                        <p><h2>ABSTRACT</h2>
<h2>Objective</h2>
<p>To analyze how shifts in population composition and variations in the behavior of different sociodemographic groups have changed abortion practices in Spain between 2011 and 2021.</p>
<h2>Methods</h2>
<p>We used data from the Voluntary Terminations of Pregnancies register and Spanish Population and Housing Censuses to examine abortion rates and demographic characteristics of women aged 12–52. We used Poisson regression models and the Oaxaca–Blinder decomposition technique to estimate the contributions of shifts in population structure and variations in behaviors to changes in abortion rates.</p>
<h2>Results</h2>
<p>We found that 40% of the decline was due to changes in sociodemographic characteristics. Specifically, the relative reduction in the number of younger women in the population and the increase in women’s education levels significantly contributed to the decline in abortion rates, while the influx of immigrant women offset some of this reduction. The reduction not explained by these factors was attributed to behavioral changes, with significant reductions in the likelihood of undertaking an abortion also occurring among women under 35, immigrants from non-high-income countries, and lower-educated women.</p>
<h2>Conclusions</h2>
<p>The study highlights the importance of including changes in the sociodemographic composition of populations in analyses of shifts in abortion rates. It also shows that behavioral changes are not uniform, and that the primary driver of change in abortion rates in Spain is the reduction in abortion rates among the subgroups that had the highest initial rates, leading to a gradual flattening of the differences between women of different ages, education levels, and migratory backgrounds.</p>

<p>Despite growing research attention on the impact of coronavirus disease 2019 (COVID-19) on maternal healthcare in low- and middle-income countries, substantial knowledge gaps remain about its population-level impact. This article uses population-based data to examine the extent of COVID-19 impact on maternal healthcare, population subgroups disproportionately impacted and subnational variations in such impact in Kenya.</p>
<div class="boxTitle">Methods</div>
<p>This was a secondary analysis of the 2022 Kenya Demographic and Health Survey data. It involved assessing differences in maternal healthcare between the pre-pandemic (before March 2020), COVID-19 onset (March–December 2020) and COVID-19 era/recovery (2021–2022) periods. Three-level logistic regression models were estimated to examine the role of multilevel predictors and subnational variations in COVID-19 impact on maternal healthcare.</p>
<div class="boxTitle">Results</div>
<p>COVID-19 significantly impacted both routine and lifesaving/emergency maternal healthcare in Kenya. Compared with births pre-pandemic, births during COVID-19 onset and era/recovery had 14% and 26%, respectively, lower likelihood of adequate antenatal care (four or more visits, starting first trimester). Births during COVID-19 onset had a 33% higher likelihood of occurring at home and 31% lower likelihood of being through C-section than pre-pandemic. Certain population subgroups were disproportionately affected, with significant subnational variations in impact.</p>
<div class="boxTitle">Conclusions</div>
<p>The findings underscore a need for targeted policy/program strategies to mitigate adverse consequences of public health emergencies among disproportionately impacted population subgroups.</p>

<p>Delirium affects up to 25% of hospitalised older patients; however, there are no effective pharmacological treatments. Accumulating evidence of brain insulin resistance and altered cerebral glucose metabolism during delirium present a promising therapeutic target.</p>
<div class="boxTitle">Objective</div>
<p>To assess the safety and efficacy of intranasal insulin in the treatment of delirium.</p>
<div class="boxTitle">Design</div>
<p>Single-centre, randomised, double-blind, placebo-controlled trial.</p>
<div class="boxTitle">Setting</div>
<p>Two geriatric medicine wards in a tertiary hospital.</p>
<div class="boxTitle">Participants</div>
<p>100 patients aged over 64 years presenting to hospital and admitted under geriatric medicine with delirium.</p>
<div class="boxTitle">Interventions</div>
<p>Participants were randomised in a 1:1 ratio to receive 20 IU of long-acting insulin or placebo intranasally twice daily until delirium resolution, hospital discharge or intervention futility defined by prespecified criteria.</p>
<div class="boxTitle">Main outcome</div>
<p>The primary outcome was delirium duration, assessed daily using the Confusion Assessment Method. Secondary outcomes included acute length of stay (LOS), delirium severity, antipsychotic use, hospital complications and mortality.</p>
<div class="boxTitle">Results</div>
<p>The intention-to-treat analysis included 97 participants [intranasal insulin <span>n</span> = 48, control <span>n</span> = 49; mean (SD) age, 87.6 (7.0) years; 63% female]. Baseline characteristics were similar between groups. Median delirium duration [days (IQR)] was 4.8 [2.9, 9.2] for intranasal insulin and 6.8 [4.0, 9.8] for the control (HR 0.7, 95% CI 0.43–1.15; <span>P</span> = .16). Median acute LOS (days) was 7.9 [4.6, 14.5] for intranasal insulin and 12.9 [6.9, 16.8] for the control (HR 0.56, 0.35–0.89; <span>P</span> = .014). No significant differences were observed in other secondary outcomes. Intranasal insulin demonstrated favourable tolerability. Overall, 86% of the participants were compliant with the intervention (≥80% of doses). Prespecified subgroup analysis revealed an age-related response, with participants aged ≤88 years showing shorter delirium duration with intranasal insulin [<span>n</span> = 46; intranasal insulin: median 3.9 (IQR 2.9, 6.9) days vs control: 7.0 (4.7, 9.7); HR 0.34, 0.16–0.74; <span>P</span> = .006], whereas no difference was observed in participants aged >88 years [<span>n</span> = 51; intranasal insulin 5.4 (2.9, 11.1) vs control 4.9 (2.6, 12.9); HR 0.87, 0.39–1.94; <span>P</span> = .73].</p>
<div class="boxTitle">Conclusion and relevance</div>
<p>This is the first study of intranasal insulin for delirium treatment. The reduced LOS combined with the observed age-related effects warrants further investigation into the clinical potential of intranasal insulin in managing delirium in older patients.</p>

                        <p><h2>Abstract</h2>
<p>In today’s society, women are increasingly vulnerable to sexual violence, whether at home, in the workplace, or in public spaces. This study aims to explore how the perceived threat of sexual violence affects the psychological well-being and daily behavior of women aged 20–30 in northern India, with a focus on fear, stress, and anxiety. A qualitative approach was employed, using semi-structured interviews with a purposive sample of 20 women. Data were collected through text-based online interviews, and thematic analysis was performed to identify key patterns and insights related to the psychological impact of sexual violence. Based on Trauma Theory, Cognitive Processing Theory, and feminist perspectives, the findings revealed that the fear of sexual violence led to heightened stress and anxiety, significantly affecting participants’ mental well-being. Many women reported behavioral changes such as avoiding certain locations and altering daily routines to stay safe. The study highlighted the role of societal norms and family pressures in shaping how women deal with these threats. This study also underscores the urgent need for mental health support for women coping with the psychological toll of sexual violence.</p>
<h2>Public Significance Statement</h2>
<p>This paper shows how fear of sexual violence affects women’s daily routines, social life, and job opportunities. It urges society to educate men to respect and live safely with women, encourages bystanders to support women in need, and calls for government initiatives to create safe spaces where women can move freely without the threat of violence, abuse, or any form of attack.</p>

<p>Schizophrenia-spectrum disorders (SSD) are characterized by interruptions in one’s sense of self, cognitive dysfunction, disorganized thinking, belief inflexibility, and unusual experiences such as hallucinations and delusions. Disruptions in metacognition—skill in reflecting upon one’s own and other’s thought processes—have increasingly been viewed as a core foundation of these features of SSD. We focus this review on Metacognitive Reflection and Insight Therapy (MERIT) and Metacognitive Training (MCT), two metacognitive therapies for SSD designed to improve integration in perceptions of self and others, and to gain greater awareness of biases and the fallibility of cognitions, respectively. We explicate their theoretical underpinnings, treatment targets, and commonly used techniques and summarize their evidence base. We also provide a brief overview of two related therapies, cognitive remediation and cognitive behavioral therapy, with a focus on identifying their metacognitive components, and we compare mechanisms of action, efficacy, and evidence base across these different approaches.</p>
