                        <p><h2>Abstract</h2>
<p>This paper serves as a renewed call for public management scholars and public leaders in liberal democracies to be the champions of accountability standards that are explicitly and implicitly inherent to liberal democratic forms of governance. This call is particularly salient amid increasing populism, polarization, and democratic backsliding. Drawing from the historical and contemporary political and legal philosophies of small-<i>l</i> liberalism and democracy advanced throughout the ages, we define a set of seven liberal democratic accountability standards focusing on matters of authority, rights, tolerance, truth claims, and professional deference. We then consider how these standards relate to some of public administration and management’s ongoing considerations of the politics-administration dichotomy, citizen engagement, and network governance, and make the case for more explicit focus on liberal democratic accountability standards in public management scholarship.</p>

<p>Several observational studies have described an inverse association between cancer diagnosis and subsequent dementia risk. Multiple biologic mechanisms and potential biases have been proposed in attempts to explain this association. One proposed explanation is the opposite expression of Pin1 in cancer and dementia, and we use this explanation and potential drug target to illustrate the required assumptions and potential sources of bias for inferring an effect of Pin1 on dementia risk from analyses measuring cancer diagnosis as a proxy for Pin1 expression.</p>
<p>Methods: </p>
<p>We used data from the Rotterdam Study, a population-based cohort. We estimate the association between cancer diagnosis (as a proxy for Pin1) and subsequent dementia diagnosis using two different proxy methods and with confounding and censoring for death addressed with inverse probability weights. We estimate and compare the complements of a weighted Kaplan–Meier survival estimator at 20 years of follow-up.</p>
<p>Results: </p>
<p>Out of 3634 participants, 899 (25%) were diagnosed with cancer, of whom 53 (6%) had dementia, and 567 (63%) died. Among those without cancer, 15% (411) were diagnosed with dementia, and 667 (24%) died over follow-up. Depending on the confounding and selection bias control, and the way in which cancer was used as a time-varying proxy exposure, the risk ratio for dementia diagnosis ranged from 0.71 (95% confidence interval [CI] = 0.49, 0.95) to 1.1 (95% CI = 0.79, 1.3).</p>
<p>Conclusion: </p>
<p>Being explicit about the underlying mechanism of interest is key to maximizing what we can learn from this cancer-dementia association given available or readily collected data, and to defining, detecting, and preventing potential biases.</p>

                        <p><p>What the heck is a representative payee?</p>
<p>Social Security approves a family member or, less frequently, an organization to receive benefits on behalf of an individual who is old and infirm or has a disability. The representative is required to ensure that the benefits are being used to meet the needs of the person in their care.</p>
<p>To gauge how well the program is working, Travis Labrum at the University of Pittsburgh conducted lengthy telephone interviews with five people with a mental illness who receive their benefits through a family member and with seven family members acting in that official role.</p>
<p>He said that two out of three people who have family representatives managing their finances said they are fairly satisfied with the arrangements. But they also said the program could be improved, including finding a way to allow them to get more involved in the financial decisions affecting them to minimize disagreements with their representatives about how to spend the modest benefits.</p>
<p>Representatives are often assigned involuntarily to people with mental illness, heightening the conflicts that can arise. Labrum’s specific interest was the program’s effectiveness for people with a mental illness because so many – one in four who have a representative payee – are dealing with schizophrenia, depression, or other psychological disorders.</p>
<p>The beneficiaries he interviewed appreciated the advantage of having a family payee, rather than an alternative arrangement with a financial, legal or other organization. “She gives me emotional support,” one said. “I trusted her,” said another, “to have my best interests at heart.” And the payees said they felt good being able to help a family member “accomplish some of the goals.”</p>
<p>Several beneficiaries also said having a family member take care of their finances, rather than an organization, is less formal. “I see her every day, talk to her every day,” one said. Someone who switched from an organization to a family member said the organization “said ‘no’ a lot more than my mom does.” Family members also can help with non-financial tasks – childcare, housekeeping, visits to a therapist – that an organization would not.</p>
<p>But family relationships can get complicated, and some people complained their representatives were not permitting them to get involved in prioritizing their limited budgets. Social Security’s disability benefit for all recipients averages $1,537 per month, and the retirement benefit averages $1,905. The maximum monthly benefit under the agency’s Supplemental Security Income program is $943.</p>
<p>One beneficiary said she would “like to be more in charge” of buying things like a haircut or some new clothes. Another said flatly that his mother, acting as his representative, has too much control: “It’s hard for me to get toiletries,” he said, and “she just keeps controlling this and controlling that.”</p>
<p>Disagreements also arise when the representative gets involved in non-financial aspects of the beneficiary’s life. One complained that the representative “doesn’t like that I buy cigarettes,” and another was overly involved in helping to decide where the beneficiary would live, which “was a little irritating.”</p>
<p>These relationships can also be difficult for the representative payees. Like caregiving, the job can feel, as one said, like a “burden.” One beneficiary was even concerned about the stress his financial difficulties were placing on his family representative. “It was better when she didn’t know how much I made,” he said. “I could pretend that everything was better than it was.” One woman who was her mother’s representative told Labrum she quit doing that job because their “continuous conflicts” were damaging their relationship.</p>
<p>The representative payees also feel the weight of responsibility for making sure the person in their care is stable and healthy. “I try to make sure he doesn’t disobey what the doctors say because they’re the ones who initially got him approved” for benefits, one representative said. “Somebody’s always watching,” and “I definitely want to make sure he has groceries, he has his rent paid, and that he doesn’t partake in drinking way too much alcohol.”</p>
<p>Some beneficiaries felt that their representatives use their mental illness as justification for taking control. Financial abuse does occur in rare cases. But Labrum said two suggestions that came out of his interviews could improve most of the relationships: more training for would-be representatives and an informal process for beneficiaries to air their concerns, short of filing a formal complaint.</p>
<p>The way beneficiaries see it, the researcher said, family members are “less skilled or knowledgeable than organizational representatives about the rules and limits of representative payeeship.”</p>

<p>Gonorrhea is the second most common bacterial sexually transmitted infection in the United States, with rising rates. Emerging antimicrobial resistance threatens public health, and vaccines are in development. This study documents patient-reported gonorrhea symptoms and health-related quality-of-life (HRQoL) impact in women who have sex with men (WSM), men who have sex with women (MSW), and men who have sex with men (MSM).</p>
<p>Methods </p>
<p>Semistructured qualitative interviews were conducted (April 2021–March 2022) among US adults with recent (≤6 months) confirmed gonorrhea. Concept saturation was achieved, confirming adequate participant numbers to meet objectives. Elicited symptom and HRQoL impacts were used to develop a conceptual disease model. Common patient-reported outcome measure (PROM) items were compared with elicited concepts.</p>
<p>Results </p>
<p>Thirty-two participants (15 WSM, 8 MSW, and 9 MSM) were included. Eight were asymptomatic, 6 had repeat infections, and 5 women and 4 men had complications (i.e., infertility or pelvic inflammatory disease, and urethritis, respectively).</p>
<p>The most frequently reported symptoms were vaginal discharge (n = 11 of 12), dysuria (n = 10 of 12), and abdominal/vaginal pain (n = 9 of 12) for symptomatic WSM; dysuria (n = 5 of 6) and penile discharge (n = 4 of 6) for symptomatic MSW; and throat, testicular, or rectal pain (n = 4 of 6) for symptomatic MSM. All (symptomatic and asymptomatic) participants reported HRQoL impacts including negative emotional experiences (n = 31 of 32), and interference with sexual activity (n = 30 of 32), relationships (n = 24 of 32), and social life (n = 17 of 32). Concepts were poorly represented in existing PROMs.</p>
<p>Conclusions </p>
<p>Participants reported a diverse range of symptoms and HRQoL impacts, to help inform the value of new treatment and prevention options. More research is needed to quantify patient burden and develop PROMs.</p>

<p>Standard-of-care nucleic acid amplification tests (routine NAATs) for Neisseria gonorrhoeae (GC) and Chlamydia trachomatis (CT) can take several days to result and therefore delay treatment. Rapid point-of-care GC/CT NAAT (rapid NAAT) could reduce the time to treatment and therefore onward transmission. This study evaluated the incremental cost per infectious day averted and overall cost of implementation associated with rapid compared with routine NAAT.</p>
<p>Methods </p>
<p>Prospective sexually transmitted infection (STI) treatment data from men who have sex with men and transgender women in San Diego who received rapid NAAT between November 2018 and February 2021 were evaluated. Historical time from testing to treatment for routine NAAT was abstracted from the literature. Costs per test for rapid and routine NAAT were calculated using a micro-costing approach. The incremental cost per infectious day averted comparing rapid to routine NAAT and the costs of rapid GC/CT NAAT implementation in San Diego Public Health STI clinics were calculated.</p>
<p>Results </p>
<p>Overall, 2333 individuals underwent rapid NAAT with a median time from sample collection to treatment of 2 days compared with 7 to 14 days for routine NAAT equating to a reduction of 5 to 12 days. The cost of rapid and routine GC/CT NAAT was $57.86 and $18.38 per test, respectively, with a cost-effectiveness of between $2.43 and $5.82 per infectious day averted. The incremental cost of rapid NAAT improved when at least 2000 tests were performed annually.</p>
<p>Conclusions </p>
<p>Although rapid GC/CT NAAT is more expensive than routine testing, the reduction of infectious days between testing and treatment may reduce transmission and provide improved STI treatment services to patients.</p>

                        <p><h3 class="a-plus-plus">Abstract</h3>
<p class="a-plus-plus">Student persistence and retention in community colleges concern scholars, policymakers, and academics in higher education at large. Previous literature suggests that a strong sense of belonging in higher education and at the institution the student is attending may be one significant factor that impacts persistence and retention. However, belonging on campus in community college settings is complex given the nature of open enrollment, lack of on-campus housing, employment status of most students, and the limited amount of time students stay at the institution. Furthermore, previous correlational studies linking a sense of belonging and academic outcomes leave scholars wondering about causation without the science to support the inquiries. Standard statistical methods of association do not allow scholars to draw causal conclusions. Direction dependence analysis (DDA) provides a new way of statistically examining the causal directionality of relationships between constructs. The current study introduces principles of direction dependence modeling and examines the causal direction of the relationship between belonging and academic self-efficacy in a large community college setting. DDA results suggest that there is evidence to suggest a causal relationship between belonging and academic self-efficacy, a strong indication that belonging causes academic motivation rather than the other way around. Implications for higher education research and potential limitations of DDA are discussed.</p>

<p>Gonorrhea’s rapid development of antimicrobial resistance underscores the importance of new prevention modalities. Recent evidence suggests that a serogroup B meningococcal vaccine may be partially effective against gonococcal infection. However, the viability of vaccination and the role it should play in gonorrhea prevention are an open question.</p>
<p>Methods </p>
<p>We modeled the transmission of gonorrhea over a 10-year period in a heterosexual population to find optimal patterns of year-over-year investment of a fixed budget in vaccination and screening programs. Each year, resources could be allocated to vaccinating people or enrolling them in a quarterly screening program. Stratifying by mode (vaccination vs. screening), sex (male vs. female), and enrollment venue (background screening vs. symptomatic visit), we consider 8 different ways of controlling gonorrhea. We then found the year-over-year pattern of investment among those 8 controls that most reduced the incidence of gonorrhea under different assumptions. A compartmental transmission model was parameterized from existing literature in the US context.</p>
<p>Results </p>
<p>Vaccinating men with recent symptomatic infection, which selected for higher sexual activity, was optimal for population-level gonorrhea control. Given a prevention budget of $3 per capita, 9.5% of infections could be averted ($299 per infection averted), decreasing gonorrhea sequelae and associated antimicrobial use by similar percentages. These results were consistent across sensitivity analyses that increased the budget, prioritized incidence or prevalence reductions in women, or lowered screening costs. Under a scenario where only screening was implemented, just 5.5% of infections were averted.</p>
<p>Conclusions </p>
<p>A currently available vaccine, although only modestly effective, may be superior to frequent testing for population-level gonorrhea control.</p>

<p class="a-plus-plus">Several studies on research misconduct have already explored and discussed its potential occurrence in universities across different countries. However, little is known about this issue in Spain, a paradigmatic context due to its consolidated scientific evaluation system, which relies heavily on metrics. The present article attempts to fill this gap in the literature through an empirical study undertaken in a specific university: Universitat Jaume I (Castelló). The study was based on a survey with closed and open questions; almost half the total population of the university’s researchers participated (505 out of 1030, i.e. 49.03%), yielding a representative sample of different academic career stages and areas of knowledge. Results show that 71.68% (n = 362) of the respondents consider at least one form of misconduct to be proliferating in their area of knowledge at the national level. This figure falls to 48.95% (n = 247) in reference to misconduct in their own institution. The most frequently reported types of misconduct linked to life with colleagues are especially the use of personal influence (in evaluation or review processes); lax supervision of doctoral theses; and the abuse of power over people in lower positions. Personal ambitions and pressure from the evaluation system are regarded as the most influential causes of misconduct proliferation, according to academics at this Spanish university.</p>

                        <p><p>School Psychology International, Ahead of Print. <br>The study employed a qualitative research approach using a phenomenological research design since the lived experiences of educational psychologists in Masvingo Province were scrutinized with the intention of establishing the extent to which educational psychologists matched the laid down specifications pertaining to their professional operations during a high inflationary environment. Three males and two females were purposely selected and in-depth interviews were utilized. Four themes emerged from the study: (a) vital roles, successful, and unsuccessful experiences, (b) diverse views on major policy documents, (c) barriers in rendering support services, and (d) perceived solutions. Results indicated that policy documents are outdated and need revision. Human and material resources were viewed a barriers to performance and perceived solutions included multidisciplinary team work, decentralization and increasing the number of psychologists, training of psychologists and reduced continuous professional development points. Educational psychologists focused more on diagnostic and behavioral consultations in urban schools. They distributed schools in urban areas depending on psychologists’ place of residences. Rural schools were minimally assessed with little provision of intervention, prevention, and health promotion services. The results provide important implications for resource provision and policy formulation.</p>
