL, ProQuest, PsycARTICLES, and PsycINFO, and the meta-analyses were performed by the comprehensive meta-analysis (CMA). JBI checklists were utilized for critical appraisal.</p>
<p>Results: </p>
<p>The pooled prevalence of PTSD in TBI and SCI survivors of RTAs was 29.4% (95% CI: 22.7% – 37.3%). The rate of PTSD in adults was 29.3% (95% CI: 23.8% – 35.5%), and it was 30.9% (95% CI: 4.4% – 81.3%) in the children subgroup. Coping styles, previous employment, acute stress disorder, and reduced awareness are some of the reported risk factors. Future longitudinal studies should further investigate the prevalence and predictors of PTSD in RTAs to identify early diagnosis and prevention strategies.</p>

<p>Having a child with autism spectrum disorder (ASD), a neurodevelopmental disorder, poses numerous challenges for parents. These parents experience high levels of parenting stress, resulting in a diminished quality of life. This study aimed to examine the relationship between self-compassion and parenting stress among parents of children with ASD. This descriptive-analytical study was carried out in 2022–2023. The study included 112 parents of children with ASD who sought assistance from autism centres in Isfahan, Iran, and were selected via convenience sampling method. Data collection involved a three-part tool: a demographic information questionnaire, the self-compassion questionnaire (SCS) and the Parenting Stress Index-Short Form (PSI-SF). The results revealed that the majority (60.7%) of parents of children with autism had low self-compassion levels. Additionally, more than 88% of participants had moderate to high levels of parenting stress. Pearson’s correlation coefficient indicated that as self-compassion increased among parents of children with autism, parenting stress levels decreased (<i>r</i> = −0.788, <i>p</i> < 0.001). The study demonstrated a significant and negative relationship between self-compassion and parenting stress among parents of children with ASD. Therefore, it is recommended that care plans for this group of parents prioritize the promotion of self-compassion as an effective coping strategy against stress.</p>

<p>Research on condom failure among African men who have sex with men (MSM) is rare; therefore, we sought to identify multilevel correlates of condom failure.</p>
<p>Methods </p>
<p>Using logistic regression, we identified the associations of various participant, partnership, and sexual interaction characteristics with condom slippage and breakage among African MSM participating in HIV Prevention Trials Network (HPTN) 075, a 1-year, multinational, prospective cohort study (2015–2017).</p>
<p>Results </p>
<p>Of 401 participants, 387 (96.5%) reported at least one sexual interaction in which condoms were used. Condom slippage was reported in 194 (10.2%) of the 1908 sexual interactions reported by participants and breakage in 223 (11.7%) interactions. Both slippage and breakage were reported less frequently by participants in ongoing intimate relationships compared with those who were not (odds ratio [OR], 0.52; 95% confidence interval [CI], 0.29–0.95; OR, 0.57; 95% CI, 0.34–0.93). Slippage and breakage occurred more frequently when sex happened under the influence of substances (OR, 1.67; 95% CI, 1.13–2.46; OR, 2.08; 95% CI, 1.48–2.93) or when sex was forced (OR, 3.18; 95% CI, 1.68–6.02; OR, 2.39; 95% CI, 1.41–4.04). Having experienced homophobia was also associated with slippage and breakage (OR, 1.32; 95% CI, 1.04–1.68; OR, 1.35; 95% CI, 1.10–1.65).</p>
<p>Conclusions </p>
<p>Ongoing condom failures suggest the importance of continued condom education of African MSM, addressing circumstances that promote or hinder optimal use, and increasing their control in sexual interactions. Experienced homophobia might as a distal factor also contribute to condom failure.</p>

<p><h2>Abstract</h2>
<p>Self-control (SC) is a robust predictor of subjective well-being (SWB), and a positive linear relationship between them is often taken as the default assumption. However, some perspectives suggest that excessive SC may diminish SWB, implying an inverted U-shaped relationship. This study examined this inverted U-shaped hypothesis among Chinese adolescents, who are raised in a culture emphasizing moderation. We analyzed five samples (total <i>N</i> = 25,936), including four cross-sectional samples, two of which were nationally representative, and a three-wave longitudinal sample collected over two years. Participants ranged from middle school to university students, with SC assessed via self- and parent-report, and SWB through multiple indicators including positive and negative affect, life satisfaction, and one-item happiness assessment. Quadratic regression analyses and Simonsohn’s two-lines test found no inverted U-shaped relationship between SC and SWB in five samples. Instead, SC consistently correlated positively with SWB across samples, regardless of measurement methods or whether data were cross-sectional or longitudinal. These findings reinforce the notion that higher SC contributes to greater SWB, with no indication of an inverted U-shape in Chinese culture. Encouraging the development of SC in adolescents might be a beneficial approach to enhancing long-term well-being, without concerns about unintended negative consequences.</p>

<p><p><imgsrc border="0" align="left" alt="image">Objective: This research investigated relationships between forward head posture, physical activity, and balance in older adults to aid in fall prevention. Methods: Thirty-two participants assessed using Godin Shephard Leisure Time Physical Activity Questionnaire, craniovertebral angle (CVA), Functional Gait Assessment (FGA), Sensory Organization Test (SOT), and limits of stability (LOS). Results: Significant relationships identified for LOS average posterior and CVA (P = .038), FGA (P = .025), and SOT (P = .036). Age had a significant inverse relationship with all variables. Age was the only predictor of FGA (P = .005). Conclusion: Posterior LOS may be appropriate to assess fall risk in older adults.</imgsrc></p>

<p><p> > <strong>Protecting an Abuser:</strong> Leaders of a Minnesota church didn’t report a parishioner to police though they knew he’d sexually abused girls for years and had been told reporting it was their duty.<br>
> <strong>Forgive and Forget:</strong> Church leaders held meetings where children were told to forgive the man who sexually abused them and forget the abuse. If they spoke of it, the sin would be theirs.<br>
> <strong>Missed Opportunities:</strong> Prosecutors had at least one opportunity to intervene but hoped educating church leaders about their duties would encourage them to cooperate with authorities.<br>

<p><p>Falls and injuries related to falls are a growing societal problem. The cause of a fall is often multifactorial in nature and injurious falls are more likely to occur in an elderly population. It has only recently been shown that hearing loss is associated with a higher risk for falling. The pathophysiology of this association is currently only partially understood and is being actively studied. There is some emerging evidence to support that managing the hearing loss through amplification may reduce the risk for falling. Proper identification and treatment of falls risk factors may reduce the likelihood of a fall and minimize this societal burden.</p>
