<p><p>There is a direct path from the Unite the Right rally of August 12, 2017, to the Stop the Steal insurrection of January 6, 2021. As I researched this book, many more towns and cities in America began seeing armed vigilantes staging rallies and protests, protecting their God-given right to this or that, including their right to threaten unarmed people. Much like school shootings, these clashes began to seem par for the course. The dark money seeding their efforts has found its way into our national politics. I watched the governor of Virginia declare war on “divisive history,” while African American fiction and history and young adult books touching on queer sexuality were removed from library shelves.</p>

<p><p><imgsrc border="0" align="left" alt="image">Background: Social determinants of health (SDOH) interventions within clinical settings have been proposed to address health-related social needs, but few studies have explored intervention needs and challenges for Latinx families. This sequential mixed-methods study examined the effect of Developmental Understanding and Legal Collaboration for Everyone (DULCE), a 6-month SDOH intervention conducted in pediatric clinics in California and Florida. Methods: Using survey data collected among Latinx caregivers who participated in the DULCE evaluation (n = 393), we examined changes in caregivers’ depression, stress, and resilience, including differences by nativity. Qualitative interviews were conducted with DULCE participants (n = 16) across nativity to elucidate the components of the intervention that impacted caregiver outcomes. Results: DULCE led to significant increases in resilience among Latinx caregivers, especially immigrants, but no changes in depression or stress. Qualitative interviews illuminated that the following themes contributed to caregivers’ resilience through the DULCE program: trusted relationship with family specialist caseworkers, connection to critical resources, addressing postpartum depression and social isolation, building caregivers’ capacity to withstand challenges, and bolstering caregivers’ confidence to seek resources independently. Discussion: Pediatric SDOH interventions are a promising strategy to support Latinx families in early childhood. Effective interventions for Latinx families, however, require strategies that pair resource connection with relationship building, address immigrants’ and Latinx system avoidance, and incorporate additional supports or boosters for families experiencing extreme stress.</imgsrc></p>

<p><h2>ABSTRACT</h2>
<h2>Introduction</h2>
<p>Most research investigating relationships between the Big Five and emotional states has focused on how emotional attributes relate to Extraversion and Neuroticism. However, the potential for discrete emotional states to enable a richer understanding of the emotive nature of all Big Five traits and their subtraits has been neglected.</p>
<h2>Methods</h2>
<p>Participants (<i>N</i> = 203) completed the Big Five Aspects Scale, watched six emotionally stimulating video clips, and self-reported their experience of basic emotions before (Baseline) and after (Reaction) each video. Spearman correlations identified state–trait relationships, followed by regression analyses to assess the unique contribution of each trait to emotional experiences.</p>
<h2>Results</h2>
<p>Conscientiousness negatively correlated with Baseline Sadness, while Agreeableness positively correlated with Reaction Disgust, Fear, and Sadness. Extraversion predicted higher Joy, and Neuroticism was linked to greater Fear and Sadness.</p>
<h2>Conclusion</h2>
<p>Findings reinforce Extraversion and Neuroticism’s links to positive and negative emotionality, respectively, while also showing that Agreeableness predicts heightened sensitivity to negative affect. Conscientiousness, particularly Orderliness, appears protective against Baseline Sadness, and Openness to Experience, especially Intellect, is linked to lower sensitivity to Surprise. Potential mechanisms underlying these relationships are discussed.</p>

<p><h2>ABSTRACT</h2>
<p>Since the seminal article on unethical pro-organizational behavior (UPB), very few studies have explored the UPB from observers’ perspectives. Using moral foundation theory, we propose that observers’ personal UPB acceptance would be based on their relative preference for loyalty over fairness value. Further, we argue that UPB observers will respond according to their perceived normative manner; that is, their perception of peer UPB acceptance would shape their pro-organizational and quiescent silence. In Study 1, we conducted a two-phased, vignette-based field survey where data were collected from 292 working individuals from the United States. We found that observers’ relative preference for loyalty over fairness affects their personal UPB acceptance. Further, because of the pro-organizational element in UPB, observers would estimate high peer acceptance for the observed UPB. Loyalty (fairness)-preferring UPB observers are likelier to feel that they are in the majority (minority) and maintain pro-organizational (quiescent) silence. In Study 2, we conducted an activity-based two-phase survey study of 131 participants from India, where we made respondents participate in an activity in which an actual UPB act takes place. Study 2 confirmed our findings about how varying moral preferences among UPB observers would lead them to a majority or minority perception and motivate them to maintain silence. This study uniquely contributes by showcasing how UPB observers are more likely to maintain silence than observers of unethical behavior for self-interest owing to the ambivalent nature of UPB.</p>

<p>The climate crisis poses the largest threat to human health and survival and has been a public health emergency for many years. It is causing harmful consequences for physical and mental health and is amplifying existing health inequities. In this call to action, we highlight the relevance of the health psychology and behavioral medicine communities in addressing the health impacts of climate change.</p>
<p>Method: </p>
<p>We identify mitigation and adaptation climate health behaviors and social changes needed that underlie the three essential objectives to address climate change and its associated health consequences: (a) rapid decarbonization, (b) drawdown of atmospheric heat-trapping gases (sequestration), and (c) adaptation.</p>
<p>Results: </p>
<p>To advance the behavioral and systemic changes necessary to protect health, we propose a 1–2–3 Transformational Model in which the larger field of health psychology and behavioral medicine promotes (1) One Health, human and planetary health by (2) targeting climate health behaviors, and (3) social change across major professional areas, including research, interventions, and education/advocacy. We urge the adoption of the social quantum change paradigm, a systems approach to understanding the process of social change, where systemic change is viewed as local to global, and the individual has an influential role.</p>
<p>Discussion: </p>
<p>These shifts in views, priorities, and methods will bolster hope, collective efficacy, and action to support the next generation of health psychology and behavioral medicine professionals. With these changes, the health psychology and behavioral medicine communities can have a more immediate and meaningful impact on the climate crisis and its associated health consequences.</p>

<p>Infection with Chlamydia trachomatis (CT) can have distinct clinical presentations, such as trachoma, or lymphogranuloma venereum (LGV). Certain populations are at greater risk for LGV acquisition and transmission, which may require a longer duration of therapy than other urogenital CT sexually transmitted infections (STIs). Commercial assays are not available in the United States to distinguish LGV from non-LGV genovars.</p>
<p>Methods </p>
<p>Lymphogranuloma venereum real-time polymerase chain reaction was performed on rectal CT-positive samples (N = 93) obtained from men (N = 80) who ordered from a mail-in self-collection STI service between April 2021 and February 2024. pmpH gene sequencing was performed on all samples to confirm LGV versus non-LGV, and multilocus sequence typing was performed on LGV-positive samples (n = 7) for additional confirmation.</p>
<p>Results </p>
<p>Lymphogranuloma venereum was detected in 7.5% (7 of 93) of samples by real-time polymerase chain reaction, with pmpH sequencing and multilocus sequence typing confirming 100% (7 of 7) of these results. Overall, pmpH sequencing data were obtained for 92% (86 of 93) of samples with the following genovar distribution based on BLAST analysis: 54% (47 of 86) J, 28% (24 of 86) F, 9% (8 of 86) E, and 8% (7 of 86) L. No individual had more than 1 LGV-positive sample. No statistically significant associations with demographic factors were identified.</p>
<p>Conclusions </p>
<p>Lymphogranuloma venereum was detected in CT-positive rectal swabs from users of an online, mail-in, self-collect STI testing platform in Maryland. These data suggest that increased LGV reflexive testing may be warranted to better understand the cotemporary epidemiology of LGV. These data also illustrate that mail-in programs for routine STI testing may be leveraged for public health surveillance purposes.</p>

<p>Pelvic inflammatory disease (PID) disproportionately impacts adolescents and young adult women. The Technology-Enhanced Community Health Nursing (TECH-N) trial demonstrated the potential benefit of a novel community health intervention for adolescents with PID. We assess the cost-effectiveness of TECH-N compared with standard care.</p>
<p>Methods </p>
<p>We constructed a cohort Markov model to assess the cost-effectiveness of TECH-N for adolescents with PID in an urban setting. The model used nine health states: PID; two states for STIs; four states for PID sequelae, recovery, and deceased states. The cohort consisted of 18-year-old female adolescents with mild to moderate PID. Transition probabilities were derived from the TECH-N clinical trial and published literature. Health state utilities were derived from published literature. Intervention costs were estimated using TECH-N data, and health state costs were derived from published literature and public databases. The model took a health system perspective over a 10-year time horizon. Sensitivity analyses were used to assess uncertainty. Cost-effectiveness was estimated using the discounted incremental cost-effectiveness ratio with effects measured in quality-adjusted life years (QALYs).</p>
<p>Results </p>
<p>Over ten years, the TECH-N cohort generated 8.16 QALYs per individual at a cost of $20,419 compared with standard care with 8.14 QALYs costing $20,492. The resulting incremental cost-effectiveness ratio was negative since the intervention produced more QALYs at a lower cost; suggesting TECH-N is cost-saving.</p>
<p>Conclusions </p>
<p>Our model suggests the TECH-N intervention yields better outcomes at lower overall costs. In addition, the nature of TECH-N lends itself to being coupled with other home-health/community strategies, which could further improve its value proposition.</p>

<p><p><imgsrc border="0" align="left" alt="image">Suicidal behavior is prevalent with first psychotic episodes, but reports of associated factors involve inconsistent findings and emphasis on schizophrenia. We evaluated suicide attempt rates and associated risk factors in 395 first-episode patients with various DSM-5-TR diagnoses with psychotic features, comparing 83 suicidal patients to others. Suicide attempt risk averaged 21.0%, with the final diagnosis ranked as follows: major depression, bipolar I depression, bipolar I mixed features, schizoaffective—depressed, unspecified psychosis, schizophrenia, schizoaffective—bipolar, bipolar I mania, delusional disorder, and none with schizophreniform or brief psychosis. Associated by multivariable modeling were initial recklessness ≥ initial impulsive violence ≥ initial anergy ≥ prior suicide attempt ≥ initial despair ≥ initially homicidal. Risk factors were similar in 36.1% of suicidal cases before and at first episodes. Suicide attempts were prevalent with hospitalized first psychotic episodes: more with major affective disorders or schizoaffective—depression than with schizophrenia or other diagnoses. Notable risk factors included initial reckless, impulsive, angry, and violent behavior, depressive features, anergy, and prolonged prodromes.</imgsrc></p>

<p><p><imgsrc border="0" align="left" alt="image">The COVID-19 pandemic is believed to have strongly impacted adolescents’ mental health through limiting their day-to-day social lives. However, little is known about the role of (social) risk/protective factors and adolescent mental health. Subclinical psychopathology, and risk and protective factors of n = 173 adolescents (T1 mean age = 16.0; 89% girls) were assessed prepandemic and early-pandemic (T1: 2018/2019; T2: May 2020). Daily-life social interactions were assessed in 6-day experience sampling periods. Correcting for age, multilevel analyses revealed declines in general psychopathology and anxiety symptoms; fewer face-to-face social interactions, more online social interactions; and higher-quality face-to-face interactions during the pandemic than before. Negative associations between psychopathology and the quality of face-to-face peer and family interactions were stronger during the pandemic than prepandemic. The results reflect that, early in the pandemic, the mental health impact on adolescents may have been limited. High-quality face-to-face interactions with family and peers seemed particularly powerful in keeping adolescents resilient.</imgsrc></p>
