<p>To build a coherent knowledge base about what psychological intervention strategies work, develop interventions that have positive societal impact, and maintain and increase this impact over time, it is necessary to replace the classical treatment package research paradigm. The multiphase optimization strategy (MOST) is an alternative paradigm that integrates ideas from behavioral science, engineering, implementation science, economics, and decision science. MOST enables optimization of interventions to strategically balance effectiveness, affordability, scalability, and efficiency. In this review we provide an overview of MOST, discuss several experimental designs that can be used in intervention optimization, consider how the investigator can use experimental results to select components for inclusion in the optimized intervention, discuss the application of MOST in implementation science, and list future issues in this rapidly evolving field. We highlight the feasibility of adopting this new research paradigm as well as its potential to hasten the progress of psychological intervention science.</p>

<p><p>International Journal of Social Psychiatry, Ahead of Print. <br>Background:After a period of institution-based mental health care, in which the asylum system was the way in which the mental patients were treated, deinstitutionalization brought a set of significant changes and transformations in the conceptualization of mental illness and, by extension, the traditional therapeutic settings in which those in most need were assisted. However, this shift in the psychiatric domain was not only accompanied by valued achievements, but also by difficulties and challenges, as has been evidenced today.Aim/objective:The aim of this paper is thus to examine the pros and cons of the closure of asylums, and the subsequent implementation of deinstitutionalization over the 60 years or so of such important transformations in the field of psychiatry.Methods:In considering this question, I examine in detail recent works of literature based on scholarly knowledge. In addition, I identify various issues involved, as well as ways of confronting these so as to attempt to overcome the difficulties they present.Results and conclusions:As I show here, the changes in the treatment and care of the mentally ill after asylum and deinstitutionalization brought a new air of hope to patients and their families, but also had undesirable effects. The paper also considers how mental health professionals today try to solve these effects on behalf of patients and society as a whole.</p>

<p>Clinical psychological science is long overdue to incorporate antiracism and social justice as core principles. To evaluate the current state of clinical science training programs’ public-facing statements on antiracism and diversity, equity, inclusion, and justice (ADEIJ), we formed a workgroup under the guidance of the Academy of Psychological Clinical Science (APCS) Task Force to Center Racial and Social Justice in Clinical Science. Our workgroup evaluated the public-facing web materials of 78 APCS doctoral and internship programs. Following existing frameworks that have conceptualized the development of multicultural organizations, we evaluated the degree to which materials mentioned inclusivity, acknowledged inequities, and reported planned and enacted actions to promote ADEIJ. We examined rationales for engaging in this work, differentiating between moral and instrumental rationales. Results showed not all programs had ADEIJ statements, but a coding scheme can reliably assess existing statements. While all programs mentioned inclusivity and most acknowledged inequities, some had subtle or implicit exclusionary language, and a minority described concrete actions. Current efforts within our field may represent more symbolic than identity or structural change. Our findings inform recommendations for improving the efforts of training programs. (PsycInfo Database Record (c) 2024 APA, all rights reserved)</p>

<p><p>Qualitative Social Work, Ahead of Print. <br>Welfare encounters are increasingly being conducted using communication devices which comprises mediated encounters. This article contributes to studies on the integration of mediated encounters into social work practice. The study adopts a symbolic interactionist perspective and is based on interviews with 24 social workers and 17 vulnerable clients. It examines the role of phone mediation in social workers’ and clients’ role performances in welfare encounters that lack non-verbal communication. This study argues that phone mediation illustrates the unequal stakes in welfare encounters: A professional work role and organisational goal attainment are at stake for social workers, whereas the livelihoods of vulnerable clients depend on welfare encounters and the social and economic support provided by them. Such stakes are often taken for granted in routine face-to-face welfare encounters; thus, phone mediation alerts the participants to the consequentiality of welfare encounters. Moreover, this article finds that phone mediation may provide confidential distance, which can be used to support hard-to-reach clients and social work practices. However, mediated encounters also run the risk of being insignificant for client trajectories and restrict the roles of social workers and clients. Overall, the study highlights everyday technologies, such as phones, that are important for the social worker-client relationship, welfare delivery, and clients’ welfare trajectories. It argues for further inclusion of everyday technologies in future research agendas that examine the digitalisation of social work and the importance of non-verbal communication.</p>

<div class="boxTitle">Abstract</div>
<p>This article reports findings from a qualitative study of the impacts of, and practitioner responses to, neoliberal social work reforms in Switzerland and England. The article highlights commonalities, and contrasts, in how neoliberalism has restructured social work across the two jurisdictions. Social workers responded to neoliberalisation in a variety of ways, often through adaptation but sometimes by challenging reforms. This resistance primarily involved individualised forms of agency promoting social justice in practice, such as carving out discretionary spaces within casework. However, in Switzerland, collective agency also emerged in the form of anti-cuts campaigning alongside service users. The literature tends to counterpose these micro and macro levels of agency, theorising discretion and resistance to the contradictions and value tensions generated by neoliberal reform as largely a product of practitioners’ individual ethical dispositions. However, we argue that enduring legacies of social justice values and relationship-based practices within social work institutions are also relevant in shaping how social workers navigate reforms. These enduring features offer institutional and normative resources of solidarity which can strengthen the possibilities for practitioner agency, at both individual <span>and</span> collective levels, to resist neoliberal reconfigurations and promote more liberatory and transformative forms of practice. We call this orientation ‘ethico-political professionalism’.</p>

<p>The purpose of this study was to examine the association between racial and economic segregation and diabetes mortality among US counties from 2016 to 2020.</p>
<p>
<st>Methods</st>
</p>
<p>We conducted a cross-sectional ecological study that combined county-level diabetes mortality data from the National Vital Statistics System and sociodemographic information drawn from the 2016–2020 American Community Survey (n=2380 counties in the USA). Racialized economic segregation was measured using the Index Concentration at the Extremes (ICE) for income (ICEincome), race (ICErace) and combined income and race (ICEcombined). ICE measures were categorised into quintiles, Q1 representing the highest concentration and Q5 the lowest concentration of low-income, non-Hispanic (NH) black and low-income NH black households, respectively. Diabetes was ascertained as the underlying cause of death. County-level covariates included the percentage of people aged ≥65 years, metropolitan designation and population size. Multilevel Poisson regression was used to estimate the adjusted mean mortality rate and adjusted risk ratios (aRR) comparing Q1 and Q5.</p>
<p>
<st>Results</st>
</p>
<p>Adjusted mean diabetes mortality rate was consistently greater in counties with higher concentrations of low-income (ICEincome) and low-income NH black households (ICEcombined). Compared with counties with the lowest concentration (Q1), counties with the highest concentration (Q5) of low-income (aRR 1.96; 95% CI 1.81 to 2.11 for ICEincome), NH black (aRR 1.32; 95% CI 1.18 to 1.47 for ICErace) and low-income NH black households (aRR 1.70; 95% CI 1.56 to 1.84 for ICEcombined) had greater diabetes mortality.</p>
<p>
<st>Conclusion</st>
</p>
<p>Racial and economic segregation is associated with diabetes mortality across US counties.</p>

<p>While it is an ethical imperative to provide multiculturally competent training, training programs vary widely in implementation. Further, the empirical evidence assessing the effectiveness of multiculturally competent training is limited. This study adds to the literature by describing a multicultural curriculum grounded in multicultural competence and providing empirical data assessing the curriculum. Our multicultural curriculum is implemented in a psychology-based counseling program and was assessed in 2010–2019, whereby 95 students completed self-report measures of awareness and knowledge as well as an evaluative assessment of multicultural competence. The measures were completed before engaging in the curriculum and repeated once they completed the curriculum. Findings indicate that the curriculum was associated with growth in self-reported awareness and knowledge but not growth in multicultural competence knowledge evaluated by a standardized test. Our clinical skills course with an accompanying self-awareness curriculum was associated with greater changes in self-reported awareness than any other element of our curriculum. Additional analyses suggest that baseline self-reported knowledge could be an impediment to gains in self-reported awareness. The results underscore the importance of combined awareness and skill development for continuous learning. (PsycInfo Database Record (c) 2024 APA, all rights reserved)</p>
