<p><p>The North Carolina General Assembly passed a law in 2012 allowing the use of continuous alcohol monitoring systems as a condition for pretrial release or probation, to mitigate punishments for impaired driving offenses and to ensure compliance with child custody and visitation orders. These systems, commonly referred to as CAM bracelets, fit around the ankle similar to GPS-tracking ankle monitors that are often used as a condition for bail or probation. <p><p>This hermeneutic phenomenological study illustrates the value of key aspects of interpretative hermeneutics for illuminating the time shared between patients and students in forensic unit for men carrying a personality disorder diagnosis through interviews. The participants were being-with in their own time and space, sharing ordinary activities and common interests, and having a laugh, ultimately making enduring connections. This was despite experiences of thrownness into the world, ward landscapes, or the bearing of diagnostic labels. Students experienced a balance between therapeuticness (leaping-ahead) and professionalism (leaping-in). They were immune to the relational vacuum of the ward office and were viewed as “sponges”, reflecting their availability to patients, as opposed to other staff occupying more distant, identity defining set roles. On transition to fully fledged staff roles, students can become holders of keys, but can weather the ride by treasuring the everyday mundane, being-with and connecting with patients. This temporality of student-patient’s relationships encompasses mutual recognition of shared humanness. This hermeneutic phenomenological study uniquely explores the time shared between patients and students in forensic settings. Learnings from their experiences as illuminated using philosophical notions aid our understanding of this time to ensure focus on human connection.</p>

<p><h2>Abstract</h2>
<h2>Objective</h2>
<p>We describe the experiences and preferences of women who switched from clinic-administered intramuscular depot medroxyprogesterone acetate (DMPA-IM) to self-administered subcutaneous DMPA (DMPA-SC) in the context of the COVID-19 pandemic.</p>
<h2>Methods</h2>
<p>We conducted interviews with women in California and Washington about their experiences with self-administered DMPA-SC. We interviewed women after their first or second self-administered DMPA-SC injection and conducted follow-up interviews after their third or fourth injection. We performed both thematic and descriptive content analyses.</p>
<h2>Results</h2>
<p>We completed 29 interviews with 15 women. Most participants (<i>n</i> = 10) were between the ages of 20 and 39 and the majority (<i>n</i> = 12) used DMPA primarily for contraception. Most (<i>n</i> = 13) described self-administered DMPA-SC as “very easy” or “somewhat easy” to use and reported greater convenience, decreased pain, fewer logistical and financial challenges, increased privacy, and improved comfort with injection compared to DMPA-IM. Participants identified difficulties obtaining DMPA-SC from pharmacies and safe needle disposal as barriers. Most (<i>n</i> = 13) would recommend DMPA-SC to a friend and desired to continue self-administration beyond the COVID-19 pandemic. Participants recommended counseling all patients about this option alongside other contraceptive methods, and offering clinician supervision, if desired.</p>
<h2>Conclusion</h2>
<p>Women who switched from in-clinic DMPA-IM to self-administered DMPA-SC during the COVID-19 pandemic preferred the latter and intended to continue self-administration. Self-administration of DMPA-SC should be routinely offered and easily accessible to patients.</p>

<p><p>By and large, public libraries have stayed public. One reason is that the private sector is hard-pressed to find a library substitute. Neoliberals gut public school budgets with the promise of so-called independent and charter schools, but in the case of libraries, personal books in private homes can’t approach the scale of what a library can offer, and no private infrastructure exists that could absorb the public work of the library. Beyond books, libraries offer storytime and play groups, film screenings and knitting classes, English language lessons and literacy training — all of it open to anyone in the community, all of it free at the point of use and participation. The programs are always changing and tightly linked to the changing needs and interests of the public. There is simply no private-sector mechanism for the production of public services at the scale of the library.</p>

<p><p>Objective: </p>
<p>Little has been written about the effects of the coronavirus disease 2019 (COVID-19) pandemic on assisted reproduction services in low and middle-income countries. The purpose of this study was to investigate the immediate and long-term effects of assisted reproductive technology services in South Africa after 27 months of various lockdowns and restrictions associated with the pandemic.</p>
<p>Methods: </p>
<p>This article draws on data collected in 2020–2023 as part of a qualitative study of assisted reproduction within sub-Saharan Africa. Twenty-nine infertility specialists and other clinical staff including embryologists, clinic coordinators, ova donor coordinators, and nurses were interviewed and asked about the effects of the pandemic on their clinics and patients. A semistructured in-depth interview technique was used to collect the data. The analysis was conducted using the thematic analysis method.</p>
<p>Result: </p>
<p>Findings revealed that the immediate effects of the pandemic included the cancellations and deferral of assisted reproductive technology cycles and decreased numbers of cross-border patients. Challenges experienced by clinics in making decisions included whether to treat COVID-positive patients, increased costs of testing, and economic losses. Longer-term effects included the reconsideration of life priorities by both specialists and patients, and improved remote consultations and networking opportunities facilitated by the widespread dissemination of online platforms. There was also increased acceptability of the shipping and use of frozen gametes, leading to a perceived growth of the South African frozen oocyte trade. There was also decreased demand for add-ons, associated with the economic downturn post-pandemic in the region.</p>
<p>Conclusion: </p>
<p>COVID-19 carried costs (cancellation of cycles, a decrease in international patients, decrease in some procedures with the post-COVID economic downturn) but also unanticipated benefits for the practices and provision of services within infertility clinics in South Africa (increased gamete and embryo shipping; virtual patient consultations; improved virtual networking; greater use of local in vitro fertilization services). Further comparisons postpandemic in other low and middle-income countries are needed to understand the long-term implications.</p>
