Abstract

Major paradigms in sociology and social sciences usually embrace either individualism or anti-individualism as fundamental worldview. This paper explores a third way between individualism and anti-individualism developed by German sociologist Niklas Luhmann in his systems theory. Luhmann treats actors or individuals as psychic systems and he distinguishes them from social systems. In a nutshell, social systems produce communication, whereas psychic systems produce consciousness. In line with anti-individualism, Luhmann therefore argues that social systems are irreducible to psychic systems and their actions. On the other hand, Luhmann joins ranks with individualism to assert that psychic systems are not rigorously constrained by social systems. Ultimately, Luhmann explains that social systems and psychic systems are part of each other’s environment and that each type of systems provides the ecological conditions that the other type depends on to emerge and grow. To discuss this third way between individualism and anti-individualism, the paper examines more specifically two central points in Luhmann’s theory: (1) how social systems and psychic systems are separated from each other, and (2) how social systems and psychic systems are coevolving.

Abstract

Objective

The state of Vermont has a statewide waiver from the centers for medicare and medicaid services to allow all-payer Accountable Care Organizations (ACOs). The Vermont all-payer model (VAPM) waiver is layered upon previous reforms establishing regional community health teams (CHTs) and medical homes. The waiver is intended to incentivize healthcare value and quality and create alignment between health system payers, providers, and CHTs. The objective of this study was to examine CHT’s trade-offs and preferences for health, equity, and spending and the alignment with VAPM priorities.

Data Sources/Study Setting

Data were gathered from a survey and discrete choice experiment among CHT leadership and CHT team members of the 13 CHTs in Vermont.

Study Design

We used conditional logit models to model the choice as a function of its characteristics (attributes) and mixed logit models to analyze whether preferences for programs varied by persons and roles within CHTs.

Data Collection/Extraction Methods

There were 60 respondents who completed the survey online with 14 choice tasks, with three program options in each task, for a total sample size of 2520.

Principal Findings

We found that CHTs prioritized programs in the community health plan and those with quantitative evidence of effectiveness. They were less likely to choose either programs targeting racial and ethnic minorities or programs having a small effect on a large population. Preferences did not vary across individual or community attributes. Program priorities of the VAPM, especially healthcare spending, were not prioritized.

Conclusions

The results suggest that the new VAPM does not automatically create system alignment: CHTs tended to prioritize local needs and voices. The statewide priorities are less important to CHTs, which have excellent internal alignment. This creates potential disconnection between state and community health goals. However, CHTs and the VAPM prioritize similar populations, indicating an opportunity to increase alignment by allowing flexible programs tailored to local needs. CHTs also prioritized programs with a strong evidence base, suggesting another potential avenue to create system alignment.

Abstract

This article presents the consensus of an National Council on Measurement in Education Presidential Task Force on Foundational Competencies in Educational Measurement. Foundational competencies are those that support future development of additional professional and disciplinary competencies. The authors develop a framework for foundational competencies in educational measurement, illustrate how educational measurement programs can help learners develop these competencies, and demonstrate how foundational competencies continue to develop in educational measurement professions. The framework introduces three foundational competency domains: Communication and Collaboration Competencies; Technical, Statistical, and Computational Competencies; and Educational Measurement Competencies. Within the Educational Measurement Competency domain, the authors identify five subdomains: Social, Cultural, Historical, and Political Context; Validity, Validation, and Fairness; Theory and Instrumentation; Precision and Generalization; and Psychometric Modeling.

Abstract

Objectives

Behavioral and psychological symptoms of dementia (BPSD) are highly prevalent in patients with Alzheimer’s disease (AD), causing burdens on caregivers. Behavioral and psychological symptoms of dementia and subclinical epileptiform discharge (SED) increased with the disease course of AD. However, the interaction between them was still unknown. The present study aimed to evaluate the associations between SED and BPSD.

Methods/Design

Patients with AD from Kaohsiung Municipal Ta-tung Hospital were included in this study. International 10–20 system scalp electroencephalography (EEG) for 13 min was performed to detect SED. Behavioral and psychological symptoms of dementia was assessed by neuropsychiatric inventory (NPI) questionnaires. The occurrence of BPSD subsyndromes was compared between patients with and without SED.

Results

Two hundred sixty-three adult patients qualified for the inclusion criteria and were enrolled in this study. The mean age of patients was 80.2 years, and approximately 62% were women. 17.1% of patients showed SED on EEG. Apathy was the most commonly reported BPSD subsyndrome in this cohort. There was no significant difference in the prevalence of BPSD between patients with and without SED. (75.6% vs. 67.4%, p = 0.2806). However, the NPI score of irritability subsyndrome was significantly higher in the SED (+) group (2.6 ± 3.7 vs. 1.2 ± 2.7, p = 0.0028). In addition, subclinical epileptiform discharge in the frontal lobe was associated with a considerably higher occurrence of hyperactivity subsyndrome, including irritability.

Conclusions

SED may not be a direct cause of BPSD, but the presence of SED may affect the manifestation of BPSD.

Abstract

Job satisfaction has long been discussed as an important factor determining individual behavior at work. To what extent this relationship is also evident in the teaching profession is especially relevant given the manifold job tasks and tremendous responsibility teachers bear for the development of their students. From a theoretical perspective, teachers’ job satisfaction should be negatively related to turnover intentions and absenteeism, and positively to high-quality teacher-student interactions (i.e., emotional support, classroom management, and instructional support), enhanced student motivation, and achievement. This research synthesis provides a comprehensive overview of the relationship between teachers’ job satisfaction and these variables. A systematic literature search yielded 105 records. Random-effects meta-analyses supported the theoretically postulated relationships between teachers’ job satisfaction and their turnover intentions, absenteeism, teacher-student interactions, and students’ outcomes. Effects were significant not only for teachers’ self-reports of their professional performance, but also for external reports. On the basis of the research synthesis, we discuss theoretical, conceptual, and methodological considerations that inform future research and prospective intervention approaches.

Abstract

This paper introduces two methods to derive internationally comparable skills and occupational distance based on machine learning and natural language processing techniques. We apply these measures to produce descriptive facts about employment transitions and workers’ wage distribution in Brazil using the universe of formal labour contracts covering the period from 2003 to 2018. Our findings show that workers who use non-routine cognitive skills intensively are better off in terms of employment, wages, and occupational switching. Overall, we observe signs of a routine-biased technological change and employment polarisation, however, this trend has only emerged since the Brazilian economic crisis of 2014.
