<p>Decision making is an intrinsic and complex aspect of social work practice, requiring consideration of diverse but connected aspects. Decisions are often required as to whether a situation requires protective state intervention or whether it reaches the criteria for public or charitable services. Such instances of deciding whether or not a situation is ‘on one side of the line or the other’ are referred to in this article as ‘threshold judgements’. This article draws on experiences and material from a range of social work contexts to explore generalisable <span>theory-informed</span> understandings of ‘threshold judgements’ and ‘threshold decisions’ to develop knowledge and skills on this topic. The article outlines <span>signal detection theory</span> and <span>evidence accumulation</span> (‘tipping point’) theory and discusses these as ways to understand the key concepts underpinning threshold decisions in social work. We then argue that although these threshold concepts are a necessary part of decision making in social work, as in many other aspects of life, they are not sufficient. Operationalising such decisions requires some form of sense-making. <span>Naturalistic decision making</span> and <span>heuristic models of judgement</span> are discussed as frameworks for practice which seem to be useful in this context.</p>

<p>The War on Diabetes campaign was launched in 2016, encouraging Singapore residents to engage in regular exercise, adopt healthy dietary habits and screen for early detection of diabetes. This study aims to examine campaign awareness and its associations with sedentary behaviour, dietary habits and identifying diabetes. Data were obtained from the nationwide Knowledge, Attitudes and Practices study on diabetes in Singapore. A total of 2895 participants responded to a single question assessing campaign awareness. The Dietary Approaches to Stop Hypertension (DASH) diet screener assessed dietary habits, and the Global Physical Activity Questionnaire (GPAQ) measured sedentary behaviour. Recognition of diabetes was established using a vignette depicting a person with diabetes mellitus. Logistic and linear regression models were used to measure the associations. Most participants were 18- to 34-years old (29.9%) and females (51.6%). About 57.4% identified the campaign. Campaign awareness exhibited positive associations with identifying diabetes based on the vignette [odds ratio (OR): 1.5; 95% confidence interval (CI): 1.1–2.2; <span>P</span> = 0.022], lower odds of sedentary behaviour ≥7 h/day (OR: 0.7; CI: 0.5–0.9; <span>P</span> = 0.018) and higher DASH scores (<span>β</span> = 1.3; <span>P</span> < 0.001). The study recognized early significant associations between the behavioural outcomes and the campaign, emphasizing the need for ongoing campaign sustainability and evaluation of its long-term impact on population health.</p>

<p>This article investigates the meaning that refugee solidarity activists supporting people on the move across the Western Balkans migratory route and at the Italian–French border attribute to the notion of ‘care’, which they use to define their solidarity practices, particularly in the aftermath of the global pandemic. By means of a content analysis of in-depth interviews with representatives of grassroots solidarity groups, the article demonstrates that ‘care’ is conceived of as having a political character, as it responds to both the crisis of health care and the restrictions on freedom of movement; a non-hierarchical connotation, which informs in- and out-group relationships; and a transformative orientation, as acts of care prefigure a society in which freedom of movement and health rights are granted to all, in contrast to the existing model of migration governance.</p>

<div class="boxTitle">Objective</div>
<p>Due to systemic inequities, Black adolescents with type 1 diabetes are more likely to have suboptimal glycemic control and high rates of diabetes distress, but tailored interventions for this population are lacking. In primary outcomes of a randomized clinical trial, a family-based eHealth intervention improved glycemic control in Black adolescents with type 1 diabetes and elevated depressive symptoms. The present study is a secondary analysis of these clinical trial data examining the moderating effect of diabetes distress on the efficacy of the intervention.</p>
<div class="boxTitle">Methods</div>
<p>Using secondary data from a multicenter randomized clinical trial (Clinicaltrials.gov [NCT03168867]), caregiver–adolescent dyads were randomly assigned to either up to three sessions of an eHealth parenting intervention (<span>n</span> = 75) or a standard medical care control group (<span>n</span> = 74). Black adolescents (10 years, 0 months to 14 years, 11 months old) with type 1 diabetes and a caregiver willing to participate were eligible. Adolescents reported their diabetes distress at baseline, and hemoglobin A1c (HbA1c) data were collected at baseline, 6-, 13-, and 18-month follow-up.</p>
<div class="boxTitle">Results</div>
<p>No between-group contrasts emerged in a linear mixed-effects regression (<span>p</span>’s > .09). Within-group contrasts emerged such that adolescents assigned to the intervention who reported high diabetes distress had lower HbA1c at the 18-month follow-up relative to baseline (<span>p</span> = .004); the 18-month decrease in HbA1c was −1.03%.</p>
<div class="boxTitle">Conclusions</div>
<p>Black adolescents with type 1 diabetes and high levels of diabetes distress showed significant decreases in HbA1c following a family-based eHealth intervention, suggesting diabetes distress may be a key moderator of intervention efficacy within this population.</p>

<p>Sub-Saharan Africa has fewer medical workers per capita than any region of the world, and that shortage has been highlighted consistently as a critical constraint to improving health outcomes in the region. This paper draws on newly available, systematic, comparable data from ten countries in the region to explore the dimensions of this shortage. We find wide variation in human resources performance metrics, both within and across countries. Many facilities are barely staffed, and effective staffing levels fall further when adjusted for health worker absences. However, caseloads—while also varying widely within and across countries—are also low in many settings, suggesting that even within countries, deployment rather than shortages, together with barriers to demand, may be the principal challenges. Beyond raw numbers, we observe significant proportions of health workers with very low levels of clinical knowledge on standard maternal and child health conditions. This work demonstrates that countries may need to invest broadly in health workforce deployment, improvements in capacity and performance of the health workforce, and on addressing demand constraints, rather than focusing narrowly on increases in staffing numbers.</p>

<p><p>Since qualifying in the 1960s and then moving into academia, Professor June Thoburn has carried out extensive research, in the UK and overseas, about child protection, family support, adoption and services for children placed away from home. Crucially, she contributed to reforms that allowed parents to attend child protection conferences. She was also part of the advisory groups on both the Children Act 1989 and the Adoption and Children Act 2002, conducting government-commissioned research that informed the latter.</p>
