<p>As the COVID-19 pandemic took hold of the United States, pregnant women and mothers of infants were among those most affected by fears of the virus, the disruption of health care systems, social isolation, and the collapse of social supports. This article describes how a group of psychoanalysts and psychoanalytic therapists in a community outreach initiative, the SPRING Project, worked to meet these mental health needs by providing affordable psychotherapy at this critical time. Two case reports highlight the ways the pandemic magnified postpartum distress as well as the relevance and efficacy of a psychodynamic clinical approach in resolving such distress. In addition, the author chronicles how early in the pandemic the SPRING Project launched support groups for pregnant women. Participants in this virtual village helped offset the painful isolation of these women and supported their emotional development during their pregnancies. <p>The rapidly expanding self-compassion research is driven mainly by Neff’s (2003a, 2003b, 2023) six-factor Self-Compassion Scale (SCS). Despite broad agreement on its six-first-order factor structure, there is much debate on SCS’s global structure (one- vs. two-global factors). Neff et al. (2019) argue for an exploratory structural equation model (ESEM) with six specific and one global bifactor (6ESEM + 1GlbBF) rather than two global factors (6ESEM + 2GlbBF). However, ESEM’s methodological limitations precluded testing the appropriate 6ESEM + 2GlbBF, relying instead on a model combining ESEM and traditional confirmatory factor analysis (6ESEM + 2CFA). Although intuitively reasonable, this alternative model results in internally inconsistent, illogical interpretations. Instead, we apply recent advances in Bayesian SEM frameworks and Bayes structural equation models fit indices to test a more appropriate bifactor model with two global factors. This model (as does 6CFA + 2GlbBF) fits the data well, and correlations between compassionate self-responding (CS) and reverse-scored uncompassionate self-responding (RUS) factors (∼.6) are much less than the 1.0 correlation implied by a single bipolar factor. We discuss the critical implications for theory, scoring, and clinical application for the SCS that previously were inappropriately based on this now-discredited 6ESEM + 2GlbCFA. In applied practice, we endorse using scores representing the six SCS factors, total SCS, and CS and RUS components rather than relying solely on one global factor. Our approach to these issues (dimensionality, factor structure, first-order and higher order models, positive vs. negatively oriented constructs, item-wording effects, and alternative estimation procedures) has wide applicability to clinical measurement (see our annotated bibliography of 20 instruments that might benefit from our approach). <h2>Abstract</h2>
<p>Missing data complicates statistical analyses in multi-site studies, especially when it is not feasible to centrally pool individual-level data across sites. We combined meta-analysis with within-site multiple imputation for one-step estimation of the average causal effect (ACE) of a target population comprised of all individuals from all data-contributing sites within a multi-site distributed data network, without the need for sharing individual-level data to handle missing data. We considered two orders of combination and three choices of weights for meta-analysis, resulting in six approaches. The first three approaches, denoted as RR + metaF, RR + metaR and RR + std, first combined results from imputed data sets within each site using Rubin’s rules and then meta-analyzed the combined results across sites using fixed-effect, random-effects and sample-standardization weights, respectively. The last three approaches, denoted as metaF + RR, metaR + RR and std + RR, first meta-analyzed results across sites separately for each imputation and then combined the meta-analysis results using Rubin’s rules. Simulation results confirmed very good performance of RR + std and std + RR under various missing completely at random and missing at random settings. A direct application of the inverse-variance weighted meta-analysis based on site-specific ACEs can lead to biased results for the targeted network-wide ACE in the presence of treatment effect heterogeneity by site, demonstrating the need to clearly specify the target population and estimand and properly account for potential site heterogeneity in meta-analyses seeking to draw causal interpretations. An illustration using a large administrative claims database is presented.</p>

<p>The use of cannabis for medical symptoms is increasing despite limited evidence for its efficacy. Expectancies—prior beliefs about a substance or medicine—can modulate use patterns and effects of medicines on target symptoms. To our knowledge, cannabis expectancies have not been studied for their predictive value for symptom relief. The 21-item Cannabis Effects Expectancy Questionnaire–Medical (CEEQ-M) is the first longitudinally validated measure of expectancies for cannabis used for medical symptoms. The questionnaire was developed for a randomized clinical trial of the effect of state cannabis registration (SCR) card ownership on symptoms of pain, insomnia, anxiety, and depression in adults (<em>N</em> = 269 across six questionnaire administrations). Item-level analyses (<em>n</em> = 188) demonstrated between-person stability of expectancies and no aggregate, within-person expectancy changes 3 months after individuals gained access to SCR cards. Exploratory factor analysis (<em>n</em> = 269) indicated a two-factor structure. Confirmatory factor analysis at a later timepoint (<em>n</em> = 193) demonstrated good fit and scalar invariance of the measurement model. Cross-lagged panel models across 3 and 12 months (<em>n</em> = 187 and 161, respectively) indicated that CEEQ-M-measured expectancies did not predict changes in self-reported cannabis use; symptoms of pain, insomnia, anxiety, and depression; and well-being. However, greater baseline cannabis use predicted more positive expectancy changes. The findings suggest that the CEEQ-M is psychometrically sound. Future work should clarify at what timescales cannabis expectancies have predictive value and how cannabis expectancies for medical symptoms are maintained and diverge from other substance use expectancies. <p>Up to 19% of postpartum mothers experience depressive symptoms, which are associated with infant development. Thus, research examining postpartum depression has implications for mothers’ and infants’ well-being. However, this research relies on the often-untested assumption of measurement invariance—that measures capture the same construct across time and sociodemographic characteristics. In the absence of invariance, measurement bias may confound differences across time and group, contributing to invalid inferences. In a sociodemographically diverse (40.7% African American, 58.9% White; 67.9% below two times the federal poverty line; 19.4% with less than high school education), rural, longitudinal sample (<em>N</em> = 1,275) of mothers, we used moderated nonlinear factor analysis (MNLFA) to examine measurement invariance of the Brief Symptom Inventory–18 (BSI-18) Depressive Symptoms subscale across time since birth, racial group, education, income, primiparity, and maternal age at childbirth. We identified evidence of differential item functioning (DIF; i.e., measurement noninvariance) as a function of racial group and education. Subsequent analyses indicated, however, that the DIF-induced bias had minimal impacts on substantive comparisons examining change over time since birth and group differences. Thus, the presence of measurement noninvariance does not appear to bias substantive comparisons using the BSI-18 Depressive Symptoms subscale across the first 2 years since birth in a sample comprising primarily African American and White mothers living in predominately rural, low-income communities. This study demonstrates the importance of assessing measurement invariance and highlights MNLFA for evaluating the impact of noninvariance as a preliminary step that increases confidence in the validity of substantive inferences. <p>Impulsivity and excitement seeking are distinctly associated with alcohol use and related problems in emerging adulthood. Specifically, impulsivity is associated with alcohol problems, while excitement seeking is associated with quantity and frequency of alcohol use (Finn, 2002; Gunn et al., 2013). Given how critical these traits are to alcohol use disorder, such that they play central roles in prominent theories such as Finn’s (2002) cognitive motivational theory (CMT), we examined how impulsivity, excitement seeking, and related constructs were captured by an instrument used in clinical practice, the Minnesota Multiphasic Personality Inventory–3 (MMPI-3; Ben-Porath & Tellegen, 2020a, 2020b). In a sample of 401 college students, we investigated MMPI-3 scales representing the CMT constructs—impulsivity/novelty seeking, excitement seeking, and low harm avoidance—by examining correlations between MMPI-3 Specific Problem scales and theoretically relevant criterion measures. Then, we used these scales to predict alcohol-related outcomes. Impulsivity (IMP), Social Avoidance (SAV), and Anxiety-Related Experiences (ARX) scales captured the core CMT constructs that are said to reflect personality-based vulnerabilities to alcohol-related problems. Further, Impulsivity and Social Avoidance scales incremented the Substance Abuse (SUB) scale in predicting risky drinking outcomes (alcohol problems and quantity and frequency of use). Overall, results support the use of select MMPI-3 scales in providing a nuanced assessment of personality-based vulnerabilities in college-aged drinkers. <h2>Abstract</h2>
<p>Child neglect has devastating enduring consequences for children and its identification and assessment remains challenging for practitioners. In England, assessment tools and standardised measures have been incorporated in welfare and safeguarding practice to help practitioners’ critical observation and analysis and improve their assessments of risk. However, the picture regarding child neglect assessment practice trends on the ground remains unknown. This mapping exercise aimed to, firstly, provide an overview of the approaches to neglect practice and neglect assessment tools promoted by Local Safeguarding Children Partnerships (LSCPs) across England, and secondly, provide a brief examination of the types of tools advocated. An email survey was sent to the safeguarding leads or chairs of LSCPs in England (<i>n</i> = 121). Forty-two valid responses were received (34.7%). Almost all participating LSCPs had a Neglect Strategy in place and recommended the use of tools and chronologies when working with neglect. Eighteen individual neglect tools were reported with only two of these having undergone psychometric testing. The findings affirm that the use of tools or instruments with low or no testing for psychometric properties continues to be widespread in children’s social care and welfare practice in England. Implications and recommendations for practice, policy and future research are discussed.</p>
